Current concepts on meniscal repairs.

The specific meniscus injury pattern were divided into many patterns. Nowadays, the meniscus root injury, radial tear meniscus, bucket handle tear meniscus and Ramp lesion were particularly focused on many way to manage and still controversial a lot of issues. Meniscus root tears (MRTs) and Ramp lesion are the most ignored, or misdiagnosed causes of chronic knee pain. Most patients delayed seeking treatment, consequently resulting in cartilage loss, and leading to the condition progressing to osteoarthritis knee. This has resulted in the rate of MR and Ramp repair increase significantly. The b…

Reflective meniscometry: a new field of dry eye assessment.

PURPOSE: To report a newly developed noninvasive method to measure the radius of tear meniscus curvature (reflective meniscometry) and its application to normal eyes, dry eyes, and other ocular surface conditions. METHODS: Two types of meniscometer--photographic and video--were devised. Both use tear meniscus as a concave mirror, and a specular reflex of an illuminated target at the meniscus was photographed or videotaped to obtain the radius of tear meniscus curvature. The photographic system was applied to 45 normal and 32 dry eyes; in the dry eyes, the radius of tear meniscus curvature was…

Staging of internal derangements of the TMJ with magnetic resonance imaging: preliminary observations.

Although MRI has been reported to be accurate in diagnosing displaced TMJ menisci, it has not been used to characterize the condition of the meniscus. A staging system of internal derangement of the TMJ using MRI signal characteristics and morphology of the meniscus is presented. A normal meniscus has the appearance of a small "drumstick" and is positioned superiorly on the mandibular condyle. It produces a small amount of intermediate signal on the MRI. A grade 1 displaced meniscus has normal shape and signal; a grade 2 meniscus has normal shape and absent signal; a grade 3 meniscus has abnor…

A Lack of Joint Line Tenderness Is Consistent With a Healed Meniscus, But Positive Clinical Examination Findings and MRI Scans Are Inconsistent in Identifying Failure After Meniscal Repair: A Systematic Review and Subgroup Meta-analysis.

BACKGROUND: The number of meniscal repairs being completed each year is increasing; however, the optimal, cost-effective postoperative assessment to determine the success or failure of a meniscal repair is not well known. PURPOSE/HYPOTHESIS: The purpose of this systematic review was to identify the clinical examination testing that correlates with objective magnetic resonance imaging (MRI) or second-look arthroscopy (SLA) findings to determine an optimal clinical workup for assessing postoperative meniscal repair healing. It was hypothesized that specific clinical tests would correlate with m

Suture Hook Versus All-Inside Repair for Longitudinal Tears of the Posterior Horn of the Medial Meniscus Concomitant to Anterior Cruciate Ligament Reconstruction: A Matched-Pair Analysis From the SANTI Study Group.

BACKGROUND: Secondary meniscectomy rates after repair of longitudinal tears of the posterior horn of the medial meniscus (PHMM) performed concomitantly with anterior cruciate ligament reconstruction (ACLR) are reported to be as high as 25% with an all inside repair technique. Posteromedial portal suture hook repair is an emerging technique; however, it is unknown whether it confers a significantly reduced secondary meniscectomy rate compared with the current gold standard. PURPOSE/HYPOTHESIS: The primary objective of this study was to compare the secondary meniscectomy rates of suture hook re…

Bucket-Handle Meniscal Tears Might Not Be an Urgency: The Time to Meniscus Repair Does Not Seem to Affect the Mid-Term Outcome-A Retrospective Study of Sixty Tears with a Mean Follow-Up of 6 Years.

Background: Bucket-handle meniscal tears are mostly treated arthroscopically. However, there is no clear evidence as to whether the time to surgery impacts the outcome and whether or not lesions should be treated urgently. Methods: Sixty patients were interviewed about the current status of their knee with a mean follow-up of 6.1 years (SD = 3.5). Forty-one patients underwent meniscus repair, and fifteen patients received partial resections. The primary outcome was the rate of reoperation after meniscus repair. Secondary outcomes were pain at rest and during exercise, return to sports, and Teg…

The management of lateral meniscus pathology does not affect anterior cruciate ligament revision rates in football players, but affect rates in other athletes: Long-term data from the Swedish Knee Ligament Registry.

PURPOSE: To investigate whether the treatment method of a concomitant lateral meniscus (LM) injury during primary anterior cruciate ligament (ACL) reconstruction (ACLR) affects ACL revision rates in football (soccer) players, nonfootball pivoting athletes and nonpivoting patients differentially, when compared to controls. METHODS: Data from the Swedish Knee Ligament Registry (SKLR) from 2005 to 2022 were used in univariate and multivariate logistic regression. The study compared two groups: patients with isolated ACLR and patients with ACLR and concomitant LM injury. These groups were further…

Editorial Commentary: Fourteen-Year Follow-Up Shows High Overall Incidence of Osteoarthritis With Anatomic Single-Bundle and Double-Bundle Anterior Cruciate Ligament Reconstruction: Perform an Anatomic Reconstruction but Also Save the Meniscus!

Literature over the past decades has evaluated the biomechanical and functional difference between anatomic single-bundle and double-bundle anterior cruciate ligament reconstructions (ACLRs). Meniscus tears, particularly medial meniscus ramp lesions or lateral meniscus root tears, are often identified during ACLRs and can lead to knee instability and accelerated arthritic changes if missed or not treated. Equivalent patient-reported clinical outcomes have been reported between single-bundle and double-bundle ACLRs; however, many of these studies were performed when meniscectomy was the predomi…

Pes Anserinus and Superficial Medial Collateral Ligament Preserving High Tibial Osteotomy Combined with Arthroscopic Medial Meniscus Root Repair for Medial Compartment Osteoarthritis with Varus Deformity: A Prospective Case Series of 20 Patients.

INTRODUCTION: Medial compartment osteoarthritis associated with varus malalignment is frequently compounded by medial meniscus root tears, resulting in altered knee biomechanics and accelerated cartilage degeneration. While high tibial osteotomy (HTO) corrects mechanical alignment, medial meniscus root repair restores hoop stress and meniscal function. A combined approach may provide superior biomechanical restoration. CASE SERIES: This prospective case series includes 20 patients with symptomatic medial compartment osteoarthritis, varus deformity, and magnetic resonance imaging-confirmed med…

SPARCL1 targeting BST2 mediates meniscal inflammation and catabolic dysfunction by activating the NF-κB/P65 pathway.

OBJECTIVES: Meniscus injury is one of the most common musculoskeletal injuries, and its pathogenesis is associated with age, mechanical stress and inflammatory injury. However, the mechanism of meniscal degeneration remains unclear and this study aimed to investigate the role of SPARCL1 in meniscal degeneration. METHODS: Transcriptome sequencing of osteoarthritic/non-osteoarthritic meniscus from the Gene Expression Omnibus (GEO) was analysed. SPARCL1 expression was verified in anterior cruciate ligament transection (ACLT) mice and human degenerated meniscus. Mice were administered SPARCL1-kno…

All-inside suture meniscal repair using suture passer.

In recent years, to save the meniscus and prevent the progression of knee osteoarthritis, the indications for meniscus repair have been expanding instead of partial menisectomy. Accordingly, various repair techniques for meniscus tears have been developed. The conventional inside-out and outside-in meniscus repair techniques and all-inside repair technique with an implant/anchor can be classified as trans-capsular (TC) repair from the perspective of suture with penetrating capsule. Recently, new suture passers for all-inside meniscus repair have been developed. To distinguish from conventional…

Bilateral discoid medial menisci accompanied by distal arthrogryposis: a case report.

Discoid meniscus shows a propensity for the lateral side, and bilateral discoid medial meniscus is extremely rare. The present patient had a history of windblown hand and clubfoot, and bilateral discoid medial meniscus was associated with distal arthrogryposis. There has been no previous case report about the coexistence of meniscal abnormalities and distal arthrogryposis. While the association between distal arthrogryposis and meniscal abnormalities is unknown, it is interesting that discoid medial meniscus (a rare meniscus abnormality) occurred in the presence of arthrogryposis. We performed…

Can the Acetabular Labrum Be Reconstructed With a Meniscal Allograft? An In Vivo Pig Model.

BACKGROUND: No single graft type has been shown to have a benefit in acetabular labral reconstruction. The native labrum and lateral meniscus share many similarities, suggesting that the meniscus may be a promising source of graft material in labral reconstruction. QUESTIONS/PURPOSES: Using a pig model, we sought to evaluate the healing process of fresh-frozen meniscus allograft for acetabular reconstruction by assessing (1) MRI and macroscopic observations of the meniscus allograft; (2) histologic appearance and immunohistologic evaluation of the meniscus allograft, native meniscus, and labr…

Case Report: Dislocation of lateral menisci secondary to congenital lateral tibiofemoral cartilage thickening in both knees.

A 24-year-old male patient complained of mild knee pain after jogging. The subsequent knee MRI demonstrated bilateral lateral thickened tibiofemoral cartilages, evidenced by deformities of the bilateral subchondral bone beneath the lateral femoral condyle cartilage. The corresponding dislocations of almost all the left lateral meniscus and part of the right lateral meniscus to the center of the joint were detected. After excluding diagnoses of congenital ring-shaped meniscus, bucket handle tear of the C-shaped lateral meniscus, and central tear of the discoid meniscus, the displacement of all …

Structure-Function relationships of equine menisci.

Meniscal pathologies are among the most common injuries of the femorotibial joint in both human and equine patients. Pathological forces and ensuing injuries of the cranial horn of the equine medial meniscus are considered analogous to those observed in the human posterior medial horn. Biomechanical properties of human menisci are site- and depth- specific. However, the influence of equine meniscus topography and composition on its biomechanical properties is yet unknown. A better understanding of equine meniscus composition and biomechanics could advance not only veterinary therapies for meni…

Sex-Specific Analysis of Quadriceps and Hamstring Strength Following Anterior Cruciate Ligament Reconstruction With or Without Meniscal Repair.

Background and Objectives: Strength deficits are extremely common following anterior cruciate ligament reconstruction (ACLR). These deficits may be further impacted by concomitant meniscus repair during ACLR. However, there remains limited evidence investigating the effect of meniscus repair on lower-limb strength after ACLR, and particularly if there are between-sex differences in strength after ACLR with and without meniscus repair. Therefore, the purpose of this study was to investigate sex-specific differences in meniscus repair during ACLR on quadriceps and hamstring isometric strength. M…

Selective Extracellular Matrix Guided Mesenchymal Stem Cell Self-Aggregate Engineering for Replication of Meniscal Zonal Tissue Gradient in a Porcine Meniscectomy Model.

Degenerative meniscus tears (DMTs) are prevalent findings in osteoarthritic knees, yet current treatment is mostly limited to arthroscopic partial meniscectomy rather than regeneration, which further exacerbates arthritic changes. Translational research regarding meniscus regeneration is hindered by the complex, composite nature of the meniscus which exhibit a gradient from inner cartilage-like tissue to outer fibrous tissue, as well as engineering hurdles often requiring growth factors and cross-linking agents. Here, a meniscus zonal tissue gradient is proposed using zone-specific decellulari…
