We present the case of a 39-year-old man with recurrence of intermittent claudication 4 years after successful Dacron patch repair of the right common femoral artery for cystic adventitial disease. Magnetic resonance angiography results were inconclusive, while conventional angiography demonstrated 90% stenosis of the right common femoral artery. The patient underwent excision and replacement of the affected artery with PTFE and was asymptomatic at 6-month follow-up. Urethral diverticulum secondary to incomplete surgical repair represents an even rarer complication. We report the case of a 7-year-old boy presenting with post-void dribbling following partial resection of a supernumerary urethra for Effman type IIA2 urethral duplication. Surgical excision of left atrial myxomas is usually curative. When the root of the pedicle and the full thickness of the adjacent interatrial septum are excised, the repair of the created atrial septal defect requires a pericardial or Dacron patch. The biatrial approach generally has been accepted as the technique having the advantages of well identifying the site of attachment and inspection of the four cardiac chambers. We proposed a modification of this technique that allows the reconstruction of the created septal defect without any foreign patch.

A new surgical technique is described for the repair of salivary fistulae. It has been used in three patients. The fistulae occurred after laryngectomy and had been unsuccessfully treated with local and regional flaps. The principle of the technique is to use a bipedicled jejunal patch as a free microvascular transfer. One patch is used for reconstruction of the pharynx and after excising the mucosa the skin defect is closed with the other patch. The mucosa is excised to expose the muscle and to create a recipient site for a split skin graft.

TFIIH is a basal transcription factor for protein-coding genes. It contains ERCC2, ERCC3, MO15 and cyclin H, polypeptides implicated in nucleotide excision repair or cell cycle regulation. The dysfunction of TFIIH could result in a large panel of genetic disorders, such as xeroderma pigmentosum, Cockayne's syndrome and trichothiodystrophy. This link between transcription, DNA repair and cell cycle has highlighted a complex and essential role for TFIIH in the cell and has provided much information on the molecular mechanisms of each of these cellular processes.

We report clinical, genetic and biochemical findings in 13 families with the photosensitive genodermatosis, xeroderma pigmentosum. All patients had a defect in repair of DNA damage provoked by ultraviolet radiation. Heterotopic gastric mucosa in the rectum produced a rare complication of rectovesical fistula. Diagnosis was made by direct biopsy and 99mTc scanning. Excision of ectopic gastric mucosa, diverting colostomy, and repair did not result in closure of the fistula. Endorectal pull-through constituted definitive treatment.
