Fluid and electrolyte problems in patients with advanced carcinoma.

Brief outlines of these problems serve to suggest some etiologic factors in their production. The patient seldom presents with a single or simple cause or solution. The body reacts with compensations or side effects, and suffers complications and mixed disorders -- all of which probabilities must be investigated and appreciated before rational therapy can be initiated.

[Characterization of full term newborns with hypernatremic dehydration].

INTRODUCTION: The hypernatremic neonatal dehydration is a severe condition whose incidence has increased in recent years resulting in compli … crease in the first 24 hours of handling was 7.74 mEq/L (0.32mEq/L per hour). The correction of the hypernatremia was 55.8% by oral intake and 4 days hospital stay on average. CONCLUSIONS: The feeding's problems came up in a (76%), primiparous mothers in an (88.4%). 90.6 percent of this population administered exclusive breastfeeding, results that can help to …

Avoiding common problems associated with intravenous fluid therapy.

Inappropriate intravenous fluid therapy is a significant cause of patient morbidity and mortality and may result from either incorrect volume (too much or too little) or incorrect type of fluid. Fluid overload has no precise definition, but complications usually arise in the context of pre-existing cardiorespiratory disease and severe acute illness. Insufficient fluid administration is readily identified by signs and symptoms of inadequate circulation and decreased organ perfusion. Administration of the wrong type of fluid results in derangement of serum sodium concentration, which, if severe

Central diabetes insipidus: a rare perioperative cause of severe hypernatraemia.

In this case report we describe the management of severe hypernatraemia following inadvertent water restriction. A 21-year-old woman with no reported medical history presented on transfer from an outside hospital with a complex volar upper extremity injury. Management both operatively and postoperatively involved a prolonged period of fasting which limited her access to drinking water Collateral history revealed that she had previously drunk copious amounts of water during the course of any given day and this had served to alleviate the dramatic symptoms of hypernatraemia that were rapidly man

Hypernatraemia and rhabdomyolysis.

A 44-year-old man with a history of childhood brain injury presented with dysarthria, confusion, reduced oral intake and reduced mobility after a week of heatwave conditions. He had severe hypernatraemia and raised serum creatine kinase levels, consistent with rhabdomyolysis. In most previous case reports linking hypernatraemia and rhabdomyolysis, other factors have potentially contributed. From the available evidence, severe hypernatraemia alone appears sufficient to induce muscle injury.

Reference chart for relative weight change to detect hypernatraemic dehydration.

OBJECTIVE: The validity of the rule of thumb that infants may have a weight loss of 10% in the first days after birth is unknown. We assessed the validity of this and other rules to detect breast-fed infants with hypernatraemic dehydration. DESIGN: A reference chart for relative weight change was constructed by the LMS method. The reference group was obtained by a retrospective cohort study. PARTICIPANTS: 1544 healthy, exclusively breast-fed infants with 3075 weight measurements born in the Netherlands and 83 cases of breast-fed infants with hypernatraemic dehydration obtained from literatur

Serum sodium concentration and intraventricular hemorrhage in premature infants.

Recent data suggest that early loss of brain tissue water content, ie, decreased extravascular cerebral tissue pressure, may play a role in the pathogenesis of germinal matrix/intraventricular hemorrhage in the premature newborn. This study examines the relationship between the concentration of serum sodium and germinal matrix/intraventricular hemorrhage in 299 premature infants with birth weights of less than 1500 g during the first 4 days of life. Intraventricular hemorrhage developed in 34 (32%) of the 106 infants with maximum serum sodium levels of 145 mmol/L or less and in 54 (28%) of 193

Hypernatraemia, diabetes mellitus, hyperprolactinaemia, retarded growth and delayed puberty in a 14 year old girl. Effect of bromocriptine treatment.

Investigations in a 14 year old girl with arrested growth for 2 years, delayed pubertal development, hypernatraemia without thirst, diabetes mellitus and hyperlipaemia are reported. The hypernatraemia was accompanied by a low vasopressin concentration with an abnormal response to thirst, high plasma renin but normal plasma aldosterone concentrations. Treatment with vasopressin and increased fluid intake decreased serum sodium levels. Serum gonadotrophins were low; GH response during an insulin tolerance test was subnormal and basal serum Prl concentration was elevated. Bone age, thyroid functi

Isosorbide in treatment of infantile hydrocephalus.

This paper reports the experiences of the second clinical trial in the use of isosorbide in the treatment of 34 selected cases of infantile hydrocephalus of all types. Subject to careful biochemical monitoring of serum electrolyte, urea, and acid-base balance, treatment with 2 g/kg body weight 6-hourly is safe. Side effects are immediately eliminated by interrupting therapy. With lower dosage, prolonged maintenance therapy was possible, for as long as 11 months, without side effects and with need for much less frequent biochemical monitoring. Isosorbide effectively prevented the need for shunt

The Association Between Serum Sodium Variation During Pediatric Intensive Care Unit Admission and the Risk of Clinical Outcomes in Critically Ill Children: A Retrospective Cohort Study.

The PICU-acquired hypernatremia (OR:5.23;95%CI:1.72-15.92) and mixed dysnatremia (OR:2.89;95%CI:1.14-7.29) had significantly higher odds for PICU mortality. Even mild sodium fluctuations (4-8mmoL/L) during PICU admission were significantly associated with higher odds of prolonged …

Multicentre pragmatic embedded stepped wedge cluster randomised trial comparing glucose 5% with sodium chloride 0.9% as the default drug diluent in the ICU: the sweet-water trial protocol.

INTRODUCTION: Hypernatraemia, defined as a plasma sodium concentration >145 mmol/L, is a frequent complication in critically ill patients treated in the intensive care unit (ICU) (= ICU-acquired hypernatraemia), with reported prevalence ranging from 4% to 26%. Hypernatraemia adversely affects various physiological functions and is associated with delirium, prolonged length of stay and increased ICU and post-discharge mortality. The sodium load from intravenous drug diluents significantly contributes to ICU-acquired hypernatraemia, with drug infusions comprising about 30% of the daily fluid vol
