[Normal pressure hydrocephalus--a rare late complication of radiography of the spine using contrast media].

The aim of the study is to inform professionals about a serious complication--the liquor malabsorption--developing in relation to the routinely performed roentgen-contrastive examination (PMG). It also demonstrates advantages of the lumbal infusion test (LIT) as a part of the algorithm of the shunt operation in patients with supposed pathology of liquor malabsorption.

Diagnostic in normal pressure hydrocephalus: A mathematical model for determination of the ICP-dependent resistance and compliance.

The internationally accepted calculation methods concerning cerebrospinal fluid dynamics proceed from a pressure independent resistance to cerebrospinal fluid outflow. In a new model we focus our attention on the pressure dependency of resistance. In our calculation model we are monitoring the complete pressure course p(t) over the time during and after the infusion. The comparison of the pressure rise On(p) during the infusion and the descent Off(p) after the infusion at the same pressure level allows one to construct all formulas for the compliance C(p) and resistance R(p). The computerized

Arachnoid cyst in the region of the fourth ventricle mimicking clinical picture of normal pressure hydrocephalus.

Arachnoid cysts in the region of the fourth ventricle are rarely reported. Two cases with intraventricular arachnoid cysts (one in the fourth and one in the right lateral) manifesting normal pressure hydrocephalus have been described in the previous literature. Here we report a clinically similar case in which the cyst was wedged in cerebellar vermis and compressed the fourth ventricle. Slow enlargement of the cyst could explain the clinical picture. The removal of the cyst resulted in a marked improvement of the symptoms despite the lack of significant changes in ventricular size.

Differentiation of normal pressure hydrocephalus and cerebral atrophy by computed tomography and spinal infusion test.

The diagnostic value of computed tomography (CT) and spinal infusion test (SIT) was investigated in 27 patients with normal pressure hydrocephalus (NPH) and 35 patients with cerebral atrophy. The most consistent CT finding of NPH was dilatation of the temporal horns, that of cerebral atrophy widening of the convexity sulci. However, 43% of patients with cerebral atrophy demonstrated no cortical atrophy. The SIT showed an excellent relation with isotope cisternography and continuous intracranial pressure recording. NPH and cerebral atrophy were correctly differentiated in 71% by CT and SIT. A n

Weight and Abdominal Pressure-Induced Shunt Trouble in Patients With Shunted Normal Pressure Hydrocephalus: A Comprehensive Study on Pressure Environment of Shunt System.

OBJECTIVES: We identified a new type of shunt malfunction (SM) in patients with normal pressure hydrocephalus (NPH). It is induced by weight change and can be treated with valve readjustment. There were two types of SM as follows: Underdrainage induced by the weight gain and overdrainage induced by the weight loss. This study aims to elucidate this mechanism by assessing the shunt pressure environment. METHODS: The total pressure environment of the shunt system was prospectively studied in patients with shunted NPH at Osaka Medical College Hospital from 1999 to 2005. We measured the pressure

Apathy in normal pressure hydrocephalus: frequency, clinical correlates, and relationship to cerebrospinal fluid removal response.

Apathy is a highly prevalent neuropsychiatric feature of normal pressure hydrocephalus (NPH), but its clinical correlates and relevance to cerebrospinal fluid (CSF) removal trial response are poorly understood. We evaluated 161 adults with possible or probable NPH whose informants completed the Frontal Systems Behavior Scale (FrSBe), which provides normative ratings for premorbid (i.e. before symptom onset) and current apathy. Apathy change was defined as current minus premorbid ratings of apathy. Among the sample, 69.6% showed clinically elevated apathy (T-score ≥ 65) compared to 18.0% premor

Electroconvulsive Therapy Ameliorates Neuropsychiatric Symptoms in an Older Adult Patient With Nonshunted Normal Pressure Hydrocephalus.

We report an 80-year-old male with nonshunted normal pressure hydrocephalus (NPH) presenting with an episode of recurrent major depressive disorder who underwent electroconvulsive therapy (ECT) with good effect and consequent amelioration of neuropsychiatric symptoms. NPH had been diagnosed several years prior, although a shunt was not pursued. On admission, it proved initially challenging to separate a mood disorder from behavioral and psychological symptoms of dementia (BPSD), as is the case for many patients with NPH. Following the diagnosis of recurrent major depressive disorder, the patie

Whole-brain connectome analysis for elucidating specific structural neural networks in idiopathic normal-pressure hydrocephalus.

OBJECTIVE: To test the hypothesis that idiopathic normal-pressure hydrocephalus (iNPH) is also associated with reproducible whole-brain structural connectivity reductions detectable by diffusion MRI connectomics, beyond the established clinical triad of gait disturbance, cognitive impairment, and urinary incontinence. MATERIALS AND METHODS: Twenty patients with iNPH (71-84 years; Evans index 32.1-43.5) diagnosed according to the Japanese guidelines and 20 age-matched healthy controls (70-88 years) underwent 3.0-T MRI. A Coronal Hydrocephalic Index (Eo Index) for Predicting Shunt Responsiveness in Idiopathic Normal Pressure Hydrocephalus.

The cerebrospinal fluid tap test is commonly used to select shunt candidates in idiopathic normal pressure hydrocephalus; however, it is invasive and has limited predictive value, as a substantial proportion of patients who tested positive on tap test do not improve after shunt surgery. This limitation highlights the need for noninvasive imaging markers to support preoperative decision-making. We retrospectively analyzed 80 patients with ventriculomegaly evaluated at a single center between 2014 and 2022, including 40 patients with idiopathic normal pressure hydrocephalus who tested positive o

Dusting off the personal archives of Salomón Hakim: unveiling the mind behind normal pressure hydrocephalus.

Dr. Salomón Hakim (1922-2011), a Colombian physician of Lebanese descent, is widely known for leading a paradigm shift in neurosurgery. In a landmark contribution to neurological medicine, he was the first to identify and characterize a previously unrecognized clinical entity now known as normal pressure hydrocephalus, or Hakim's syndrome, marking a pivotal moment in modern understanding of the disease. Dr. Salomón Hakim was inspired by his father and, since childhood, increasingly curious about physics and the natural sciences. This curiosity led him to develop abilities that would later cont

Beyond Hakim's triad: a systematic review of atypical clinical manifestations in idiopathic normal pressure hydrocephalus.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is characterized by Hakim's triad of gait disturbances, cognitive decline, and urinary incontinence. However, atypical presentations complicate diagnosis and remain poorly understood. This systematic review aimed to comprehensively analyze atypical symptoms of iNPH and their response to cerebrospinal fluid (CSF) diversion therapy. METHODS: Following PRISMA guidelines, a systematic search was conducted on PubMed using the keyword "normal pressure hydrocephalus symptoms." Articles from 1914 to 2024 were screened. Causal relationship between Baff-R expression and normal pressure hydrocephalus: insights from Mendelian randomization analysis.

OBJECTIVES: Previous studies have suggested a possible link between normal pressure hydrocephalus (NPH) and immune factors, but the causal relationship between NPH and Baff-R expression on immune cells remains enigmatic. This study used a Mendelian randomization (MR) method to elucidate this association. METHODS: The study used data from the Finngen genome-wide association study (GWAS) and included a large European cohort of 767 patients with NPH and 375,610 controls. Baff-R genetic results in 3,757 individuals of European ancestry had 22 Baff-R-related traits. Different MR techniques were us

Single-cell elderly blood-CSF atlas implicates peripherally influenced immune dysregulation in normal pressure hydrocephalus.

We have generated a single-cell RNA sequencing atlas of peripheral blood and ventricular CSF in idiopathic normal pressure hydrocephalus (iNPH) patients totaling 140,207 single-cell transcriptomes. We found proinflammatory alterations in peripheral blood and CSF monocytes in iNPH patients with lower baseline cognitive function. We also identified CSF cell populations likely representing periventricular sloughing of degenerating neuroglial cells. Our findings suggest possible immune dysregulation in the blood and CSF of iNPH patients.

# Language processing characteristics in normal pressure hydrocephalus: insights from eye-tracking analysis of incorrect responses.

INTRODUCTION: In patients with Normal Pressure Hydrocephalus (NPH), the aging process results in decreased efficiency of cerebrospinal fluid circulation, leading to ventricular enlargement. This enlargement compresses several brain structures, impairing functions such as visual perception, semantic memory processing, and phonological encoding contributing to language processing difficulties. This study examines real-time language processing in NPH patients and healthy elderly (HE) controls by comparing their eye movements during correct and incorrect responses in a lexical retrieval task (LRT)

Idiopathic normal pressure hydrocephalus: associations between CSF biomarkers, clinical symptoms, and outcome after shunt surgery.

BACKGROUND: The neurochemical alterations in cerebrospinal fluid (CSF) associated with the typical symptomatology in idiopathic normal pressure hydrocephalus (iNPH) and their association with outcome after shunt surgery are unsettled. AIM: To explore associations between concentrations of CSF biomarkers reflecting amyloid- and tau pathology, neuronal degeneration as well as astrocytic activation and the characteristic symptomatology in iNPH and to examine whether these biomarkers can predict the postoperative outcome in all patients and in patients without evidence of Alzheimer's disease (AD)

Instrument-supported gait analysis characterizes gait domain changes in patients with suspected normal pressure hydrocephalus.

BACKGROUND: Idiopathic Normal Pressure Hydrocephalus (iNPH) is a potentially reversible cause of cognitive impairment, urinary incontinence, and gait disturbances, which typically present with a characteristic slow, shuffling, and wide-based gait. Gait velocity, which is reduced relative to healthy controls, improves in iNPH patients following a spinal tap test. This study aimed at evaluating the criterion of a 20% gait velocity improvement in the 10 m walk test to identify responders and non-responders in a cohort of patients with probable iNPH receiving a spinal tap test as well as the added

Identifying differential brain structures and genetic mechanisms between Alzheimer's disease and idiopathic normal pressure hydrocephalus.

BackgroundIdiopathic normal pressure hydrocephalus (INPH) is a reversible neurological disorder presenting with cognitive decline, urinary incontinence, and gait disturbance, yet it is often misdiagnosed as Alzheimer's disease (AD) due to overlapping features. Magnetic resonance imaging (MRI) highlights structural differences, but their causal links to disease manifestations remain unclear.ObjectiveTo investigate the causal relationships between brain structures and INPH/AD through Mendelian randomization (MR) and to explore genetic mechanisms underlying structural variations.MethodsWe analyze
