Frontonasal duct injury is a critical factor in the treatment of frontal sinus fractures. We present the case of a 43-year-old man who sustained multiple comminuted fractures of the anterior table of the frontal sinus and of the orbital roof as the result of a gunshot. After adequate debridement, the combined bone and soft tissue defect was reconstructed with a titanium mesh and a free radial forearm flap. The frontonasal duct injury was treated by surgical obliteration with a custom designed fascio-subcutaneous extension of the free radial forearm flap. The postoperative course was uneventful

The use of endoscopically assisted techniques allows limited incisions for the open reduction of condylar process fractures. Intraoperative control after fracture reduction in areas of limited exposure and visibility is possible. In addition, the risk of facial nerve damage and extensive visible scars can be reduced by using minimally invasive endoscopic techniques. Although the use of endoscopically associated techniques in maxillofacial surgery offers many options and advantages, indications for open treatment of maxillofacial trauma have not changed. The repair of severely dislocated and co

BACKGROUND: The traditional strategy for fixing intra-articular distal humerus fractures is double plating placed in an orthogonal configuration, based on posterior approach. With a combined medial and lateral approach, a novel configuration of plating (combined anteromedial and anterolateral plating) has been used. In this study, we investigated the biomechanical properties of the novel plating by comparing it with some traditional strategies.
METHODS: Based on the 3D morphology of a healthy subject's humerus, models of three types of intra-articular distal humeral fractures were established

Medial-end clavicle fractures are rare, and the published literature is limited. Most injuries can be managed nonoperatively with favorable outcomes; however, displaced or comminuted fractures, symptomatic delayed union/nonunion, or fixation failure may require surgery, and operative management is challenging because of the proximity to vital mediastinal structures. We describe a 38-year-old man with a left medial-end clavicle fracture sustained in a fall from approximately 9-10 m. Two prior plate fixation attempts at an outside clinic failed to maintain stable reduction, with persistent pain

BACKGROUND: Historically, depressed skull fractures that warranted surgery were treated in 2 stages: the first stage involved debridement and craniectomy, followed by the second stage of delayed cranioplasty. More recently, single-stage autologous cranioplasty has been proven to be safe. However, there is a paucity of literature regarding single-stage titanium mesh cranioplasty when autologous repair is not possible.
METHODS: A retrospective review identified 22 patients who underwent single-stage titanium mesh cranioplasty for the acute treatment of comminuted depressed skull fractures. Frac

BACKGROUND: The purpose of this study was to investigate the mechanical stability of the posterior acetabular column plate and different posterior acetabular wall prostheses used in treating posterior acetabular fractures with or without comminution.
METHODS: The unilateral normal ilium was reconstructed, and a model of posterior acetabular wall fracture was established on this basis. The fracture fragment accounted for approximately 40% of the posterior acetabular wall. The posterior acetabular column plate and different posterior acetabular wall prostheses were also designed. Using static a

BACKGROUND: The increasing use of orthopedic total hip arthroplasty implants has led to a consequent rise in the incidence of associated periprosthetic fractures (PPFs). Analysis of the National Joint Registry data showed the choice of cemented hip stem influenced the risk of a PPF occurring. However, the effect of implant design in relation to the risk of PPFs has not been investigated.
METHODS: The main objective is to compare the biomechanics of PPFs as a failure of the Exeter V40, CPT, and DePuy C-Stem stems in a composite Sawbone model to identify whether a difference in the risk of frac

Distal humeral fractures are difficult to treat. In the elderly population, the problems are compounded by osteoporosis and gross comminution. Open reduction and internal fixation for such fractures is sometimes difficult and may be associated with poor results. Total elbow arthroplasty has been suggested as a last-ditch effort to salvage functional use for such difficult fractures in the elderly. We followed seven patients (seven elbows) with a mean age of 81.7 years at the time of injury. Open reduction and internal fixation was considered a difficult option for these fractures. They were tr

BACKGROUND: Because of strong loads acting in the elbow joint, intraarticular fractures with a methaphyseal comminuted fracture site at the distal humerus demand a lot from the osteosynthetic care. Ambiguities arise concerning to the anatomic position of the implants and the resulting mechanic performance. The aim of this biomechanical study was to compare the performance of different anatomical plate configurations for fixation of comminuted distal humerus fractures within one system.
METHODS: In an artificial bone model two perpendicular and one parallel plating configuration of a dedicated

INTRODUCTION: Proximal humerus fractures (PHF) are common in the elderly, accounting for significant morbidity and mortality. Non-surgical treatment is a common option for low-demand elderly patients, but it can lead to unsatisfactory functional outcomes in some cases. The identification of predictive factors for poor prognosis in non-surgical management remains unclear. This study aimed to determine the predictive factors for poor prognosis in elderly patients treated non-surgically for displaced PHF and to assess associated complications.
METHODS: A retrospective cohort study was conducted

Background Total elbow arthroplasty is an accepted procedure for the treatment of acute comminuted distal humeral fractures in elderly. Few long-term outcomes are available. The purpose of this study was to examine long-term clinical and radiological outcomes of prosthesis performed (January 2002-June 2015) for complex intra- fractures of the distal humerus (AO/C) in patients older than 65 years with low functional demands. Materials and Methods Further inclusion criteria were: the availability of a clinical and radiological follow-up of minimum 5 years and pattern of closed fracture. Demograp

Fractures of the acetabulum in elderly patients were often caused by low energy trauma. Fractures involving anterior column are more common and often associated with impaction and comminution. Osteoporosis further complicates the management. Percutaneous fracture fixation has low morbidity but it is a technically demanding procedure. Open reduction and fracture fixation is done with or without simultaneous total hip replacement. Delayed total hip replacement is considered in posttraumatic arthritis patients. Patients with minimal displaced fractures, associated both-column fractures with secon

An indirect system to reduce and hold comminuted fractures of the distal radius was developed in the mid 1970's starting with a simple threaded distraction rod and advancing to a versatile system of rods and universal clamps. This system has gained wide acceptance since then and is used for the wrist joint and for other indications in surgery of the hand, the upper limb, the foot and in paediatric orthopaedics.
