Anxiety, heart rate and their interrelation at mental stress in school children.

Heart rate (HR) was measured in 63 ten year old boys during psychic stress induced by dental examination and the reading of a difficult test before a public. By means of the CMAS questionnaire, a teacher's questionnaire, rating of manifestations during the examination and selfcomparison by the children, we tried to assess the level of their anxiety. We looked for correlations between the individual measures of this personality trait and between anxiety and autonomic (HR) reactivity. From the aspect of HR the whole experiment appeared as a uniform, ever increasing stress, including socalled res…

Effect of demand characteristics on heart rate during a behavioral avoidance test.

A 2 X 2 experimental design utilized two levels of verbally reported snake fearfulness (high-low) and two levels of experimental demand (high-low) with 16 college student subjects during a behavioral avoidance test. Subjects given high-demand instructions for heart-rate increase increased their heart rate more than subjects given low-demand instructions. Results were specific to heart rate, as demand produced no effects on approach behaviors or fear ratings. Heart rate during the behavioral avoidance test was shown to be as susceptible to experimental demand as other studies have shown approac…

Clinical significance of fetal heart rate patterns during labor. I. Baseline patterns.

From a population of 2,774 high-risk patients monitored during labor, 1,304 single pregnancies in cephalic presentation and with direct monitoring for at least 1 hour before completion or cesarean-section were studied. The maternal and fetal clinical data and the tracings were hand reviewed, coded, and programmed for computer analysis. In the record were studied baseline, its changes (tachycardia, fixed, saltatory), the accelerations, and the decelerations (early, variable, late). Fifty-four per cent had some type of FHR deceleration. Accelerations were recorded in over 12 per cent of all case

Impairment of autonomically mediated heart rate control in patients with cardiac dysfunction.

Patients with cardiac disorders have defective parasympathetic control of heart rate. To evaluate the possibility of similar changes in sympathetic control of heart rate, we compared reflex chronotropic responses to 80 degree upright tilt and nitroglycerin-induced hypotension in 31 cardiac patients and 7 normal individuals before and after partial parasympathetic blockade with atropine. Tilting revealed an attenuation of the normal heart rate increase in patients; the magnitude of this defect was greatest in patients with more severe symptoms (class III) and evidence of left ventricular dysfun…

Accuracy of Photoplethysmography-Derived Pulse Rate Variability Compared with Electrocardiography-Derived Heart Rate Variability: A Systematic Review and Meta-Analysis.

Photoplethysmography-derived pulse rate variability (PPG-derived PRV) is increasingly used in wearable and camera-based biomedical sensors as a low-burden alternative to electrocardiography-derived heart rate variability (ECG-derived HRV), but its agreement with ECG-derived measurements remains uncertain. This systematic review and meta-analysis evaluated the accuracy of PPG-derived PRV compared with ECG-derived HRV in healthy or apparently healthy non-clinical populations, focusing on the root mean square of successive differences (RMSSD) and the standard deviation of normal-to-normal interva…

Heart Rate, Acidosis, Consciousness, Oxygenation, and Respiratory Rate: A Perfect Weaning Index or Just a New Kid on the Block.

UNLABELLED: Successful weaning is when spontaneous breathing is sustained for more than 48 hours after extubation. Despite a plethora of individual and composite weaning indices being available, most indices have not found much clinical utility, and weaning continues to be largely based on clinical assessment. Heart rate, acidosis, consciousness, oxygenation, and respiratory rate (HACOR) is a new score for prediction of failure of noninvasive ventilation (NIV) in hypoxemic patients receiving NIV. The present study explores its utilization in weaning from invasive ventilation. HOW TO CITE THIS…

Sleep Rate Mode of Pacemaker-Dependent Patients with Sick Sinus Syndrome Increases Dipper Blood Pressure and Dipper Heart Rate Patterns.

Cardiovascular event rates of patients with a dipper blood pressure (BP) and dipper heart rate (HR) pattern are lower than those of patients with nondipper BP and HR patterns. However, how the pacemaker mode affects the diurnal BP and HR patterns remains unclear. We enrolled nine patients (average age 74.4 ± 6.6 years, 4 males and 5 females) with sick sinus syndrome who required atrial pacing. We investigated sequential 6-month pacing regimens (DDD mode at 60 bpm and sleep rate mode). We set the lower rate of sleep rate mode as follows: 60 bpm during the daytime and 50 bpm during the nighttime.…

A meta-analysis investigating the outcomes and correlation between heart rate variability biofeedback training on depressive symptoms and heart rate variability outcomes versus standard treatment in comorbid adult populations.

BACKGROUND AND AIM: Heart rate variability biofeedback (HRVB) has previously been used to ameliorate depressive symptoms but its uses for tackling depressive symptoms in an array of comorbid adult patients is less established. This meta-analysis aims to evaluate whether HRVB is a useful tool to reduce depressive symptoms and improve HRV relative to standard treatment in adult comorbid populations, while also attempting to establish the association between the two outcomes. METHODS: An extensive literature review was conducted using several databases including PubMed, Cinahl, Medline, Web of s…

Assessing the impact of a combination of sofosbuvir and daclatasvir treatment for hepatitis C virus infection on heart rate, rhythm and heart rate variability using 24-hour ECG monitoring.

BACKGROUND: Direct-acting antiviral agents (DAAs) cure patients with hepatitis C virus (HCV) infection. Concerns have arisen the occurrence of significant bradyarrhythmias during treatment with DAAs. The aim of this study was to assess the impact of a DAA combination for the treatment of HCV infection on heart rate, rhythm, and heart rate variability (HRV) using 24-h ECG monitoring. RESULTS: A prospective randomized study of 50 treatment-naïve patients with HCV infection treated with a combination of sofosbuvir 400 mg daily and daclatasvir 60 mg daily for 12 weeks. Surface ECG and 24-h ECG mo…

Heart Rate Variability for Risk Assessment of Myocardial Ischemia in Patients Without Known Coronary Artery Disease: The HRV-DETECT (Heart Rate Variability for the Detection of Myocardial Ischemia) Study.

Background Detecting significant coronary artery disease (CAD) in the general population is complex and relies on combined assessment of traditional CAD risk factors and noninvasive testing. We hypothesized that a CAD-specific heart rate variability (HRV) algorithm can be used to improve detection of subclinical or early ischemia in patients without known CAD. Methods and Results Between 2014 and 2018 we prospectively enrolled 1043 patients with low to intermediate pretest probability for CAD who were screened for myocardial ischemia in tertiary medical centers in the United States and Israel.…

A novel 'splice site' HCN4 Gene mutation, c.1737+1 G>T, causes familial bradycardia, reduced heart rate response, impaired chronotropic competence and increased short-term heart rate variability.

BACKGROUND: The most important molecular determinant of heart rate regulation in sino-atrial pacemaker cells includes hyperpolarization-activated, cyclic nucleotide-gated ion channels, the major isoform of which is encoded by the HCN4 gene. Mutations affecting the HCN4 gene are associated primarily with sick sinus syndrome. METHODS AND RESULTS: A novel c.1737+1 G>T 'splice-site' HCN4 mutation was identified in a large family with familial bradycardia which co-segregated with the disease providing a two-point LOD score of 4.87. Twelve out of the 22 investigated family members [4 males, 8 femal…

Resting Heart Rate Variability is Independently Associated with Visceral Fat Rating Scores in Saudi Adult Males.

BACKGROUND: Visceral adipose tissue (VAT) may be a specific modifiable contributor to body composition-related autonomic impairment. OBJECTIVES: To compare heart rate variability (HRV) between groups stratified by visceral fat rating (VFR) and compare associations between HRV and body composition metrics. METHODS: A cross-sectional study was conducted on healthy men (n=99,age=37.8±13.4 years, body mass index [BMI]=26.9±4.6 kg/m2). HRV was derived from 5-minute electrocardiographic recordings. Body composition (body fat percentage, VFR, and muscle mass to visceral fat ratio [MMVFR]) was estim…

Validity of heart rate and ratings of perceived exertion as indices of exercise intensity in a group of children while swimming.

The purpose of this study was to investigate the validity of heart rate (fc) and ratings of perceived exertion (RPE) as indices of exercise intensity in a group of children while swimming. Six healthy male swimmers, aged 10-12, swam tethered using the breast-stroke in a flume. The resistance started at 1.0 kg and increased in 1.0 kg steps up to the point of their exhaustion. The subjects swam for 5 min during each period, with a rest of 10-20 min until they had returned to their resting fc level. The last exercise intensity was with the maximal mass the subjects could support for 2 min. The la…

Influence of socioeconomic status on rates of advanced heart failure therapies.

BACKGROUND: Socioeconomic deprivation is associated with a lower likelihood of referral for advanced heart failure (HF) evaluation, but it is not known whether it influences rates of advanced HF therapies independently of key hemodynamic measures and comorbidity following advanced HF evaluation in a universal healthcare system. METHODS: We linked data from a single-center Danish clinical registry of consecutive patients evaluated for advanced HF with patient-level information on socioeconomic status. Patients were divided into groups based on the level of education (low, medium, and high), co
