The "sagging rope sign" in avascular necrosis in children's hip diseases--confirmation by 3D CT studies.

Growth disturbance of the proximal femoral epiphysis and physis secondary to avascular necrosis (AVN) in a variety of children's hip disorders produces changes in the femoral head and neck that make radiographic interpretation difficult. The enlarged overhanging femoral head produces radiographic markings on the femoral neck which are sometimes confusing. These have sometimes been misinterpreted as growth arrest lines. Apley and Wientroub reintroduced Perkins' description of the "sagging rope" sign in AVN of the femoral head, and Clarke clarified that this puzzling radiographic transverse meta

Novel IGF1R Variants in Short Stature: Lessons from Two Patients and Outcome of Growth Hormone Therapy.

The growth hormone (GH)-insulin-like growth factor 1 (IGF1) axis is essential for the regulation of growth. IGF1 exerts its effects through the IGF1 receptor type 1 (IGF1R) that plays a pivotal role in fetal and postnatal growth. Pathogenic monoallelic IGF1R variants are known to cause pre-and postnatal growth restriction, often accompanied by normal or elevated serum IGF1 levels. Herein, the clinical and genetic characteristics of two cases with IGF1R novel variants, describing their growth patterns, endocrinological findings, and response to recombinant human GH (rhGH) therapy are presented.

Genetic Findings in Short Turkish Children Born to Consanguineous Parents.

INTRODUCTION: The diagnostic yield of genetic analysis in the evaluation of children with short stature depends on associated clinical characteristics, but the additional effect of parental consanguinity has not been well documented. METHODS: This observational case series of 42 short children from 34 consanguineous families was collected by six referral centres of paediatric endocrinology (inclusion criteria: short stature and parental consanguinity). In 18 patients (12 families, group 1), the clinical features suggested a specific genetic defect in the growth hormone (GH) insulin-like growt

Growth without growth hormone and similar dysmorphic features in three patients with sporadic combined pituitary hormone deficiencies.

BACKGROUND/AIMS: Mutations in the HESX1 gene are associated with a broad spectrum of phenotypes: septo-optic dysplasia, midline defects, pituitary abnormalities with consequent hypopituitarism, isolated growth hormone (GH) deficiency or combined pituitary hormone deficiencies (CPHD). This study examined the prevalence of mutations in the HESX1 gene in patients with CPHD. PATIENTS/METHODS: Sixty patients with sporadic CPHD without septo-optic dysplasia were screened for mutations in HESX1. RESULTS: Three patients were found to be heterozygous for the same Asn125Ser variant in the HESX1 gene.

Growth hormone deficiency in adults: characteristics and response to growth hormone replacement.

Despite adequate adrenal, gonadal, and thyroid hormone replacement, many adults with hypopituitarism have a recognizable syndrome of weakness and diminished sense of well-being, accompanied by alterations in metabolism and body composition, as well as increased mortality. Short-term treatment with human growth hormone improves many of these abnormalities, but a clear improvement in functional status has yet to be demonstrated. Until such an effect is shown, the use of growth hormone replacement in adults with hypopituitarism remains investigational.

Evaluation of growth hormone secretion: provocative testing vs endogenous 24-hour growth hormone profile.

A total of 55 children underwent hGH provocative testing with two or more provocative agents, and measurement of endogenous 24-hour hGH secretion. Patients were divided into groups according to their peak hGH secretory response to provocative testing and their mean 24-hour hGH concentration. Peak hGH response to provocative testing was significantly greater in control children and in children with hGH neurosecretory dysfunction (GHND) than in the classical hGH deficient group. Mean 24-hour hGH concentration was significantly greater in the control group than in either the classical hGH deficie

Effect of androgen on growth hormone secretion and growth in boys with short stature.

The response of plasma growth hormone (GH) to insulin-induced hypoglycaemia (IIH) and arginine infusion (AI) was studied in 22 young males (ages 8 to 17 years) with short atature and absent or delayed sexual maturation, before and after androgen administration. During initial evaluation, 5 patients had blunted GH response to IIH, 12 responded subnormally to AI and 4 failed to respond normally to either stimulus. These same studies were repeated in each patient following androgen administration. The source of androgen was as follows: a) 5 days of testosterone propionate (25 mg intramuscularly d

A46, a benzothiophene-derived compound, suppresses Jak2-mediated pathologic cell growth.

Hyperkinetic Jak2 tyrosine kinase signaling has been implicated in several hematological disorders, including myeloproliferative neoplasms. Effective Jak2 inhibitors can have significant therapeutic potential. Here, using structure-based virtual screening, we identified a benzothiophene-derived Jak2 inhibitor named A46. We hypothesized that this compound would inhibit Jak2-V617F-mediated pathologic cell growth. To test this, A46 was analyzed for its ability to inhibit recombinant Jak2 protein catalysis; suppress Jak2-mediated pathogenic cell growth in vitro; inhibit the aberrant ex vivo growth

The mechanism of action of non-coding RNAs in placental disorders.

Placental complication arises due to various risk factors occurring during the pregnancy period, leading to an increased morbidity rate. Placenta related disorders are one of primary reason for pregnancy related complications and the clinical incidences are seen to be on the rise. Most of the common disorders associated with placenta are pre-eclampsia, recurrent spontaneous abortions, intra-uterine growth restriction etc. Several studies have been done to understand the genetics and immunological attributes leading to the development of placenta associated complications. In the recent years, s

Genetic analysis of spinal dysraphism with a hamartomatous growth (appendix) of the spinal cord: a case series.

BACKGROUND: Spinal dysraphism with a hamartomatous growth (appendix) of the spinal cord is better known as herniated spinal cord. There are many arguments in favour of considering it a developmental defect. From this point of view, it is a type of neural tube disorder. Neural tube disorders can be caused by multiple factors, including a genetic factor. A common genetic defect in patients with a spinal dysraphism with a hamartomatous growth of the spinal cord is sought for. CASE PRESENTATION: In two patients with a symptomatic lesion and referred to an academic hospital a genetic analysis was

Therapeutic Potential and Molecular Mechanisms of the Multitargeted Flavonoid Fisetin.

Flavonoids effectively treat cancer, inflammatory disorders (cardiovascular and nervous systems), and oxidative stress. Fisetin, derived from fruits and vegetables, suppresses cancer growth by altering cell cycle parameters that lead to cell death and angiogenesis without affecting healthy cells. Clinical trials are needed in humans to prove the effectiveness of this treatment for a wide range of cancers. According to the results of this study, fisetin can be used to prevent and treat a variety of cancers. Despite early detection and treatment advances, cancer is the leading cause of death wor

Surgical Management of Spinal Disorders in People with Mucopolysaccharidoses.

Mucopolysaccharidoses (MPS) are a group of inherited, multisystem, lysosomal storage disorders involving specific lysosomal enzyme deficiencies that result in the accumulation of glycosaminoglycans (GAG) secondary to insufficient degradation within cell lysosomes. GAG accumulation affects both primary bone formation and secondary bone growth, resulting in growth impairment. Typical spinal manifestations in MPS are atlantoaxial instability, thoracolumbar kyphosis/scoliosis, and cervical/lumbar spinal canal stenosis. Spinal disorders and their severity depend on the MPS type and may be related t

Ghrelin gene variants and eating disorders.

Genetic factors have been implicated in playing a significant role in susceptibility to eating disorders such as anorexia nervosa (AN) and bulimia nervosa (BN). Genetic variants of ghrelin, an endogenous acylated peptide that stimulates growth hormone secretion, enhances appetite, and increases body weight, have been investigated in association with eating disorders, as changes in the ghrelin/growth hormone secretagogue receptor (GHSR)/ghrelin O-acyltransferase (GOAT) system have been implicated in its pathology. Although most candidate-gene association studies have not been able to identify g

Breast diseases in adolescents.

Fibroadenomas and breast growth disorders are the most common breast diseases in adolescent women. Assessment of breast disorders in this age group generally involves clinical evaluation through history and physical examination and when is needed ultrasonography. Due to the absence of breast cancer in adolescent women, it is easy to reassure women at the first consultation. Breast growth disorders can lead great psychological and physical embarrassment. Treatment consists of surgical procedures when the cosmetic defect is severe. According to the ANDI classification, small fibroadenomas are no…
