Economic impact of motorcycle helmets: from impact to discharge.

BACKGROUND: The economic impact of helmet use remains controversial. Previous studies of injured motorcyclists suggest a marginal inpatient hospital cost difference between helmeted and unhelmeted riders. The purpose of this study was to expand the economic analysis of motorcycle helmet utilization to the point of injury by including motorcycle crash patients who do not require hospital admission. METHODS: Prehospital motorcycle crash data were collected from the National Highway Transportation Safety Administration (NHTSA) General Estimates System (GES) database from 1994 to 2002 with respec

The biomechanics of spinal injuries.

This manuscript is directed to review the epidemiology of spinal trauma, the anatomy of the vertebral column, spinal ligaments, muscles, motion of the spine and spinal cord. Because little information is available on the material properties of ligaments and the components of the vertebral column, this material is also included. A review of the experimental spinal cord injury models is given because of the concerted interest in this area today. Laboratory studies conducted in animals to develop typical spinal cord injury models and the corresponding alterations in perfusion and metabolic pathwa

Early treatment of coronal synostosis with endoscopy-assisted craniectomy and postoperative cranial orthosis therapy: 16-year experience.

OBJECT: The objective of this study was to present the authors' 16-year experience treating coronal craniosynostosis in infants using endoscopy-assisted techniques and postoperative cranial orthoses. METHODS: A total of 128 synostosed coronal sutures in 115 patients were treated between 1996 and 2012 by endoscopically resecting a strip of bone containing the stenosed suture via a 2-3 cm incision made at the ipsilateral stephanion. Data were obtained from a prospective database. Following surgery, patients were fitted with custom cranial orthoses to help correct preoperative craniofacial defor

Reality and risk of contact-type head injuries related to bicycle-mounted child seats.

OBJECTIVE: The authors have treated numerous children who have been injured by falling from bicycle-mounted child seats. Despite the greatly increased use of such seats, the understanding of their risk and the importance of helmet use remains alarmingly poor. The objective of this study was to confirm the risk of bicycle-mounted child seats and to evaluate the efficacy of helmets, seat belts, and back seat height in terms of preventing or mitigating contact-type head impacts that occur in falls from bicycle-mounted child seats. MATERIALS AND METHODS: Biometrical dummy tests were performed to

Injury Risk Functions for the Midsized Male Wrist and Elbow as a Result of Behind Shield Blunt Trauma.

Ballistic shields protect users from a variety of threats, including projectiles. Shield back-face deformation (BFD) is the result of the shield deflecting or absorbing a projectile and deforming toward the user. Back-face deformation can result in localized blunt loading to the upper extremity, where the shield is supported by the user. Two vulnerable locations along the upper extremity were investigated-the wrist and elbow-on eight postmortem human subjects (PMHS) using a pneumatic impacting apparatus for investigating the fracture threshold as a result of behind shield blunt trauma (BSBT).

Point-of-care ultrasound in obstetrics and gynecology: latest advancements and evolution.

Point-of-care ultrasound (POCUS) in obstetrics and gynecology has evolved from a pragmatic bedside adjunct into a rapidly expanding clinical, educational and technological field. Since the publication of earlier narrative reviews, the discipline has been reshaped by handheld ultrasound systems, wireless probes, cloud-based image management, tele-ultrasound, artificial intelligence (AI) for image acquisition and interpretation, and the first specific practice guidance for POCUS in obstetrics and gynecology. This narrative review summarizes recent advances and proposes a clinically useful framew

Helmets Matter: Kentucky All-Terrain Vehicle Crashes Seen at a Tennessee Trauma Center.

All-terrain vehicle (ATV) safety laws, including helmet use, vary by state and are sporadically enforced. Kentucky state laws require safety helmets only for younger riders. We hypothesized that ATV riders injured in Kentucky and seen at a Tennessee trauma center would more likely be unhelmeted, have more severe head injuries, and have higher mortality rates than those injured in Virginia or Tennessee. A Trauma Registry review of 750 injured ATV riders from June 1, 2005, through June 1, 2015 examined state location of accident, helmet use, markers of injury severity, and outcomes. Multiple log

Cycling in the city: an in-depth examination of bicycle lane use in a low-income urban neighborhood.

BACKGROUND: Regular physical activity such as biking can help prevent obesity and chronic disease. Improvements in cycling infrastructure are associated with higher overall cycling rates, but less is known about bike lane utilization in low-income urban neighborhoods. METHODS: During the summer of 2009, 4 Central Brooklyn streets with bicycle lanes were studied using camcorders to record for a total of 40 hours. Video recordings were coded for behaviors and characteristics of cyclists and motorists. An intercept survey (N = 324, 42% participation rate) captured information on cyclist demograp

A 10-year population survey of spinal trauma and spinal cord injuries after road accidents in the Rhône area.

Fatalities or injuries following motorized and non-motorized vehicle accidents (MNMVA) are reported by police or health care systems. However, limited data exist for spinal injuries. Using an epidemiological database of road accidents occurring in a defined geographic area, we measured the incidence of major spinal trauma (MST, Abbreviated Injury Scale [AIS] score 2 or more), spinal cord injury (SCI, AIS score 4 or more), and associated lesions over a 10-year period (1997-2006). Among the 97,341 victims included, 21,623 (22.2%) suffered spinal trauma, but only 1523 (1.6%) and 144 (0.2%) sustai

Development and pilot testing of a bicycle safety questionnaire for adult bicyclists.

Most deaths associated with bicycle riding are reported in those over the age of 16 and the highest death rate per million trips is reported in those over the age of 50, yet most efforts to reduce deaths among bicycle riders have focused on children. The purpose of this study was to pilot test a new instrument designed to assess knowledge of bicycle laws and perception of risk related to specific host behaviors and agents of injury. A pilot study using a descriptive correlational design was conducted among a convenience sample of 104 adult bicycle riders. A 25-item questionnaire was administer

Bicycle injuries: road trauma is not the only concern.

Data on bicycle injury presentations at a public hospital emergency department for 1991-95 inclusive were analysed to determine the nature and extent of bicycle injuries in the community. There were 599 bicycle injury presentations during the study period, representing more than 2.0% of all injury-related presentations to the emergency department. The main outcome measures were severity and type of injury. Distribution by gender, age, helmet use (1991 and 1992 only), location and mechanism was examined. Rider-only injuries (falls or collisions with stationary objects) accounted for 79.3% of al
