Experiences of surviving a cardiac arrest after therapeutic hypothermia treatment. An interview study.

BACKGROUND: Cardiac arrest (CA) is often associated with high mortality. In Sweden, it is reported that 13-52 per 100,000 people suffer out-of-hospital CA, and survival to one month is 2-14%. OBJECTIVE: This study aimed to describe people's experiences of surviving a CA after therapeutic hypothermia treatment. METHOD: A descriptive qualitative design was used. Data were collected through individual interviews with seven CA survivors. The collected data were analyzed using qualitative content analysis. RESULTS: The analysis resulted in six subthemes and three emerging themes. The themes were

The influence of intrapartum opioid use on breastfeeding experience at 6 weeks post partum: A secondary analysis.

OBJECTIVE: To examine breastfeeding experiences up to 6 weeks postpartum for mothers administered intranasal fentanyl, subcutaneous fentanyl or intramuscular pethidine for intrapartum analgesia. DESIGN: A secondary analysis was undertaken using the per-protocol dataset to examine the third phase of a larger randomised controlled trial. This phase of the study examined breastfeeding intention and experience from the first hour of birth to 6 weeks postpartum. Medical records were audited and women were contacted at 6 weeks postpartum to complete a telephone questionnaire. SETTING: Two maternit

Responding to ACEs With HOPE: Health Outcomes From Positive Experiences.

This article introduces a framework called "HOPE: Health Outcomes From Positive Experiences." The HOPE framework focuses on the need to actively promote positive childhood experiences that contribute to healthy development and well-being, as well as prevent or mitigate the effect of adverse childhood experiences and other negative environmental influences. Key positive childhood experiences fall within 4 broad categories: being in nurturing, supportive relationships; living, developing, playing, and learning in safe, stable, protective, and equitable environments; having opportunities for cons

How knowledge of adverse childhood experiences can help pediatricians prevent mental health problems.

INTRODUCTION: The purpose of this study was to gain a better understanding of the perspectives of low-income, minority primary caregivers to inform the design of pediatric interventions to prevent mental health problems. METHOD: The authors conducted a follow-up study of female primary caretakers of children ages 4-5 years old in an urban northeastern pediatric practice, including 5 mothers of children with low Child Adverse Childhood Experiences (C-ACE) scores and 13 mothers of children with high C-ACE scores. RESULTS: Participation in early speech therapy, preschool programs, or mental hea

Kindling of life stress in bipolar disorder: comparison of sensitization and autonomy models.

Research on life stress in bipolar disorder largely fails to account for the possibility of a dynamic relationship between psychosocial stress and episode initiation. The kindling hypothesis (Post, 1992) states that over the course of recurrent affective disorders, there is a weakening temporal relationship between major life stress and episode initiation that could reflect either a progressive sensitization or progressive autonomy to life stress. The present study involved a comprehensive and precise examination of the kindling hypothesis in 102 participants with bipolar II disorder that allo

Psychotic-Like Experiences and Nonsuicidal Self-Injury in England: Results from a National Survey [corrected].

BACKGROUND: Little is known about the association between psychotic-like experiences (PLEs) and nonsuicidal self-injury (NSSI) in the general adult population. Thus, the aim of this study was to examine the association using nationally-representative data from England. METHODS: Data from the 2007 Adult Psychiatric Morbidity Survey was analyzed. The sample consisted of 7403 adults aged ≥ 16 years. Five forms of PLEs (mania/hypomania, thought control, paranoia, strange experience, auditory hallucination) were assessed with the Psychosis Screening Questionnaire. The association between PLEs and

Childhood emotional and sexual maltreatment moderate the relation of the serotonin transporter gene to stress generation.

Emerging evidence suggests that the tendency to generate stressful life events may be, at least in part, genetically determined. However, the role of the early environment in shaping responses to later stressors is crucial to fully specifying biogenetic models of stress generation. The current study examined the moderating role of childhood emotional, physical, and sexual maltreatment on the relation of the serotonin-transporter-linked promoter region (5-HTTLPR) polymorphism of the serotonin transporter gene to proximal independent, dependent, and dependent-interpersonal life events. This ques

Adversity, cannabis use and psychotic experiences: evidence of cumulative and synergistic effects.

BACKGROUND: There is robust evidence that childhood adversity is associated with an increased risk of psychosis. There is, however, little research on intervening factors that might increase or decrease risk following childhood adversity. AIMS: To investigate main effects of, and synergy between, childhood abuse and life events and cannabis use on odds of psychotic experiences. METHOD: Data on psychotic experiences and childhood abuse, life events and cannabis use were collected from 1680 individuals as part of the South East London Community Health Study (SELCoH), a population-based househo

Predictors of the onset of depression in young children: a multi-method, multi-informant longitudinal study from ages 3 to 6.

BACKGROUND: Despite growing interest in depression in young children, little is known about which variables predict the onset of depression in early childhood. We examined a range of predictors of the onset of depression diagnoses in a multi-method, multi-informant longitudinal study of a large community sample of young children from ages 3 to 6. METHODS: Predictors of the onset of depression at age 6 were drawn from five domains assessed when children were 3 years old: child psychopathology (assessed using a parent diagnostic interview), observed child temperament, teacher ratings of peer fu

[Analysis of posttraumatic stress disorders in Polish soldiers who returned from stabilization mission in Iraq].

AIMS: The aim of the survey was to establish whether PTSD is present among Polish soldiers returning from a one-year deployment to Iraq and an analysis of its individual symptoms. METHODS: Sixty soldiers were examined, including 30 who returned from the Iraqi mission and 30 who remained in Poland. Five analysing devices were used: (IPSA), (STAI), (BDI), a PTSD questionnaire and a socio-demographical form. RESULTS: A significant number of soldiers experienced a traumatic event during the mission in Iraq. Although the Iraq deployment did not change the level of depression and anxiety among the

PTSD symptoms among police officers: associations with frequency, recency, and types of traumatic events.

Policing necessitates exposure to traumatic, violent and horrific events, which can lead to an increased risk for developing post-traumatic stress disorder (PTSD). The purpose of this study was to determine whether the frequency, recency, and type of police-specific traumatic events were associated with PTSD symptoms. Participants were 359 police officers from the Buffalo Cardio-Metabolic Occupational Police Stress (BCOPS) Study (2004-2009). Traumatic police events were measured using the Police Incident Survey (PIS); PTSD was measured using the PTSD Checklist-Civilian Version (PCL-C). Associa

Distinguishing primary and secondary variants of callous-unemotional traits among adolescents in a clinic-referred sample.

The current study used model-based cluster analyses to determine if there are 2 distinct variants of adolescents (ages 11-18) high on callous-unemotional (CU) traits that differ on their level of anxiety and history of trauma. The sample (n = 272) consisted of clinic-referred youths who were primarily African American (90%) and who came from low-income families. Consistent with hypotheses, 3 clusters emerged, including a group low on CU traits, as well as 2 groups high on CU traits that differed in their level of anxiety and past trauma. Consistent with past research on incarcerated adults and

Stress, mental health, and burnout in national humanitarian aid workers in Gulu, northern Uganda.

This study examined the mental health of national humanitarian aid workers in northern Uganda and contextual and organizational factors predicting well-being. A cross-sectional survey was conducted among 376 national staff working for 21 humanitarian aid agencies. Over 50% of workers experienced 5 or more categories of traumatic events. Although, in the absence of clinical interviews, no clinical diagnoses were able to be confirmed, 68%, 53%, and 26% of respondents reported symptom levels associated with high risk for depression, anxiety disorders, and posttraumatic stress disorder (PTSD), res

Reactivity of affect and self-esteem during remission in bipolar affective disorder: an experimental investigation.

BACKGROUND: Bipolar affective disorder (BPAD) is characterised by a lifelong vulnerability to develop episodes of depressed or elevated mood in response to stressful life events involving achievement or failure. We hypothesised that this latent vulnerability can manifest as reactivity of affect and self-esteem to experimentally induced experiences of success and failure and is shaped by history of childhood adversity. METHODS: Twenty-four people with remitted BPAD and twenty-four healthy controls underwent anagram-solving tasks designed to generate experiences of success and failure in two se

The role of stress on physiologic responses and clinical symptoms in irritable bowel syndrome.

Studies support the concept that irritable bowel syndrome (IBS) is a biopsychosocial disorder that can be explained by a neurobiological model which postulates stress-induced alterations in central stress and arousal circuits and activation of parallel motor outputs from brain regions that can affect bodily function and behavior. Sustained stress can result in chronic overactivity or underactivity of allostatic (or adaptive) systems, including the hypothalamic-pituitary-adrenal (HPA) axis, autonomic nervous system, metabolic, and immune systems, can occur. Animal and human studies have demonst

Auditory hallucinations in childhood: associations with adversity and delusional ideation.

BACKGROUND: Previous work suggests that exposure to childhood adversity is associated with the combination of delusions and hallucinations. In the present study, associations between (severity of) auditory vocal hallucinations (AVH) and (i) social adversity [traumatic experiences (TE) and stressful events (SE)] and (ii) delusional ideation were examined. METHOD: A baseline case-control sample of children with and without AVH were re-assessed on AVH after 5 years and interviewed about the experience of social adversity and delusions. RESULTS: A total of 337 children (mean age 13.1 years, S.D.

Auditory hallucinations and posttraumatic stress disorder within schizophrenia and substance abuse.

There is a high prevalence of traumatic events within individuals diagnosed with schizophrenia and of auditory hallucinations within individuals diagnosed with posttraumatic stress disorder (PTSD). However, the relationship among the symptoms associated with these disorders remains poorly understood. We conducted a multidimensional assessment of auditory hallucinations within a sample diagnosed with schizophrenia and substance abuse, both with and without comorbid PTSD. The results suggest a rate of comorbid PTSD similar to those reported in other studies. Patients who have comorbid PTSD repor

[Dream quality, trauma and suicide in in adjustment disorder].

In adjustment disorder (ICD: F43.2) the danger of suicide is greater, and specific dream quality may be characteristic of this state, too. Moreover adjustment disorder, suicide and quality of dream can be related to different types of trauma the patient had during life. Considering these aspects we examined with questionnaires 41 patients with adjustment disorder and 41 control persons with no diagnosed psychiatric disorder. Our results suggest that in adjustment disorder the danger of suicide is significant, nightmare and dreams with negative affect often occur. If these patients went through

Positive affect, intuition, and feelings of meaning.

Subjective rationality, or the feeling of meaning, was identified by William James (1893) as a central aspect of the non-sensory fringe of consciousness. Three studies examined the interaction of positive affect (PA) and individual differences in intuitive information processing in predicting feelings of meaning for various stimuli and life events. In Study 1 (N = 352), PA and intuition interacted to predict understanding for ambiguous quotes and abstract artwork. In Study 2 (N = 211), similar interactions were found for feelings of meaning for fans after their football team lost a conference
