Neutropenia, fever, and infection.

With the advances in the management of various neoplastic diseases and subsequent improvement in "disease-free" states, complications of therapy--particularly, infectious complications--have evolved as stumbling blocks to survival. Among neutropenic (absolute neutrophil count below 1,000/mm3) patients with cancer, infection is the major autopsy-determined cause of death. With expected "cure rates" of childhood leukemia approaching 60 to 70 percent, it seems unreasonable to lose such patients to an infectious cause of death, yet this, indeed, happens. The purpose of this review is to (1) define

35 cases of botonneuse Fever admitted in our hospital during the course of a year and a half are presented. They were all children between the age of 18 months and 11 years. The most common features were: fever, maculopapulous rush and "tache noir". Weil Felix test was not of diagnostic value in one third of the cases. 71 per cent of the children recieved etiological treatment (doxicicline or cloramfenicol); the remaining 29% only got simptomatic measures. A cure was achieved in every case. Nevertheless, the manifestatios lasted longer in those not treated with antibiotic therapy.

[Boutonneuse fever and tourism].

In the last few years some cases of boutonneuse fever have been "imported" into Central and Northern European countries, where it was previously unknown, by tourists returning from African or Mediterranean countries where the disease is endemic. Data on the subject are summarised and the possibility of ticks carrying the disease being transported into disease-free countries is analysed. The results of a clinicoepidemiological survey on boutonneuse fever contracted by foreigners in areas where the disease is actively endemic are reported. It would appear the few cases were involved and that the…

Hemostasis in typhoid fever.

Abnormalities of hemostasis in vivo and in vitro were defined in a prospective study of patients with typhoid fever. In a group of 56 patients with a proven diagnosis of typhoid fever, hemostatic abnormalities were frequently found. In patients with normal clotting tests the fibrinogen level was found to be higher than in normal controls. This finding established a new normal level for patients with typhoid fever. Using this redefined normal level it was found that low fibrinogen was associated with other hemostatic abnormalities. Low fibrinogen levels in patients with typhoid fever are possib…

Rift Valley fever retinitis.

A 57-year-old Dutch woman, who had lived and worked in Tanzania for 26 years, developed serious, progressive bilateral retinitis after suffering from a fever of unknown origin. Fundus abnormalities in both eyes suggested that the woman had been infected with Rift Valley fever virus. Serologic studies confirmed this suspicion. White macular lesions accompanied to extensive vasculitis appear to be specific for Rift Valley fever retinitis.
