Early biochemical response to parathyroidectomy for primary hyperparathyroidism and its predictive value for recurrent hypercalcemia and recurrent primary hyperparathyroidism.

BACKGROUND: The traditional definition of cure after parathyroidectomy (PTX) for primary hyperparathyroidism is normocalcemia. Our hypothesis was that early postoperative levels of serum calcium and parathyroid hormone after PTX would have predictive value for later recurrence. METHODS: We performed a retrospective study of 1,146 patients with primary hyperparathyroidism who underwent PTX and had long-term biochemical follow-up. The first postoperative serum level of calcium and parathyroid hormone values were used to categorize patients into the following four early biochemical response grou…

Unanticipated remission of primary hyperparathyroidism following cinacalcet.

Primary hyperparathyroidism (PHPT) is the most common etiology of hypercalcemia in an ambulatory setting and often arises from adenomatous growth of one (occasionally more than one) parathyroid gland. Patients with severe and symptomatic hypercalcemia, renal insufficiency, nephrolithiasis, osteoporosis, and younger than 50 are candidates for surgical removal of parathyroid adenoma(s). Medical treatment with a calcimimetic agent is offered to improve severe hypercalcemia when surgery is refused or considered unsafe. Here, we report a 68-year-old man with severe hypercalcemia due to PHPT and cor…

Hyperparathyroidism: historical milestones and modern therapeutic strategies.

The Viennese Medical School was at the forefront of the clinical understanding of the relationship between parathyroid and bone metabolism. A century ago, F. Mandl (1925) and E. Gold (1927) described two patients clinically (osteitis fibrosa cystica) and biochemically (hypercalcemia, hypercalciuria) with a disease which Gold had already termed (primary) hyperparathyroidism (PHPT) in 1928. The two patients were successfully treated by removing one enlarged parathyroid gland each. After the clinical and biochemical diagnosis was confirmed, surgery was recommended. In 1933, Mandl summarized his r…

Hyperparathyroidism-Jaw Tumor Syndrome: A Rare Case.

Hyperparathyroidism-jaw tumor (HPT-JT) syndrome is an uncommon autosomal dominant disorder associated with primary hyperparathyroidism due to parathyroid adenomas and ossifying jaw tumors. We present the case of a 31-year-old male who was incidentally diagnosed with severe parathyroid hormone-dependent hypercalcemia following a traumatic upper limb fracture. Subsequent evaluation confirmed the diagnosis of HPT-JT syndrome. Early recognition, prompt surgical management, and genetic confirmation are essential to ensure appropriate treatment and prevent long-term complications.

Audiovestibular Outcomes After Parathyroidectomy in Primary Hyperparathyroidism.

OBJECTIVE: To prospectively evaluate audiovestibular outcomes following parathyroidectomy in patients with primary hyperparathyroidism (pHPT) using a comprehensive battery of audiological, vestibular, and patient-reported outcome measures. METHODS: This prospective observational study included 31 adult patients undergoing parathyroidectomy for pHPT caused by parathyroid adenoma. Assessments were performed preoperatively and at prespecified postoperative time points: audiometry at month 1; vHIT, VNG, and VDI at week 1 and month 1; and SVV on day 2, week 1, and month 1. The primary outcome was …

Heritable hyperparathyroidism: Genetic insights and clinical implications.

Familial or heritable hyperparathyroidism (FHPT) is seen in approximately 10-15 % of patients with primary hyperparathyroidism (PHPT). Once the diagnosis of PHPT is established, consideration of heritable forms should be made in patients with positive family history, young onset, multi-glandular disease, and recurrent or persistent disease. FHPT encompasses both syndromic and non-syndromic forms. Syndromic forms include multiple endocrine neoplasia (MEN) types 1, 2, 3 and 4, hyperparathyroidism-jaw tumor syndrome, hereditary pheochromocytoma and paraganglioma, and the more recently reported, B…

Persistent hyperparathyroidism after kidney transplantation in children.

BACKGROUND: Persistent hyperparathyroidism after kidney transplantation (KT) has been reported in up to 50% of adult recipients, but pediatric data remain limited. We evaluated the prevalence, skeletal manifestations, and risk factors for persistent hyperparathyroidism in children following KT. METHODS: In this retrospective cohort study, 107 pediatric KT recipients (58% male; median age 10.3 years) transplanted between 2004 and 2019 were analyzed. Persistent hyperparathyroidism was defined as a median parathyroid hormone (PTH) > 65 pg/mL between 3 and 12 months post-KT. Risk factors for pers…

Chapter 4: Differential diagnosis of primary hyperparathyroidism.

The differential diagnosis of primary hyperparathyroidism can be considered clinically, biologically and radiologically. Clinically, primary hyperparathyroidism should be suspected in case of diffuse pain, renal lithiasis, osteoporosis, repeated fracture, cognitive or psychiatric disorder, or disturbance of consciousness. Nevertheless, the differential diagnosis of primary hyperparathyroidism is mainly biological, particularly in atypical forms, which must be differentiated from hypercalcemia with hypocalciuria or non-elevated PTH on the one hand, and from normo-calcemia with elevated PTH, hyp…

Complex Primary Hyperparathyroidism: Hereditary and Recurrent Disease.

Primary hyperparathyroidism can be sporadic or part of a genetic syndrome, such as MEN1 or HPT-JT. Diagnosis of hereditary HPT requires a thorough history and physical. Parathyroidectomy is curative with greater than 95% success. However, some patients have persistent or recurrent disease requiring reoperation. Reoperative parathyroidectomy is technically challenging, and localizing the pathologic gland can difficult. Patients needing reoperation should undergo evaluation by a high-volume surgeon. Care should be taken to obtain all of the preoperative workup and operative note from the initial…

Hyperparathyroidism Jaw Tumor Syndrome, an Unforeseen Diagnosis.

Asymptomatic primary hyperparathyroidism (PHPT) is often missed in developing nations due to limited formal healthcare exposure and biochemical screening programs. Many patients are thus only diagnosed once symptomatic. We present a 32-year-old female who developed bony protrusions in her jaw during pregnancy, resulting in a stillbirth. Three months later, during a dental consultation for worsening toothache, jaw abnormalities were detected. Radiological studies revealed bilateral mandibular radiolucent lesions, and bone biopsy confirmed histological features consistent with a brown tumor. The…

Primary hyperparathyroidism: diagnostic features and surgical outcomes.

BACKGROUND: Primary hyperparathyroidism is characterized by elevated plasma calcium levels due to inappropriate secretion of parathyroid hormone (PTH) in most cases by an adenomatous or hyperplastic parathyroid. We present a retrospective analysis of a large cohort of patients operated on of parathyroidectomy in our center analyzing their diagnostic characteristics, intraoperative match and surgical outcomes. METHODS: We included patients with benign parathyroid disease who underwent parathyroidectomy associated or not with hemi- or total thyroidectomy at the Sant'Anna University Hospital of …

Primary hyperparathyroidism: from guidelines to outpatient clinic.

Primary hyperparathyroidism (PHPT) is a common endocrine disease characterized by hypercalcemia due to inappropriately high parathyroid hormone secretion. While in the typical, symptomatic form of the disease diagnosis is set easily and standard management is surgical removal of the hyperfunctioning parathyroid (HP), this may not be the case in more subtle forms of PHPT, such as the asymptomatic and the normocalcemic PHPT. Localization of the HP could also be challenging, especially in small-sized adenomas, ectopic lesions or multiglandular disease. An experienced surgical team is essential to…

[Sporadic primary hyperparathyroidism with multiple parathyroid adenomas].

Multiple lesions of the parathyroid glands (PTG) in primary hyperparathyroidism (PHPT) can be sporadic or develop as part of hereditary syndromes, manifesting at young age. There the description of a severe sporadic PHPT with big parathyroid neoplasms in the young patient is presented. Clinical data made it possible to suspect MEN-1 syndrome or parathyroid carcinomas; however, mutations CDKN, CDC73, MEN1 were excluded. The patient underwent removal of three identified tumors: benign adenomas of the left PTG and hyperplasia of the right one. Postoperative hypocalcemia and severe hungry bone syn…

Lithium-Induced Hyperparathyroidism: An Ill-defined Territory.

Lithium is the gold standard treatment for bipolar disorder. Studies have shown an association between lithium and hyperparathyroidism. However, there is limited data regarding the management of lithium-induced hyperparathyroidism. We present a clinical conundrum which physicians frequently encounter-an excellent lithium responder refractory to other treatments who developed lithium-induced hyperparathyroidism. Medical treatment with cinacalcet was successful in management of hyperparathyroidism without discontinuing lithium maintenance therapy.
