OSA and Cardiac Arrhythmogenesis: Mechanistic Insights.

A surge of data has reproducibly identified strong associations of OSA with cardiac arrhythmias. As an extension of epidemiologic and clinic-based findings, experimental investigations have made strides in advancing our understanding of the putative OSA and cardiac arrhythmogenesis mechanistic underpinnings. Although most studies have focused on the links between OSA and atrial fibrillation (AF), relationships with ventricular arrhythmias have also been characterized. Key findings implicate OSA-related autonomic nervous system fluctuations typified by enhanced parasympathetic activation during

The Janus face of adenosine: antiarrhythmic and proarrhythmic actions.

Adenosine is a ubiquitous, endogenous purine involved in a variety of physiological and pathophysiological regulatory mechanisms. Adenosine has been proposed as an endogenous antiarrhythmic substance to prevent hypoxia/ischemia-induced arrhythmias. Adenosine (and its precursor, ATP) has been used in the therapy of various cardiac arrhythmias over the past six decades. Its primary indication is treatment of paroxysmal supraventricular tachycardia, but it can be effective in other forms of supraventricular and ventricular arrhythmias, like sinus node reentry based tachycardia, triggered atrial t

Inducibility of atrial and ventricular arrhythmias along the ligament of marshall: role of autonomic factors.

BACKGROUND: The mechanism(s) underlying atrial fibrillation (AF) initiation along the ligament of Marshall (LOM) remains controversial. OBJECTIVES: We sought to examine the role of the autonomic nervous system in arrhythmogenesis along the LOM. METHODS: In 31 anesthetized dogs, a left thoracotomy exposed the LOM. During atrial pacing, high-frequency stimulation (HFS: 200 Hz, 0.1 ms pulse width, 40 ms duration, 0.6-12 V) was delivered during atrial refractoriness to different sites of LOM (LOM(CS)= near coronary sinus; LOM(LIPV)= near left inferior pulmonary vein; LOM(LS-LIPV)= between LIPV a

Paroxysmal atrial fibrillation: a disorder of autonomic tone?

Electrophysiological characteristics of atrial cells (action potential duration and refractoriness, conduction speed) are modulated differently by vagal and sympathetic influences. The former tends to favour macro-reentry phenomena whereas the latter favours abnormal automaticity and triggered activity. In normal hearts vagal influences are predominant, thus explaining that the clinical pattern of vagal-mediated paroxysmal atrial fibrillation is preferentially observed in the absence of detectable heart disease, in young male adults, with an ECG pattern of common flutter alternating with fibri

[Role of the autonomic nervous system in the genesis of the electrophysiologic pattern of the double nodal pathway].

Thirty-five patients (age: 53 +/- 13 years) with dual A-V nodal pathways pattern were studied following autonomic blockade (propranolol 0.2 mg/kg and atropine 0.04 mg/kg). Six of them had "idionodal" paroxysmal reentrant supraventricular tachycardia. After autonomic blockade the dual A-V nodal pathways pattern remained in 20 patients (57%) and disappeared in 15; however, in the latter, the effective refractory period of the fast pathway decreased significantly and the disappearance of the slow pathway conduction may not have been real since the marked shortening of the fast pathway refractory

Adaptive PID Control for Chronotropic Efficiency in Cardiac Pacemakers.

Chronotropic Incompetence (CI) has recently been discovered as a sign of near-future heart failure. Having the ability to correct CI could avoid or delay chronic heart failure. Pacemakers account for the heart's failed electrical conduction for specific cardiac arrhythmias such as Tachy-Brady Syndrome, Atrial Fibrillation, and many other conditions, most of which arise from a failing sinoatrial (SA) node. Because of the increasing relevance of cardiovascular disease and, as a result, pacemakers, modeling patients with similar conditions as a control system is of great importance. We introduce

Age-related differences in the direct cardiac effects of cisapride: narrower safety range in the hearts of young rabbits.

Although cisapride is widely used to treat gastrointestinal motility disorders, it has been associated with QT prolongation, torsades de pointes, and cardiac arrest. Only in children, however, has atrioventricular (AV) block after cisapride been reported. This study used Langendorff perfusion to determine the direct effects of cisapride (0.03, 0.1, 0.3, and 1 microM) on the conduction properties of neonatal (<7 d) and adult (>3 mo) rabbit hearts. At a clinically relevant dose (0.03 microM), cisapride slowed the recovery of the His-Purkinje system. At 0.1 microM, the refractoriness of the His-P

Electrocardiographic imaging: technical developments and future applications for non-invasive electroanatomical study of the human heart.

Electrocardiographic imaging (ECGI) is a non-invasive technique that combines patient-specific cardiac anatomy with numerous body-surface potentials to gain insight into the in vivo electrical epicardial activity across the whole heart. This review summarises some of the latest hardware developments for ECGI data acquisition as well as innovative software solutions to address the inverse problem of electrocardiography. It delves into some of the successful clinical applications of ECGI while highlighting the hurdles that still stand in the way of its more widespread roll-out to healthcare. As

Ventricular activation and repolarization in response to physiological and conventional pacing using ultra-high-frequency electrocardiography.

BACKGROUND: Physiological pacing targeting the cardiac conduction system is increasingly being adopted as an alternative to conventional right ventricular (RV) pacing for the treatment of bradyarrhythmias, although its effects on ventricular repolarization remain underexplored. OBJECTIVE: This study evaluates depolarization and repolarization responses to different pacing techniques using ultra-high-frequency electrocardiograms (UHF-ECGs). METHODS: Temporary pacing was performed at different cardiac areas in 178 patients with bradycardia. Depolarization was assessed via QRS duration (QRSd),

The influence of delayed ventricular activation on cardiac repolarization: Insights from electrocardiographic imaging.

In healthy human hearts, the T-wave is concordant with the QRS-complex, which is often explained by the negative relationship between the activation (AT) and repolarization time (RT). Delayed ventricular conduction is typically accompanied by discordant T-waves, but the link to regional repolarization is unknown. The present study aimed to compare AT-RT relationships in patients with narrow QRS-complexes (nQRS) vs. patients with left bundle branch block (LBBB) or intraventricular conduction disturbances (IVCD) using electrocardiographic imaging to reconstruct epicardial unipolar electrograms.

Origin of ventricular fibrillation triggers in a model of localized repolarization heterogeneity.

BACKGROUND: Heterogeneities in ventricular repolarization contribute significantly to the genesis of ventricular fibrillation (VF). Although clinical arrhythmias are spontaneously triggered by premature ventricular complexes, these triggers are difficult to document and little is known about their site of origin. OBJECTIVES: The purpose of this study was to characterize spontaneous VF initiation in an experimental model of repolarization heterogeneity and to identify the origin of triggers in relation to the spatial dispersion of repolarization. METHODS: Spatially limited repolarization hete

Redefining left bundle branch block from high-density electroanatomical mapping.

BACKGROUND: The existing ECG criteria for diagnosing left bundle branch block (LBBB) are insufficient to distinguish between true and false blocks accurately. METHODS: We hypothesized that the notch width of the QRS complex in the lateral leads (I, avL, V5, V6) on the LBBB-like ECG could further confirm the diagnosis of true complete left bundle branch block (t-LBBB). We conducted high-density, three-dimensional electroanatomical mapping in the cardiac chambers of 37 patients scheduled to undergo CRT. These patients' preoperative electrocardiograms met the ACC/AHA/HRS guidelines for the diagn

Pulsed low-energy stimulation initiates electric turbulence in cardiac tissue.

Interruptions in nonlinear wave propagation, commonly referred to as wave breaks, are typical of many complex excitable systems. In the heart they lead to lethal rhythm disorders, the so-called arrhythmias, which are one of the main causes of sudden death in the industrialized world. Progress in the treatment and therapy of cardiac arrhythmias requires a detailed understanding of the triggers and dynamics of these wave breaks. In particular, two very important questions are: 1) What determines the potential of a wave break to initiate re-entry? and 2) How do these breaks evolve such that the s

Pseudo normalization of H-V interval due to intra-His block in a patient with fasciculoventricular accessory pathway.

A 57-year-old male suffering from cardiogenic syncope was found to have preexcited QRS on surface ECG at admission. A dual-chamber ICD was implanted after discovering intermittent high degree A-V block and ventricular tachycardia during hospitalization. An EP study was performed 2 days later. Fasciculoventricular accessory pathway was diagnosed based on the fixed H-V interval with different A-H interval when atrial activation conducted to ventricle. However, the H-V interval was normal, which can be explained by intra-His block based on the findings of two split His potentials, the second of w

Transient left septal and anterior fascicular block associated with type 1 electrocardiographic Brugada pattern.

The left septal fascicular block (LSFB) or blockage of the middle fibers of the left bundle branch is probably caused mainly by - in the developed world - the proximal obstruction of the left anterior descending artery (LAD) before its first anterior septal perforator branch (S1). The association of transient LSFB and left anterior fascicular block (LAFB) - left bifascicular block - and the electrocardiographic type 1 Brugada pattern (BrP) has not been described in the literature yet.
