Secondary pancreatic involvement by diffuse large B-cell lymphoma presenting as acute pancreatitis: treatment and outcome.

We report the case of a 33-year-old man who presented with a large B-cell non Hodgkin's lymphoma presenting as acute pancreatitis. Abdominal CT showed diffuse swelling of the pancreas, with two distinct masses in the corpus and the tail. Thoracic CT showed a markedly enlarged mediastinum, with a voluminous mass in the middle mediastinum. Direct biopsy of this mass revealed a large B-cell lymphoma. Chemotherapy followed by peripheral blood cell autotransplantation led to complete disappearance of the pancreatic and mediastinal masses. Fatty diarrhea occurred after chemotherapy, probably owing t

A comprehensive analysis of B symptoms reveals prognosis and heterogeneity across different sites of diffuse large B-cell lymphoma.

Diffuse large B-cell lymphoma (DLBCL) is a highly aggressive form of non-Hodgkin lymphoma. B symptoms, including fever, night sweats and weight loss, are often overlooked but carry prognostic significance. The primary site of DLBCL also plays a crucial role in diagnosis and treatment decisions, and our study aims to explore the relationship among primary sites of DLBCL, the presence of B symptoms and patients' survival. 15,267 patients' data were analyzed in our study to investigate the potential relevance among B symptoms, primary sites and patients' survival. We assessed the influence of pri

Real-world clinical features and survival outcomes in diffuse large B-cell lymphoma patients with hepatitis B virus infection.

OBJECTIVE: Hepatitis B virus (HBV) infection is associated with the incidence and prognosis of diffuse large B-cell lymphoma (DLBCL), and previous studies differ in terms of clinical characteristics and prognostic factors. In this study, we explored the clinical features and prognostic characteristics of real-world DLBCL patients infected with HBV. METHODS: Patients with pathologically diagnosed primary DLBCL at West China Hospital of Sichuan University were enrolled. Patients with follicular lymphoma-transformed DLBCL, primary central nervous system DLBCL, and hepatitis C virus, hepatitis E

Primary thyroid diffuse large B-cell lymphoma: a case report and focused review of the literature.

Primary thyroid diffuse large B-cell lymphoma (DLBCL) is a rare subset of primary extranodal DLBCL. Thus, there are limited evidence-based specific recommendations regarding its diagnosis and treatment. We present a case report of a patient with relapsed primary thyroid DLBCL, followed by a focused literature review covering key epidemiological, diagnostic, and therapeutic aspects. In 2017, a 74-year-old woman was diagnosed with primary thyroid non-GCB-type DLBCL (Ann Arbor IE, IPI 2) after presenting with dyspnoea, hoarseness, and cough. Treatment with R-CHOP and CNS prophylaxis achieved comp

A palmitoylation-related signature predicts prognosis and immune microenvironment features in diffuse large B-cell lymphoma.

BACKGROUND: Diffuse large B-cell lymphoma (DLBCL) is the most common aggressive lymphoma and remains highly heterogeneous in prognosis. Palmitoylation, an important post-translational modification, has been implicated in tumor biology, but its role in DLBCL, particularly in relation to the immune microenvironment, remains unclear. METHODS: Transcriptomic and clinical data from 706 patients with DLBCL were obtained from the Gene Expression Omnibus database. Prognosis-associated palmitoylation-related genes were identified by differential expression and univariate Cox regression analyses. An op

Misdiagnosis analysis and multidisciplinary collaborative treatment of primary cutaneous diffuse large B-cell lymphoma: case report.

Primary cutaneous diffuse large B-cell lymphoma (PC-DLBCL) is a rare subtype of non-Hodgkin lymphoma with nonspecific cutaneous manifestations, leading to frequent misdiagnosis. This case describes an elderly patient with severe ulcerative skin lesions who was misdiagnosed repeatedly and required multidisciplinary orthopedic-hematologic management, thus providing valuable evidence for clinical practice. An 85-year-old male presented with progressive erythematous papules, ulcers, and subcutaneous nodules on the right forearm, chest, back, and left upper arm. Pathology and immunohistochemistry c

Multiple pulmonary cavities as the prominent manifestation of diffuse large B-cell lymphoma: a case report.

A 60-year-old man complaining of fever, lacking in strength and with poor appetite for half month, was admitted to our Department of Infectious Diseases and Hepatic Diseases. The patient's chest computed tomography revealed multiple cavities in the lungs. And he was diagnosed with diffuse large B-cell lymphoma following the positron emission tomography-computed tomography scan and laparoscopic lymph node biopsy. His symptoms and the pulmonary cavity lesions improved significant after eight cycles of rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone chemotherapy.

Incidental Detection of Diffuse Large B-cell Lymphoma by Non-invasive Prenatal Testing: A Case Report.

Non-invasive prenatal testing (NIPT) is widely used for prenatal screening of common fetal aneuploidies through analysis of cell-free DNA (cfDNA) in the maternal plasma. Because cfDNA originates from both placental and maternal sources, genome-wide NIPT may incidentally reveal acquired chromosomal abnormalities associated with maternal malignancies. We report the case of an asymptomatic pregnant woman whose first-trimester NIPT revealed multiple complex chromosomal imbalances incompatible with a fetal origin and suggestive of circulating tumor-derived cfDNA. Extensive biological and radiologic

Splenic FDG PET uptake and CT volume as prognostic biomarkers in diffuse large B cell lymphoma.

OBJECTIVES: To evaluate whether baseline splenic ^18F-FDG PET uptake and CT-derived spleen volume predict early progression (≤ 36 months) and progression-free survival (PFS) in diffuse large B cell lymphoma (DLBCL), and whether they interact. MATERIALS AND METHODS: Retrospective cohort of adults with newly diagnosed DLBCL undergoing baseline ^18F-FDG PET/CT and CT. Splenic PET positivity was defined visually (uptake exceeding hepatic background); spleen volume was measured semi-automatically. Early progression/relapse was analysed as a 36-month milestone endpoint and PFS as time-to-event. Mul

Improving the predictive value of end-of-treatment PET/ CT in diffuse large B-cell lymphoma.

The 5-point Deauville score (DS) assesses end-of-treatment (EOT) response on positron emission tomography-computed tomography (PET/CT) in diffuse large B-cell lymphoma patients, categorizing scans as 'positive' or 'negative' for complete metabolic response. However, the positive predictive value (PPV) is suboptimal at 60%. We evaluated whether quantitative PET parameters combined with clinical data could improve prediction of treatment failure in EOT PET-positive patients. Baseline and EOT PET/CT scans of 138 patients in DS groups 4-5 were analyzed. Lesions were segmented using a semi-automate

Integrative Molecular Analysis Reveals Determinants of Clinical Outcomes in TP53-Mutated Diffuse Large B-Cell Lymphoma.

PURPOSE: Mutations in TP53, detected in over 20% of diffuse large B-cell lymphomas (DLBCLs), are associated with poor prognosis. However, clinical outcomes among patients with TP53-mutant disease vary, with some patients showing treatment responses similar to those with wild-type TP53. This study aims to understand the clinical and molecular determinants underlying poor outcomes in TP53-mutant DLBCL. METHODS: Clinical and molecular data for 3,091 patients were derived from 10 cohorts of patients with newly diagnosed DLBCL treated with frontline rituximab-based immunochemotherapy regimens. Tar

Tumour-Derived Extracellular Vesicles Induce Diffuse Large B-Cell Lymphoma Progression Through Macrophage-Mediated MDSC Accumulation.

Elevated peripheral myeloid-derived suppressor cells (MDSCs) predicted suppressed anti-tumour immunity and poor prognosis in diffuse large B-cell lymphoma (DLBCL) patients. Here we demonstrated that tumour-derived extracellular vesicles (EVs) served as a messenger, delivering cholesterol from lymphoma cells to macrophages. Through internalisation of tumour-derived EVs, macrophages were activated and secreted IL-1β via the NLRP3/IL-1β axis, leading to IL-1β-mediated MDSC expansion, as well as T cell suppression and exhaustion in DLBCL. Furthermore, macrophage depletion in vitro and in vivo impe

Extranodal involvement defines distinct immune-molecular phenotypes and clinical outcomes in diffuse large b-cell lymphoma.

Extranodal involvement (ENI) is incorporated into routine risk assessment for diffuse large B-cell lymphoma (DLBCL), but treating ENI as a single binary adverse feature may conceal clinically relevant heterogeneity in extranodal burden, anatomical distribution, treatment feasibility, and immune-molecular context. We evaluated ENI as a burden- and site-aware phenotype rather than as a uniform descriptor. We retrospectively studied 710 adults with newly diagnosed DLBCL treated with first-line immunochemotherapy between June 2011 and December 2024. Outcomes were analyzed according to ENI status,

Clinical Characteristics and Prognostic Factors of Primary Bone Diffuse Large B-Cell Lymphoma: A Retrospective Study.

BACKGROUND: Primary bone diffuse large B-cell lymphoma (PB-DLBCL) is a rare extranodal lymphoma with heterogeneous clinical features and survival outcomes. Although most patients respond favorably to modern immunochemotherapy, reliable baseline prognostic indicators remain insufficiently defined. METHODS: We retrospectively analyzed 424 patients with histologically confirmed PB-DLBCL. Clinical characteristics, treatment patterns and survival outcomes were evaluated. Overall survival (OS) and progression-free survival (PFS) were estimated using the Kaplan-Meier method. Prognostic factors were

Cost-effectiveness of polatuzumab vedotin plus chemoimmunotherapy for untreated diffuse large B-cell lymphoma in China.

In recent years, antibody-drug conjugates have become clinically significant in oncology therapeutics. Results from the POLARIX trial revealed that polatuzumab vedotin in combination with rituximab, cyclophosphamide, doxorubicin, and prednisone (Pola-R-CHP) versus rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone (R-CHOP) reported a 7.7% improvement in the 3-year progression-free survival (PFS), with no difference in overall survival (OS). This study aimed to assess the cost-effectiveness of polatuzumab vedotin in the first-line treatment of diffuse large B-cell lymphom

A Cost-Effectiveness Analysis of Diffuse Large B-Cell Lymphoma Treatment Pathways in the United States.

UNLABELLED: Background. Chimeric antigen receptor (CAR) T-cell therapies are approved as second-line (2L) or later therapy for diffuse large B-cell lymphoma (DLBCL). Recently, bispecific T-cell antibodies (BsAbs) have been approved as third-line (3L) treatments. The cost-effectiveness of different treatment sequences is unknown. This study aims to evaluate the cost-effectiveness of axicabtagene ciloleucel (axi-cel) compared with other treatment options for 2L DLBCL, from a US health care perspective at a cost-effectiveness threshold of $150,000 per quality-adjusted life-year (QALY).
