Assessment of Small Paediatric Burns: A Coin-Based System.

A review of paediatric burns in our burns facility in the United Kingdom demonstrated variable accuracy of size, and a majority documented as <1% total body surface area (TBSA). Accurate assessment is important for medical records, clinical management and non-accidental injuries. We propose to assess burn size with a coin-based system, where small burns are described by single/multiple sterling coins. Participants were asked about their confidence in evaluating small paediatric burns. Participants were given ten scenarios which included photographs of paediatric patients with small burns. They

Enhancing Learning Health Systems in Primary and Community Care: Contributions of NHS Library and Knowledge Specialists.

CONTEXT: Learning communities driving the data-knowledge-practice learning cycle are at the core of learning health systems (LHS). With the right team in place, including knowledge management expertise, LHS accelerates improvements, delivering on the healthcare quadruple aim and health equity. Within the National Health Service (NHS) in England, knowledge management and librarian expertise exist mainly within acute hospital settings. This makes it harder for those developing LHS in community health settings to benefit from this expertise. EXPERIENCE REPORT METHODS AND RESULTS: Three proof-of-

The Baseline Self-Efficacy Scores to Manage Chronic Diseases Amongst Nigerian Patients with Non-Communicable Diseases are Sub-Optimal.

Nigeria is one of the developing countries with a major burden of non-communicable diseases (NCDs) without a disease self-management program in its healthcare system. Thus, this study was aimed to assess the baseline self-efficacy levels of Nigerian patients with chronic NCDs at tertiary level hospitals. This is a cross sectional study of 286 NCD patients attending tertiary level clinics from 26 July to 27 October 2023. Patients were interviewed on a 10 Self-Efficacy to Manage Chronic Disease (SEMCD) item scales that were considered suitable for the Nigerian population. The 10 SEMCD questions

Results of Lung Cancer Screening in the Community.

PURPOSE: To address doubts regarding National Lung Screening Trial (NLST) generalizability, we analyzed over 6,000 lung cancer screenings (LCSs) within a community health system. METHODS: Our LCS program included 10 sites, 7 hospitals (2 non-university tertiary care, 5 community) and 3 free-standing imaging centers. Primary care clinicians referred patients. Standard criteria determined eligibility. Dedicated radiologists interpreted all LCSs, assigning Lung Imaging Reporting and Data System (Lung-RADS) categories. All category 4 Lung-RADS scans underwent multidisciplinary review and manageme

The crucial role of non-specialty local hospitals and clinics in managing advanced stages of adult congenital heart disease in an era of increasing prevalence.

A middle-aged woman with complex congenital heart disease (CHD) and stage D heart failure-including a double outlet univentricular heart-faced difficulty accessing a specialised centre due to dyspnoea, frailty and geographic distance. Despite attempting to seek care at local hospitals and clinics, she was turned away due to the absence of specialists in adult CHD (ACHD). Over time, our local tertiary care hospital, though lacking the expertise to care for her ACHD initially, coordinated a gradual transition of care from the ACHD centre to our facility by overlapping services and addressing bot

From shunt dependency to shunt freedom: Endoscopic third ventriculostomy in ventriculoperitoneal shunt-dependent communicating tetraventricular hydrocephalus: A case report.

BACKGROUND: Hydrocephalus represents pathological dilatation of the ventricular system caused by impaired cerebrospinal fluid (CSF) circulation and absorption, resulting in increased intracranial pressure. Ventriculoperitoneal shunting (VPS) remains the standard treatment for communicating hydrocephalus; however, it is associated with significant long-term complications, including infection and mechanical failure. Endoscopic third ventriculostomy (ETV), traditionally indicated for obstructive hydrocephalus, has increasingly been explored in selected cases of complex and communicating hydroceph

Skin infections in Australian Aboriginal children: a narrative review.

Impetigo, scabies, cellulitis and abscesses are common in Australian Aboriginal children. These conditions adversely affect wellbeing and are associated with serious long term sequelae, including invasive infection and post-infectious complications, such as acute post-streptococcal glomerulonephritis and acute rheumatic fever, which occurs at the highest documented rates in the world in remote Aboriginal communities. Observational research in remote communities in northern Australia has demonstrated a high concurrent burden of scabies and impetigo and their post-infectious complications. Few d

Complex autonomic dysfunction in cardiovascular, intensive care, and schizophrenic patients assessed by autonomic information flow.

BACKGROUND: The cardiovascular control system is mediated by mechanisms acting at different time scales, such as heart period, vagal, sympathetic, and other slower controllers. Since these elements are interrelated in a complex manner, classical control theory fails and information-based description, based on autonomic information flow (AIF) functions, is appropriate. We investigated the hypothesis that AIF functions of typical time scales specifically characterize autonomic dysfunction and prognosis. MATERIALS AND METHODS: Holter recordings of patients with multiple organ dysfunction syndrom

Efficacy and economic impact of a WeChat-based integrated care model for pulmonary rehabilitation in COPD: a retrospective propensity score matched study.

BACKGROUND: Pulmonary rehabilitation is essential for patients with chronic obstructive pulmonary disease (COPD). However, pulmonary rehabilitation is often underutilized in COPD patients, primarily due to fragmented healthcare delivery systems and suboptimal patient compliance, contributing to recurrent hospital readmissions with a substantial economic healthcare system burden. We aimed to evaluate the impact of a novel WeChat-facilitated hospital-community-family integrated rehabilitation model on clinical outcomes and healthcare resource utilization in moderate to severe COPD patients. A Transition of Care Service to Reduce Hospital Readmissions for High-Risk Cardiology Patients (RECARD): Protocol for a Pre-Post Interventional Trial.

BACKGROUND: Transition of care (TOC) is a high-risk period for medication errors as patients move between multiple health care professionals, settings, and changing medication regimens. Ineffective communication systems further increase the likelihood of medication harm and hospital readmission. A TOC service with tailored, patient-specific support may help reduce these risks. Cardiovascular medicines significantly contribute to medication harm, and patients recovering from acute myocardial infarction (AMI) or cardiothoracic surgery (CS) are vulnerable due to complex regimens and frequent medi

Improving Pneumococcal Vaccination Rates During Hospitalization via Resident Education Intervention: A Hospital-Based Quality Improvement Project in a Teaching Community Hospital.

BACKGROUND: Pneumococcal vaccination coverage among eligible adults in Japan remains suboptimal. This gap may be partly attributable to insufficient physician and patient education, as well as limited coordination in delivery of preventive care. Therefore, we conducted a hospital-based quality improvement project combining resident education with system-level workflow modifications to enhance pneumococcal vaccination rates. METHODS: A single-center quality improvement study was conducted at a teaching community hospital in Japan, comparing outcomes between pre-intervention and intervention pe

Impact of prior metabolic surgery on outcomes after hospitalization for acute COPD exacerbation: a nationwide analysis.

BACKGROUND: Obesity commonly coexists with chronic obstructive pulmonary disease (COPD) and exacerbates disease severity through impaired respiratory mechanics and systemic inflammation. Metabolic surgery has shown durable metabolic and pulmonary benefits, yet its impact on inpatient outcomes during acute COPD exacerbation admission remains unclear. OBJECTIVES: To evaluate the association between prior metabolic surgery and inpatient outcomes among adults hospitalized for acute exacerbation of COPD (AECOPD). Longitudinal trends in community-onset bacteraemia caused by ceftriaxone non-susceptible Escherichia coli, Proteus mirabilis, Klebsiella oxytoca, and Klebsiella pneumoniae (2016-2024).

BACKGROUND: The increasing prevalence of ESBL-producing Enterobacterales complicates treatment and worsens outcomes. However, contemporary data describing the prevalence and temporal trends in established risk factors for community-onset ceftriaxone non-susceptible (CRO-NS) bacteraemia remain limited. METHODS: This retrospective cohort study included adults with community-onset bacteraemia due to Escherichia coli, , Klebsiella pneumoniae, Klebsiella oxytoca, or Proteus mirabilis, evaluated at Phoebe Putney Health System (Albany, GA) between January 2016 and November 2024. Contraceptive use and maternal mortality in Indonesia: a community-level ecological analysis.

BACKGROUND: Prior studies have shown that contraceptive use reduces maternal mortality independently of other maternal health services. The present study took advantage of geographically detailed Indonesian data to study the interplay between contraceptive use and other risk and protective factors for maternal mortality at the community level, a level of analysis where the protective effects of family planning can be best understood. METHODS: Data from the 2015 Intercensal Population Survey (SUPAS) and the 2014 Village Potential Survey (PODES) were used to construct a series of census block-l

Mapping chronic illness in the age of globalization: reclaiming the good for the chronically ill.

Until recently, infectious diseases were the main cause of death worldwide. New medical discoveries and the evolution of public health improved life expectancy and the ability to survive acute threats, thus changing the course of diseases from acute to chronic. Today, chronic illness is the most important health concern worldwide. Chronic illness increases existing poverty and pushes other people into it. As nurses, members of the healthcare system and members of this world, we cannot forget that our response toward globalization and chronic disease has to be centered in leadership through reo

Setting up the first human milk bank in Uganda: a success story from Nsambya hospital.

BACKGROUND: The World Health Organization (WHO) strongly recommends the use of donor human milk (DHM) for low birth weight infants when mother's own milk is unavailable or insufficient. However, the use of DHM requires the availability of human milk banks (HMBs), the majority of which are in middle and high-income countries. Developing countries offer multiple opportunities and challenges regarding the establishment and operationalization of HMBs. This study describes the experience in setting up the HMB in Uganda at St. Francis Hospital Nsambya. METHODS: The establishment of the first HMB in
