Measurement and reduction of occupational exposure to inhaled anaesthetics.

The occupational exposure of hospital staff to inhaled anaesthetics was investigated using a personal sampling device that provides a measure of the average concentrations breathed by a person over a period of time, as distinct from the spot sampling in the general environment. The anaesthetist's average exposure to nitrous oxide and halothane during complete operating sessions was twice that expected from simple dilution of the escaping gases by the operating room ventilation. The sampling technique was also used to evaluate the effect of (1) redirection of the waste gas outflow; (2) active s

Infection-Phase Biomolecular Coronas in Inhaled Nanomedicine.

An inhaled nanomedicine encounters two successive forms of remodeling: device-related changes during aerosol generation and biological conditioning after deposition. Respiratory infection adds a moving interface, as epithelial secretion, surfactant composition, plasma leakage, and neutrophil-derived material vary across airway compartments and over time. Pulmonary and disease-corona studies already establish that host environments influence nanoparticle identity. The next question is how this knowledge should change formulation selection and dosing for infected lungs. We propose an infection-p

Adherence to Inhaled Corticosteroids and Inflammatory Phenotypes in Asthma.

BACKGROUND AND AIM: Adherence to inhaled corticosteroids (ICS) is essential for asthma control, yet little is known about whether adherence differs across inflammatory phenotypes. Patients with type 2 (T2)-low biomarkers have reduced corticosteroid responsiveness, which may influence adherence. We aimed to evaluate adherence to ICS across inflammatory phenotypes and to examine whether associations between adherence and clinical risk factors differ by inflammatory phenotype. MATERIAL AND METHODS: This retrospective cohort study included adult asthma patients managed at a respiratory outpatient

Behaviour change intervention for inhaler disposal, Spain.

OBJECTIVE: To analyse metered-dose inhaler disposal behaviour in asthma patients, determine the patient characteristics that contribute to appropriate disposal and quantify the effect of a pharmacy-led educational intervention. METHODS: We conducted a multicentre cross-sectional study in 40 Spanish hospitals followed by a pre-post quasi-experimental evaluation. We surveyed adults with uncontrolled severe asthma receiving biological therapy to determine sociodemographic, health, environmental attitude and disposal behaviour characteristics. At the time of biological therapy dispensation, we pr

Modeling environmental inhalant exposure in rheumatoid arthritis.

The mucosal origins hypothesis posits that environmental inhalant exposures, including cigarette smoke (CS) and crystalline silica (c-silica), trigger immune responses in the lung mucosa, an extra-articular site, which precede initiating events of rheumatoid arthritis (RA) pathogenesis in distant joints. Epidemiological data strongly associates these exposures with RA risk, especially in genetically susceptible individuals carrying HLA-DRB1 alleles, and with the production of autoantibodies such as anti-citrullinated peptide antibodies (ACPA) and rheumatoid factor (RF). However, establishing c

Inhalational Agents as Intensive Care Unit Sedatives.

Inhaled anesthetics are gaining recognition as alternatives to intravenous sedatives in the intensive care unit. As sedatives for mechanically ventilated patients, they offer rapid onset, bronchodilation, and minimal systemic metabolism. Historically, volatile anesthetics were mostly used in refractory status asthmaticus or epilepticus using an anesthesia machine with a vaporizer. Sedation with the volatile agents can provide effective sedation and potentially promote faster extubation for mechanically ventilated patients. Although they offer unique advantages in this population, risks such as

Development of a System to Deliver Inhalational Antibiotics to Marmosets.

BACKGROUND: Inhalational antibiotics have been used effectively to treat chronic diseases such as Pseudomonas aeruginosa infections associated with cystic fibrosis. This approach may enhance treatment options for difficult-to-treat, acute pneumonic diseases. Liposomal encapsulated ciprofloxacin (Lipoquin and/or Apulmiq) has provided protection in murine models of plague, anthrax, Q fever and tularemia. Development of the ability to deliver these drugs to nonhuman primates (NHPs) would enable further extrapolation of the data observed in small animal models of infection to humans. METHODS: In …

The effects of airway disease on the deposition of inhaled drugs.

INTRODUCTION: The deposition of inhaled medications is the first step in the pulmonary pharmacokinetic process to produce a therapeutic response. Not only lung dose but more importantly the distribution of deposited drug in the different regions of the lung determines local bioavailability, efficacy, and clinical safety. Assessing aerosol deposition patterns has been the focus of intense research that combines the fields of physics, radiology, physiology, and biology. AREAS COVERED: The review covers the physics of aerosol transport in the lung, experimental, and in-silico modeling approaches

Caring for patients with advanced COPD: beyond the inhalers….

UNLABELLED: COPD affects millions of people worldwide. Patients with advanced COPD have a high symptom burden. Breathlessness, cough and fatigue are frequent daily symptoms. Guidelines often focus on pharmacological treatment, especially inhaler therapy, but other approaches in combination with medications offer symptomatic benefit. In this review, we take a multidisciplinary approach with contributions from pulmonary physicians, cardiothoracic surgeons and a physiotherapist. The following areas are addressed: oxygen therapy and noninvasive ventilation (NIV), dyspnoea management, surgical and

Poisonings from hydrocarbon inhalant misuse in Australia.

BACKGROUND AND AIMS: Inhalational misuse of volatile substances has been a significant public health concern because of the risk of sudden death and associated chronic complications such as encephalopathy. The Australian Government released a Consensus-based clinical practice guideline in 2011 on the management of volatile substance use in Australia, which noted a lack of available data particularly on harms. This study aimed to measure (1) the number of calls received by the New South Wales Poisons Information Centre (NSWPIC) regarding inhalational hydrocarbon exposures or poisonings and (2) …
