Atrial Fibrillation in Patients with Congenital Heart Disease.

Advances in surgical techniques have led to the survival of most patients with congenital heart disease (CHD) up to their adulthood. During their lifetime, many of them develop atrial tachyarrhythmias due to atrial dilatation and scarring from surgical procedures. More complex defects and palliative repairs are linked to a higher incidence and earlier occurrence of arrhythmias. Atrial fibrillation (AF) is common in patients who have atrial septal defects repaired after age 55 and in patients with tetralogy of Fallot repaired after age 45. Patients with dextrotransposition of the great arteries

Occlusion of Sano conduit with the Amplatzer vascular plug: a reliable method for staged elimination of accessory pulmonary blood flow in single ventricle palliation.

BACKGROUND: Staged palliative surgery is often required to regulate pulmonary and systemic blood flow in children with complex congenital heart disease. Accessory sources of pulmonary blood flow have been utilized to provide satisfactory pulmonary blood flow after Glenn anastomosis in select patients. Prolonged exposure to increased flow can lead to elevated pulmonary arterial pressures and volume loading of the ventricle, potentially limiting candidacy for Fontan completion. OBJECTIVE: To review the feasibility, technique, and outcome of transcatheter closure of the Sano conduit utilizing th

Surgery for arrhythmias in children.

BACKGROUND: Transcatheter radiofrequency ablation to treat supraventricular and ventricular arrhythmias has supplanted routine surgical ablative therapy and redefined its role. A small population of arrhythmia patients now requires surgical ablation: those who have failed catheter ablation, patients with concomitant congenital heart disease in association with arrhythmias, those with atrial fibrillation and very young patients for whom transcatheter techniques are prohibitive because of small size, cyanosis or distorted anatomy. METHODS: From July 1992 through August 2003, 133 patients underw

Impact of Atretic Aortic Valve on Systemic Ventricular Function After the Norwood Procedure.

BACKGROUND: Aortic atresia (AA) is considered a risk for the Norwood procedure. This study aimed to compare the longitudinal ventricular function (VF) and atrioventricular valve (AVV) regurgitation in patients with AA and aortic stenosis (AS). METHODS: Using serial echocardiographic images in patients undergoing the neonatal Norwood procedure between 2001 and 2020, systemic VF assessed by ejection fraction and the degree of AVV regurgitation were compared between the patients with AA and AS. RESULTS: A total of 335 consecutive patients were included, consisting of 273 with hypoplastic left h

Retrospective study of results of Kawashima procedure.

BACKGROUND: Patients with single ventricle physiology and an interrupted inferior caval vein undergo the Kawashima procedure. With increasing follow-up, pulmonary arteriovenous malformations may develop requiring hepatocardiac venous redirection. METHODS: Between January 2005 and December 2012, 15 patients underwent the Kawashima procedure. Preoperative and postoperative characteristics along with functional class, intra-operative details, post-operative course, saturations and haematocrit were recorded. RESULTS: Median age at operation was five years (range 1- 20 years). Five patients had a

A reconsideration of risk factors for the Fontan operation.

We reviewed our experience in 38 patients who underwent a Fontan operation. In the first five patients ages 7.5 to 23 years (mean, 15 years), a conduit was placed from the right atrium to the small right ventricle or the pulmonary artery (PA). The remaining 33 patients, ages 7 months to 14 years (mean, 4.8 years), had a modified Fontan operation with direct systemic venous or right atrial to PA anastomosis. The diagnoses were tricuspid atresia (n = 14), single ventricle (n = 10), hypoplastic right or left ventricle (n = 9), double-outlet right ventricle with inlet ventricular septal defect and

Decellularized vs Non-decellularized Allogeneic Pulmonary Artery Patches for Pulmonary Arterioplasty.

We studied pulmonary artery size, reinterventions, and panel reactive antibodies in patients with single-ventricle physiology who underwent a pulmonary arterioplasty with decellularized (DAPAP) and non-decellularized allogeneic pulmonary artery patches (non-DAPAP). Retrospective review identified 59 patients with single-ventricle physiology who underwent pulmonary arterioplasty from 2008 to 2017: 28 patients underwent arterioplasty with DAPAP and 31 patients with non-DAPAP. Demographic and operative variables were similar between groups. Among patients who underwent a Norwood procedure, a righ

Differences in cerebral and muscle oxygenation patterns during exercise in children with univentricular heart after Fontan operation compared to healthy peers.

BACKGROUND: We assess whether the lower exercise tolerance in children with univentricular heart (UVH) after Fontan operation is associated … ar TOI and lower amplitude cerebral TOI correlated with higher VO2peak. CONCLUSION: Children after Fontan procedure have different oxygenation mechanisms at muscular and cerebral level. This reflects a different balance between O2 supply to O2 demand which might contribute to the reduced exercise tolerance in this patient population.

Functional assessment of the Fontan operation: combined M-mode, two-dimensional and Doppler echocardiographic studies.

Combined M-mode, two-dimensional and Doppler echocardiographic studies were used to assess the postoperative status of 33 patients who had undergone the modified Fontan procedure. Twenty-four patients had surgical repair with use of a simple direct right atrium to pulmonary artery anastomosis. The remaining patients had repair with use of a prosthesis or associated Glenn shunt. Twenty-seven patients were studied early in the postoperative period (2 months or less) and the remaining patients were studied up to 6 years postoperatively. A total of 36 examinations were performed. Of the 33 patient…

Hypokalemia-Induced Rhabdomyolysis Following Prolonged Diarrhea in a Child With Congenital Heart Disease Receiving Loop Diuretics: A Case Report and Literature Review.

Hypokalemia is a rare but important cause of rhabdomyolysis in children. Unlike typical rhabdomyolysis, hyperkalemia may be absent because the triggering factor is hypokalemia, potentially delaying diagnosis. We report a five-year-old boy with hypoplastic left heart syndrome after a Fontan procedure who developed severe hypokalemia and rhabdomyolysis following prolonged diarrhea while receiving loop diuretics. He presented with acute lower extremity weakness and inability to stand. Laboratory evaluation revealed potassium 1.9 mmol/L and creatine kinase 9,013 U/L, peaking at 32,908 U/L. Urinaly…

Fontan-associated liver disease in children following surgical correction of congenital heart defects.

The Fontan procedure is a palliative surgical strategy used in the management of severe congenital heart defects, resulting in the creation of a single-ventricle circulation. In this physiological configuration, venous blood from the systemic circulation flows directly into the pulmonary arteries, bypassing the heart muscle. The operation is typically performed in early childhood and significantly improves long-term prognosis, enabling survival into the third decade of life and beyond. Over time, however, children and young adults develop complications related to the altered circulatory system…

An Expanding Perspective: Extreme Aortic Dilation in a Patient With Single Ventricle Palliation.

Aortic dilation following single ventricle palliation is increasingly recognized, although its long-term clinical implications remain unclear. We present the case of a 38-year-old male born with a bicuspid aortic valve, d-transposition of the great arteries, ventricular septal defect, right ventricular hypoplasia with a straddling right atrioventricular valve. He was palliated with Fontan circulation and developed severe neo-aortic root and arch dilation with native aortic valve stenosis, requiring surgical intervention. His neo-aortic root measured 81 × 65 mm, necessitating a Bentall procedur

[Perioperative Care for Patients with Hypoplastic Left Heart Syndrome].

Hypoplastic Left Heart Syndrome (HLHS) is one of the most challenging congenital heart defects which require surgical interventions during neonatal period. In normal risk cohort, its surgical outcome has been improved dramatically since Dr. William Norwood reported the 1st successful case more than 30 years ago. The introduction of routine bilateral pulmonary artery banding with subsequent Norwood operation during early infantile( we call it as "rapid 2 stage Norwood operation") has brought an earlier hemodynamic stabilization after Norwood operation. However, the outcome of high risk patients

Total cavopulmonary connection with extraatrial tunnel.

Between April 1997 and February 2000, total cavopulmonary connection with an extraatrial tunnel was used to treat 9 cases of complicated congenital heart disease: single ventricle (4), double-outlet right ventricle (3), mitral atresia (1), and tricuspid atresia (1). There was no mortality. One patient developed bacterial endocarditis and required reoperation after 52 days to replace the tunnel. At follow-up ranging from 11 months to 3 years, 3 patients were in New York Heart Association functional class I, and 6 were in class II. One patient with single ventricle had refractory supraventricula

Native Tricuspid Valve Endocarditis Delayed Glenn Procedure: A Case Report.

Infective endocarditis in infants with congenital heart disease is exceedingly rare and thus, treatment guidelines and expected outcomes are understudied. We present a 3-month-old girl diagnosed with native tricuspid valve endocarditis at the time of her pre-Glenn cardiac catheterization. She had resultant tricuspid valve and right ventricular dysfunction. After initiating intravenous antibiotics, she underwent an operation including a bidirectional Glenn, Sano shunt extirpation, tricuspid valve repair, and central pulmonary artery stent insertion. At 9 months postoperatively, the patient cont
