Atlanto-axial rotary instability (Fielding type 1): characteristic clinical and radiological findings, and treatment outcomes following alignment, fusion, and stabilization.

Atlanto-axial instability (AAI) is common in the connective tissue disorders, such as rheumatoid arthritis, and increasingly recognized in t … ness (all P < 0.01), vertigo and arm weakness (both P = 0.01), and syncope, nausea, joint pain, and exercise tolerance (all P < 0.05). The diagnosis of Fielding type 1 AAI requires directed investigation with dynamic imaging. Alignment and stabilization is associated with improvement of pain, syncopal and near-syncopal episodes, sensorimotor function, and exercise t …

Physiologic effects of bowel preparation.

PURPOSE: Despite the universal use of bowel preparation before colonoscopy and colorectal surgery, the physiologic effects have not been described in a standardized setting. This study was designed to investigate the physiologic effects of bowel preparation. METHODS: In a prospective study, 12 healthy volunteers (median age, 63 years) underwent bowel preparation with bisacodyl and sodium phosphate. Fluid and food intake were standardized according to weight, providing adequate calorie and oral fluid intake. Before and after bowel preparation, weight, exercise capacity, orthostatic tolerance,

Prediction of heat tolerance from heart rate and rectal temperature in a temperate environment.

To determine if heat tolerance could be predicted from responses to exercise in temperature conditions, 51 young men performed 15 min of bench stepping at an average work load of 80 W at 23 degrees C. On the following day they attempted to perform 3 h of bench stepping at 40 W in heat (39.3 degrees C dry bulb, 30.3 degrees C wet bulb). Of these subjects, 4 were heat intolerant, judged by previous heat stroke episodes during field marches, 12 were heat acclimated, and 35 were unacclimated. The heat-intolerant subjects showed the highest heart rates (HR) and rectal temperatures (Tre) at 23 degre

Exercise alveolar oxygen extraction rate reflects ventilatory efficiency and predicts outcomes in idiopathic pulmonary fibrosis.

BACKGROUND: Oxygen uptake (V'O2) obtained from expiratory gas analysis is generally calculated using minute ventilation (V'E) and the inspired‒expired mean oxygen (O2) concentration difference (ΔFO2) during cardiopulmonary exercise testing (CPET). We have reported that ΔFO2, which is associated with ventilatory efficiency, is independent of V'E at peak exercise and affects exercise tolerance in respiratory diseases other than idiopathic pulmonary fibrosis (IPF). We hypothesized that similar results are obtained in IPF, and that ΔFO2 is a prognostic factor for survival in IPF. METHODS: Forty-t…

Dynamic laryngeal narrowing during exercise: a mechanism for generating intrinsic PEEP in COPD?

INTRODUCTION: Patients with COPD commonly exhibit pursed-lip breathing during exercise, a strategy that, by increasing intrinsic positive end-expiratory pressure, may optimise lung mechanics and exercise tolerance. A similar role for laryngeal narrowing in modulating exercise airways resistance and the respiratory cycle volume-time course is postulated, yet remains unstudied in COPD. The aim of this study was to assess the characteristics of laryngeal narrowing and its role in exercise intolerance and dynamic hyperinflation in COPD. METHODS: We studied 19 patients (n=8 mild-moderate; n=11 sev…

Exercise capacity in patients with obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome (OSAS) is a common disease characterized by repetitive partial or complete closure of the upper airway duri … etween hemodynamic responses to exercise and sleep apnea severity were also noted. We conclude that exercise tolerance can be limited due to hypertensive response in about 20% of patients. Patients with severe OSAS have exaggerated hemodynamic response to exercise and delayed post-exercise blood pressure recovery. Cardiopulmonary response to exercise seems to be rel …

Does dynamic hyperinflation contribute to dyspnoea during exercise in patients with COPD?

Dynamic hyperinflation (DH) during exercise occurs in most but not all patients with advanced chronic obstructive pulmonary disease (COPD). It is not known whether the presence or absence of DH has implications for dyspnoea and exercise tolerance. Therefore, we compared detailed ventilatory and sensory responses to exercise in hyperinflators and nonhyperinflators with moderate-to-severe COPD. Nonhyperinflators (n=65) were retrospectively identified from a sample of 427 patients and case-matched to a group of hyperinflators (n=65) based on sex, age, body mass index and % predicted forced expira…

Exercise-induced anaphylaxis and urticaria.

Physical exercise is a stimulus capable of provoking urticaria and anaphylaxis in certain individuals. The cutaneous manifestations of EIA include erythema, pruritus, and urticarial whealing. Symptoms may also progress to angioedema, laryngeal edema, bronchospasm, and hypotension. Attacks are consistently associated with increases in serum histamine levels, and atopic individuals are more commonly affected. At least two distinct diseases cause EIA, including CU and classic EIA. A variant form of EIA may also exist. CU episodes are induced by increases in body temperature occurring secondary to

Influence of Fibroblast Growth Factor 21 on Delayed Glycemic Improvement Following Acute Exercise in Type 2 Diabetes Mellitus.

BACKGRUOUND: Exercise positively influences glycemic control. Some individuals experience greater glycemic stability on the day after exercise, even without additional physical activity. However, the mechanisms underlying this delayed glycemic improvement remain unclear. METHODS: Seventy-one patients with type 2 diabetes mellitus were assigned to either a 60-minute exercise group or a resting group. Serum fibroblast growth factor 21 (FGF21) levels and untargeted metabolomic profiles were assessed at multiple time points before and after exercise. Interstitial glucose levels were monitored usi

Clinical significance of exercise-induced hypoalgesia in individuals with temporomandibular disorders and neck pain: A clinical trial protocol.

Evidence reports positive effects of neck motor control and aerobic exercises (AEs) to improve pain in individuals with temporomandibular disorders (TMD) and neck pain. A single bout of exercise typically leads to an increase in pain thresholds up to 30 min post-exercise, known as exercise-induced hypoalgesia (EIH). Studies evaluating the effects of aerobic and neck motor control exercises on EIH in individuals with chronic neck pain and TMD are limited. Measuring treatment effects and determining the clinical significance based on exercise types and loads and EIH response can improve clinical

Can an echocardiographic score predict who will benefit clinically from balloon dilation of the mitral valve?

Deciding whether a patient with sub-optimal mitral valve anatomy will benefit from percutaneous mitral valvotomy remains a demanding clinical problem. We assessed the ability of an established echo score applied to transoesophageal images to predict absolute increases in mitral valve area and improvement in exercise capacity. Twenty five consecutive patients undergoing routine percutaneous mitral valvotomy were studied. Changes in exercise tolerance were measured by serial cardiorespiratory treadmill exercise testing. Before the procedure, exercise duration was directly related to mitral valve…

The role of exercise intensity in motor and non-motor symptoms of Parkinson's disease: mechanisms, clinical evidence, and personalized exercise prescription parameters.

Parkinson's disease (PD) is the second most common neurodegenerative disorder, featuring progressive dopaminergic neuronal loss, Lewy body (LB) pathology, and a broad spectrum of motor and non-motor symptoms. Although dopaminergic pharmacotherapy remains the cornerstone of symptomatic treatment, long-term medication use is frequently complicated by motor fluctuations, dyskinesia, and limited efficacy for several non-motor symptoms. Exercise has therefore gained increasing attention as a non-pharmacological intervention for PD. This narrative review summarizes current evidence on how exercise i

Aerobic exercise attenuates high-fat diet-induced glycometabolism impairments in skeletal muscle of rat: role of EGR-1/PTP1B signaling pathway.

OBJECTIVE: Impaired skeletal muscle glycogen synthesis contributes to insulin resistance (IR). Aerobic exercise reported to ameliorate IR by augmenting insulin signaling, however the detailed mechanism behind this improvement remains unclear. This study investigated whether aerobic exercise enhances glycogen anabolism and insulin sensitivity via EGR-1/PTP1B signaling pathway in skeletal muscle of rats. METHODS: Sprague-Dawley rats fed a high-fat diet (HFD), and performed treadmill exercise training for 6-week. Oral glucose tolerance test was conducted to confirm the IR. Periodic Acid-Schiff (
