Intramedullary cortical fragment in tibial nailing: push it, remove it or ignore it?

Intramedullary nailing of long bones is a safe procedure, with excellent long-term results. Even in apparently simple fractures, many complications may arise. Incarceration of a cortical fragment in the medullary canal is a fearsome situation, which may lead to severe complications and, consequently, poor outcomes. The surgeon should be aware of this risk and, after careful analysis of the pre-operative imaging, must remove or, at least, disengage the fragment from the medullary canal.

[Extensive emphysema after intramedullary nailing of a pertrochanteric fracture : Life-threatening infection or benign complication].

The development of emphysema after intramedullary nailing can represent an easily manageable complication but in the differential diagnostics it could, however, be a life-threatening infection with Clostridium perfringens. This is a report about the case of an extensive subcutaneous and retroperitoneal emphysema, which developed after intramedullary nailing of a pertrochanteric femoral fracture and where such an infection was suspected.

Is there a standard rehabilitation protocol after femoral intramedullary nailing?

INTRODUCTION: The purpose of this article was to describe the impairments and functional deficits associated with femoral shaft fractures treated with an intramedullary (IM) nail and to illustrate an evaluation-based rehabilitation program designed to target these functional deficits. LITERATURE REVIEW: The current literature describing management of diaphyseal femur fractures is replete with evidence regarding surgical management and optimal bone healing. However, a dearth exists with respect to postoperative rehabilitation recommendations, targeting the impairments that often limit return t

[Intramedullary osteosynthesis for treatment of pseudarthrosis].

The main causes of long bone fractures healing complications are: insufficient vascularisation of the bone fragments and excessive movement in the fracture site. Intramedullary osteosynthesis preceded by intramedullary reaming (internal decortication) ensure refreshing of the bone ends and a good mechanical stabilisation in cases with pseudoarthroses. Long bone fractures healing complications occurring during either conservative or operative treatment can be treated using this method. Intramedullary nailing with reaming usually suffices in the treatment of pseudoarthroses of the lower extremit

Early experience with closed interlocking medullary nailing of the femur.

We report on the first 14 patients treated with the Grosse Kempf femoral locking nail in our hospital. As with simple closed nailing of the femur attention to detail in the operating theatre is important. To date there have been only a few minor complications with the procedure, apart from one non-union. It allows stabilization of complicated fractures of the femoral shaft particularly in patients with multiple and often bilateral injuries caused by high velocity. It is concluded that it is an excellent procedure for complicated unstable fractures of the femoral shaft which would otherwise be

Remote nailing of intertrochanteric and subtrochanteric fractures of the femur.

Remote nailing of intertrochanteric and subtrochanteric fractures of the femur is a definite addition to the armamentarium in the management of these injuries. The advantages of the technique are too great to ignore. The surgeon should not be misled, however, into thinking that this is an easy operation or that it represents a panacea for difficult fracture situations. Considerable time must be spent learning the operative technique, its pitfalls and complications, and the postoperative management of patients. After the operation we allow our patients early weight bearing in the knee-extended

Complications and Outcomes After Surgical Intervention in Clavicular Nonunion: A Systematic Review.

BACKGROUND: Surgical repair of clavicle fractures is being employed more frequently, although most fractures are still treated conservatively. Both can result in nonunion. Current treatments for clavicle nonunion include open reduction with internal fixation (ORIF) plating without bone graft, ORIF plating with bone graft, and intramedullary pin fixation. METHODS: We performed a systematic review and meta-analysis of studies reporting outcome, complication, and reoperation rates following surgical treatment for clavicle nonunion. Subgroup analysis was undertaken for outcome and complication ra

Finite Element Analysis of Proximal Femoral Bionic Nail (PFBN), Proximal Femoral Nail Antirotation and InterTan for Treatment of Reverse Obliquity Intertrochanteric Fractures.

OBJECTIVE: Reverse obliquity intertrochanteric fracture is an unstable type of fracture. Current guidelines recommend intramedullary fixation, but there are still complications such as screw removal, hip varus, nail withdrawal, and nail fracture. The objective of this study was to use finite element analysis to compare the biomechanical properties of the novel proximal femoral bionic nail (PFBN), proximal femoral nail antirotation (PFNA), and combined compression interlocking intramedullary nail (InterTan) in the treatment of reverse obliquity intertrochanteric fractures (AO/OTA 31-A3.1). [Local complications of intramedullary nailing].

In the period from January 1990 to the end of December 1995 intramedullary nailing was performed in 294 patients with 156 fractures of the femur and 162 fractures of the tibia. Early nailing within the first 24 h after trauma was realized in 70% of the femur and in 64% of the tibia fractures. A synopsis focusing on local complications after intramedullary nailing is given on the basis of our results and a thorough analysis of the literature. Iatrogenic fractures of the femoral neck (0-5%), of the proximal femur (0.8-11.2%) and of the tibial head (0-8.3%) represent important intraoperative comp

"Fixator-assisted Nailing" Technique for Closed Segmental Tibia Shaft Fracture with Extensive Soft Tissue Injury - A Case Report.

INTRODUCTION: Segmental tibia fractures with extensive soft tissue injuries are rare and surgical intervention is challenging with no definitive treatment strategies. CASE REPORT: A 52-year-old man presented with closed right segmental tibia and fibula fracture with extensive blistering of skin caused due to road traffic accident. Distal pulses were palpable and there were no signs of compartment syndrome and other systemic injuries. In the presence of extensive blistering, a monoplanar external fixator was applied within 24 h of injury. 3 weeks later, skin condition was conducive for interna

Closed management of distal femoral fractures.

Since closed treatment is effective in 80% to 85% of distal femoral fractures, operative intervention is not recommended. Unnecessary surgery produces unwarranted complications. The cast-brace technique should be used in the first week or two after injury to prevent deformities commonly associated with distal femoral fractures. The thigh cast should be carefully molded well over the femoral condyles. Distal femoral traction is preferred to proximal tibial traction to correct fracture deformities. If the closed reduction technique is not satisfactory in the early period after injury, operative

Closed reduction and percutaneous stabilization of pediatric T-condylar fractures of the humerus.

T-condylar fractures of the humerus are a rare fracture pattern in children. The usual recommendation is to treat them with open reduction and internal fixation, in accordance to the usual practice in adult fracture patterns. This involves extensive surgical approach to the elbow to allow anatomic reduction and placement of hardware for rigid fixation. The authors present a technique of closed reduction of the intra-articular component of the T-condylar fracture that is stabilized with partially threaded pins that afford interfragmentary compression, followed by the use of two elastic titanium

Treatment of open fractures of the tibial shaft with the use of interlocking nailing without reaming.

Fifty open fractures of the tibial shaft that were treated with débridement and interlocking nailing without reaming were followed for an average of twelve months. Most of the fractures were the result of high-energy trauma, and 68 per cent of the fracture wounds were grade III. Forty-eight (96 per cent) of the fifty fractures united at an average of seven months; there were no malunions. There were four infections (8 per cent), all at the sites of grade-III fractures. Locking screws broke in five tibiae (10 per cent), but the breakage did not result in a loss of reduction. Three nails broke,

Technique for facilitating closed reduction of difficult flexion type supracondylar humeral fracture in children.

The displaced flexion type supracondylar humeral fractures (SCHF) are inherently unstable and there is great intraoperative difficulty in obtaining and maintaining the fracture reduction by closed means. We introduced a technique for closed reduction and K-wires pinning of displaced flexion type SCHF. Fourteen patients with flexion-type SCHF (9 boys and 5 girls) underwent a reduction technique using a construct of three K-wires. The proximal wire was used for rotational control of the proximal fragment and the two distal wires were used for correction of the flexion and rotational deformity of

Comparison of four surgical methods for pediatric forearm double diaphyseal fractures: a retrospective analysis.

OBJECTIVE: One of the most frequent fractures in children is a double forearm fracture. They make up 26% of children's long bone fractures in the upper extremities and their incidence has increased in recent years. In this study, pediatric patients with double forearm fractures were treated using plate screw, intramedullary K-wire (I-KW), intramedullary titanium elastic nails (TENs), and hybrid fixation (HF) to compare the radiographic and functional results. PATIENTS AND METHODS: The printed and digital medical records were retrospectively examined from our hospital's archives after receivin

[Combined osteosynthesis in diaphyseal fractures of the long tubular bones].

An operative method of union and fixation of long tubular bones consisting in combination of the compressive-destruction and intramedullary osteosynthesis is described. The method is minimally traumatic and gives high degree of stabilization of fragment ends. Early functional load on the extremity facilitates the union of fractures.
