Finite element analysis confirms the optimal apex position in medial opening wedge high tibial osteotomy to avoid lateral hinge fracture.

PURPOSE: Lateral hinge fracture is a significant complication of medial opening wedge high tibial osteotomy. While fracture risk is closely associated with the osteotomy apex position, the optimum position remains variable within the literature. Our hypothesis is that stresses at the osteotomy apex predicted by finite element analysis can be used to identify an apex position which minimises intra and postoperative fracture risks. METHODS: A finite element model was studied to investigate the effect of varying the hinge position on fracture risk and severity for a given bone geometry; variable

Simulation-based prediction of bone healing and treatment recommendations for lower leg fractures: Effects of motion, weight-bearing and fibular mechanics.

Despite recent experimental and clinical progress in the treatment of tibial and fibular fractures, in clinical practice rates of delayed bone healing and non-union remain high. The aim of this study was to simulate and compare different mechanical conditions after lower leg fractures to assess the effects of postoperative motion, weight-bearing restrictions and fibular mechanics on the strain distribution and the clinical course. Based on the computed tomography (CT) data set of a real clinical case with a distal diaphyseal tibial fracture, a proximal and a distal fibular fracture, finite ele

Severe osteoporosis: diagnosis of non-hip non-vertebral (NHNV) fractures.

Osteoporotic bone, structurally altered because of reduction of bone mineral density and quality deterioration, can easily head for fracture after minimum mechanical stress.THE MOST COMMON SITES OF FRACTURE, OTHER THAN SPINE AND HIP, ARE, IN DECREASING ORDER: distal radius, forearm, proximal humerus, other femoral sites, ribs, pelvis, tibia and fibula, metatarsal bone and calcaneum. The role of diagnostic imaging is essential in detecting fractures for their immediate and correct assessment, which is necessary to the planning of treatment, whether conservative or surgical. Imaging is also impo

Economic impact of orthopaedic care for non-fatal gunshot wounds: analysis of a public health crisis.

BACKGROUND: The purpose of this study was to perform an epidemiological evaluation and an economic analysis of 90-day costs associated with … fracture management of the spine $431,021.33, followed by the pelvis $392,658.45 and later by tibia/fibula fractures $342,316.92. CONCLUSIONS: The 90-day direct charges and reimbursements of orthopedic care for non-fatal GSWs are of significant amounts per patient. While the number of fatal GSWs has received much attention, non-fatal GSWs have a large economic an …

Outcome of Danis-Weber Type-B ankle fracture treated by Pre-contoured Distal Fibular Locking Plate.

Danis-Weber type-B ankle fracture is one of the most common injuries in young and active individual. This fracture offers a considerable challenge to orthopedic surgeon. Though there are several options for treating such type of fracture, ORIF by Pre-contoured Distal Fibular Locking Plate is the preferred option in the recent years. This descriptive type of observational study was performed from July 2017 to June 2019 in NITOR. Thirty (30) patients, 22 male and 8 female with an average age of 39 years with Danis-Weber type-B ankle fracture underwent ORIF by Pre-contoured Distal Fibular Locking

Below-Knee Amputation Resulting from an Arterial Thrombosis from Tourniquet Use in a Patient with Undiagnosed Factor V Leiden: A Case Report.

CASE: A 38-year-old woman presented with previously undiagnosed factor V Leiden (FVL), who suffered a complete superficial femoral arterial thrombosis after tourniquet use during the surgical repair of one of her bilateral tibial plafond fractures. This patient's injury eventually resulted in a below-knee amputation. CONCLUSION: We recommend expanding hypercoagulable screening on patients with risk factors based on a detailed history and physical examination. We also recommend limiting or negating tourniquet use in patients with FVL or other hypercoagulable disorders.

Pedicled vascularised fibular grafting in a flow-through manner for reconstruction of infected non-union of the tibia with preservation of the peroneal artery: a case report.

We report on an 82-year-old man with an infected non-union of the right tibia in which the peroneal artery was the sole artery supplying the lower leg, owing to peripheral arterial disease. He underwent tibial reconstruction using the pedicled vascularised fibular graft in a flow-through manner (without ligation of either the proximal or distal ends of the peroneal artery) and achieved successful bony union.

Free vascularized bone transfer in limb salvage surgery of the upper extremity.

Salvage of the upper extremity following tumor resection may require reconstruction of massive bone defects. This more commonly results from malignant bone tumors, but defects requiring reconstruction also may occur following resection of locally aggressive benign tumors. Examples include osteosarcoma, chondrosarcoma, Ewing sarcoma, giant cell tumor, and certain soft tissue sarcomas invading bone. Obtaining satisfactory results using conventional bone grafting techniques is at best challenging when defects of substantial size are encountered. Free vascularized bone grafting procedures provide

The point of view of the clinician: a prospective study of the mechanism of accidents and the morphology of tibial and fibular shaft fractures.

In a prospective study of 210 tibial shaft fractures, the accident mechanisms and the resultant fracture morphologies were analyzed. 86 fractures occurred due to indirect impact. The fracture morphology in this group consisted of short and long spiral fractures resulting from rotational injuries complicated by anterior torsion butterfly fragments if the person fell forward, posterior torsion butterfly fragments if they fell backwards and complicated by multiple torsion butterfly fragments if it was a high velocity injury. 124 fractures occurred due to direct impact. The fracture morphology in

Treatment of tibial defects and nonunions using ipsilateral vascularized fibular transposition.

Eight patients had nine ipsilateral vascularized fibular transpositions (IVFTs) in the period 1978-1987. The procedure, which involves no microvascular anastomosis, was done for difficult problems of the tibia including two long-standing posttraumatic nonunions, three allograft nonunions after tumor surgery, and four segmental tibial defects also after tumor surgery. The average duration of problems before surgery was 3.5 years, and the patients averaged 3.1 procedures before IVFT. The patients were followed for an average of 52.4 months. The average time to union was 4.2 months, and in all pa

Fractures of the posterior tibial margin: their role in the prognosis of malleolar fractures.

One hundred forty-two patients with malleolar fractures were followed up for an average of 5.7 years postoperatively. In 80 cases (56.4%) we found no lesion of the distal articular surface of the tibia; 62 patients (43.6%) had a fracture of the posterior tibial margin. Both groups were closely matched with regard to fractures of the lateral and medial malleolus. The long-term results were significantly poorer when a fracture of the posterior tibial margin was additionally present, even in cases with a small marginal fragment of the lip of the tibia. Further, we found a clear relationship betwe

Bone remodeling as an expression of altered phenotype: studies of fracture healing in untreated and cured osteopetrotic rats.

The healing of tibial fractures was studied in 3 groups of young littermate rats; untreated osteopetrotic (ia/ia), normal littermates (ia/+) and osteopetrotic (ia/ia) cured of the disease 3 days earlier by transplanted normal spleen cells. The fracture site was completely remodeled in 4 weeks in normal rats, 6 weeks in transplanted (cured) osteopetrotic rats and 9 weeks in untreated osteopetrotic rats. The delayed healing in untreated osteopetrotic rats resulted from reduced resorption present in this mutation. Increased resorption brought about by transplantation of normal spleen cells accele

[Analysis of complications in the treatment of fracture leg fractures with unreamed intramedullary nailing].

Based on their group of 52 patients the authors analyze the complications they encountered in the treatment of crural fractures by a not predrilled intramedullary nail. Consistent with the literature they found that this method is successful in the treatment of closed and open fractures of the crural diaphysis. They mention, however, a relatively high percentage of complications which in the long run are not a impediment for healing of the fracture but influence it. In the discussion these complications are analyzed in detail in an attempt to find a solution and to avoid them in future patient

Do Emergency Medicine Residency Graduates Feel Prepared to Manage Closed Fractures After Training?

OBJECTIVES: Fractures comprise 3% of all emergency department (ED) visits. Although emergency physicians are often responsible for managing most of the initial care of these patients, many report a lack of proficiency and comfort with these skills. The primary objective was to assess how prepared recent emergency medicine (EM) residency graduates felt managing closed fractures upon completion of residency. Secondary objectives included whether residency training or independent practice contributed most to the current level of comfort with these procedures and which fractures were most commonly

Evidence-based protocol for prophylactic antibiotics in open fractures: improved antibiotic stewardship with no increase in infection rates.

BACKGROUND: Evidence-based guidelines for prophylactic antibiotic use in open fractures recommend short-course, narrow-spectrum antibiotics for Gustilo Grade I or II open fractures and broader gram-negative coverage for Grade III open fractures. No studies to date have assessed the impact of these guidelines on infection rates in open fractures. Infection rates before and after the new protocol implementation were examined. METHODS: A new protocol was implemented including antibiotic prophylaxis based on grade of open fracture: Grade I/II fractures, cefazolin (clindamycin if allergy); Grade I
