Dislocations and fracture-dislocations of the scaphoid are rare injuries. When they occur in a volar direction, they may be complicated by median nerve compression. While volar dislocations and fracture-dislocations of the scaphoid presenting late with median nerve compression have been reported, such injuries presenting with acute median nerve compression have not been reported. We describe a case of volar fracture-dislocation of the proximal half of the scaphoid causing median nerve compression and presenting with acute carpal tunnel syndrome. Urgent open reduction and internal fixation of t

A 25-year-old man complained of severe pain in the right foot after a traffic accident. There was a wound on the medial aspect of the foot extending over the length of the first metatarsal. There was no sign of vascular compromise and sensations were intact. Radiographs showed dislocation of the first tarsometatarsal (Lisfranc) and metatarsophalangeal joints with the head of the first metatarsal facing proximally and plantarward (reverse floating first metatarsal), a segmental fracture of the second metatarsal, fracture dislocation of the third metatarsal from the metatarsophalangeal and tarso…

Proximal humeral fractures account for 4% to 5% of all fractures, and most of these are minimally displaced and less prone to neurovascular injuries. This article presents a case of a 65-year-old man who injured the posterior circumflex humeral artery following a fracture dislocation of the proximal humerus leading to a life-threatening hemorrhagic complication during surgical fixation of the dislocated proximal humeral fracture. Preoperative vital signs were normal. Using the deltopectoral approach, the fracture site was exposed and the dislocated head was extracted. Blood pooled and overflow…

This is a retrospective analysis of the clinical and radiological outcome in 24 patients with acute or chronic posttraumatic elbow instability, who were treated with open reduction, internal fixation and a hinged external fixator. The instability was acute after elbow fracture dislocation in 11 cases; the other 13 had chronic posttraumatic instability of the elbow. Concentric stability and a sufficient range of motion of the elbow joint were achieved in all cases. The addition of a hinged external fixator in noncompliant patients, who underwent open reduction and internal fixation of an acute …

BACKGROUND: Complex posterior fracture-dislocations of the shoulder are rare and often associated with poor long-term function regardless of the choice of treatment. The purposes of this study were to evaluate the epidemiology and pathological anatomy of posterior fracture-dislocations of the shoulder and to assess the clinical and radiographic outcomes of a specific treatment protocol of open reduction and internal fixation.
METHODS: We studied the demographic details of a consecutive series of twenty-six patients (twenty-eight shoulders in nineteen men and seven women with a mean age of fif

Posterior dislocation of the shoulder is a rare and commonly missed injury. While prompt diagnosis and treatment is important to prevent untoward sequelae, it is often diagnosed and treated too late. The first reported case of a posterior fracture-dislocation of the humeral head following a domestic electrocution accident in Singapore is described in a 52-year-old man. The injury was missed by several doctors before a humeral head replacement was done. The diagnostic pitfalls and management of this injury and ways to avoid a missed or delayed diagnosis are discussed.

Traditionally, complex wrist injuries were identified and characterized on the basis of observation of bony injury and presumption of soft tissue embarrassment, followed by employment of a classification scheme or cumbersome nomenclature. The extent of tissue compromise was often underestimated, leading to undertreatment and inferior results. This paper encourages physicians to think more mechanistically about the pattern of injury by relying on the concept of "transverse" force transmission to identify the global extent of wrist trauma. By employing a basic understanding of the components of

We present a case of late-onset ulnar tunnel syndrome following a Colles fracture. The nerve palsy was caused by a vascular branch that stretched over the ulnar head, compressing the nerve and generating friction against the ulnar head when the forearm was rotated. This is the first report of such a pathomechanism.

A fracture of the cervical spine is a dangerous and life-threatening injury in patients suffering from ankylosing spondylitis. A 71-year-old motorist with a long history of ankylosing spondylitis was injured during a car accident. A rare transverse fracture through the body of C6 occurred and in addition a lesion of the trachea was found. This combination of injuries is described for the first time. Special problems relating to the initial rescue, diagnosis and treatment are discussed according to the literature. The necessity of handling the cervical spine with extreme caution during all medi

Thoracic duct injuries accompanying blunt thoracic trauma are rare. A significant number of these lesions, however, are associated with fracture-dislocation of the spine. In this report, we discuss the surgical management of chylothorax in this setting.

We describe a case of a 6-year-old girl with a posterolateral elbow dislocation and a concomitant fracture of the lateral humeral condyle. After reduction of the dislocation, the fracture was diagnosed and treated by open reduction and fixation, with a good functional result. In doubtful cases, oblique, heterolateral, and varus stress films, or even arthrography may be necessary.

Three patients were treated with external fixation with iliofemoral distraction as an alternative treatment of comminuted central fracture-dislocations of the hip. The patients were mobilized on the first postoperative day and discharged from the hospital an average of 10 days postaccident. The frame was left in place for 6 weeks while the traction was maintained. Full weight bearing was started 3 months after the injury. A full range of motion in the injured hip was achieved at 4 months postoperation. No signs of avascular bone necrosis were seen during follow-up for up to 2 years. The clinic

"Complete" fracture-dislocation of the thoracic spine without neurologic deficit, a rarely described injury, occurred in a 29-year-old coal miner. Partial lesions or those without neurologic deficit may be associated with spontaneous decompression. Gross translational displacement of one vertebrae on another generally compresses interposed neural structures between the columns and/or fragments. Multiple pedicular fractures or posterior column disruption from the displaced anterior column functionally widens the neural canal, even with vertebral bodies side by side. The rib cage stabilizes the

A patient is presented who had sustained a high posterior cervical esophageal laceration (secondary to an automobile accident) with concomitant fracture of the C(4) - C(5) spine. It was treated by Gulbrandson conversional method. To the author's knowledge, this represents the first recorded rupture of the cervical esophagus associated with high cord lesions as a result of blunt trauma to the neck.

BACKGROUND: Although rare, blunt traumatic aortic injury occurs in the aortic isthmus and rarely in the thoracoabdominal aorta.
CASE PRESENTATION: A 56-year-old man became unconscious after falling from the third floor during a suicide attempt. Computed tomography revealed a thoraco-abdominal aortic injury (pseudo-aneurysm) and a posterior dislocation fracture at L1/L2. Thoraco-abdominal aortic graft replacement was performed 3 d after the injury, and posterior spinal fusion was performed 10 d later.
CONCLUSIONS: Thoracic endovascular aortic repair is usually performed for blunt traumatic ao

Testicular dislocation is a rare condition in which one or both testes are displaced from the scrotum to an ectopic location and is often overlooked due to associated injuries. A 17-year-old adolescent presented three weeks after a road traffic accident with left inguinal swelling and pain. Initial assessment revealed a pelvic fracture treated conservatively. Examination showed left inguinal tenderness, and ultrasonography confirmed the testis in the inguinal region. Surgical exploration revealed displacement at the inguinal ring. The testis was repositioned and orchidopexy performed. Recovery

Anterior dislocation of the hip associated with an ipsilateral femoral neck fracture is exceedingly rare and typically results from high-energy trauma. We describe the case of a 33-year-old man who was engaged in a car crash and suffered numerous injuries, among them an anterior hip dislocation with the femoral neck trapped in the obturator foramen. The patient was treated in a single surgical procedure using a dual approach: resection of the femoral head through the Stoppa approach and placement of a primary total hip replacement through the Hardinge approach. The procedure was completed with
