European position paper on drug-induced sleep endoscopy: 2017 Update.

BACKGROUND: The first edition of the European position paper (EPP) on drug-induced sleep endoscopy (DISE) was published in 2014 with the aim to standardise the procedure, to provide an in-depth insight into the main aspects of this technique and to have a basis for future research. Since 2014, new studies have been published concerning new sedative agents or new insights into the pattern/levels of the obstruction depending on the depth of sedation. Therefore, an enlarged group of European experts in the field of sleep breathing disorders (SBD), including the most of the first DISE EPP main aut

A Modified Mask for Continuous Cardiac Monitoring during Positive Airway Pressure Therapy.

A Positive Airway Pressure mask was modified to be able to measure the heart rate of the wearer during Positive Airway Pressure (PAP) therapy. The mask was modified by attaching sensors to measure Electrocardiography (ECG) and Photoplethysmography (PPG) signals, from which a heart rate was extracted. The ECG signal was recorded using stainless steel electrodes positioned on the wearer's head and neck. The PPG signal was measured from the wearer's forehead using a reflectance pulse oximeter attached to the mask. The modified device was tested on 19 healthy participants (mean age30.5 ± 9 years)

Acute Illness Protocol for Maple Syrup Urine Disease.

Inborn errors of metabolism (IEMs) are genetic disorders that disrupt enzyme activity, cellular transport, or energy production. They are individually rare but collectively have an incidence of 1:1000. Most patients with IEMs are followed up by a physician with expertise in biochemical genetics (metabolism), but may present outside this setting. Because IEMs can present acutely with life-threatening crises that require specific interventions, it is critical for the emergency medicine physician, pediatrician, internist, and critical care physician as well as the biochemical geneticist to have i

Comparison of FFT and marginal spectra of EEG using empirical mode decomposition to monitor anesthesia.

BACKGROUND AND OBJECTIVE: Intraoperative awareness refers that patients can recall aspects of their surgery after being put under general anesthesia. This distressing complication causes affected patients to be conscious and probably feel pain, leading to emotional trauma or other sequelae. Monitoring and administrating the depth of anesthesia is necessary to prevent patients from awareness during a medical operation. In this paper, we analyzed the electroencephalograms (EEGs) of patients to characterize their anesthesia. The data set, "awareness" and "anesthesia" groups, each contained 558 sa

Parallel reaction monitoring (PRM) and selected reaction monitoring (SRM) exhibit comparable linearity, dynamic range and precision for targeted quantitative HDL proteomics.

UNLABELLED: High-density lipoprotein (HDL), a lipid nanoparticle containing many different low abundance proteins, is an attractive target for clinical proteomics because its compositional heterogeneity is linked to its cardioprotective effects. Selected reaction monitoring (SRM) is currently the method of choice for targeted quantification of proteins in such a complex biological matrix. However, model system studies suggest that parallel reaction monitoring (PRM) is more specific than SRM because many product ions can be used to confirm the identity of a peptide. We therefore compared PRM an

Cardiopulmonary bypass considerations for pediatric patients on the ketogenic diet.

BACKGROUND: There is a population of children with epilepsy that is refractory to anti-epileptic drugs. The ketogenic diet, a high-fat, low-carbohydrate regimen, is one alternative treatment to decrease seizure activity. Special considerations are required for patients on the ketogenic diet undergoing cardiopulmonary bypass (CPB) to prevent exposure to glucose substrates that could alter ketosis, increasing the risk of recurrent seizures. CASE STUDY: A 2-year-old, 9 kilogram male with a history of infantile spasms with intractable epilepsy, trisomy 21 status post tetralogy of Fallot repair, p

Four phases of intravenous fluid therapy: a conceptual model.

I.V. fluid therapy plays a fundamental role in the management of hospitalized patients. While the correct use of i.v. fluids can be lifesaving, recent literature demonstrates that fluid therapy is not without risks. Indeed, the use of certain types and volumes of fluid can increase the risk of harm, and even death, in some patient groups. Data from a recent audit show us that the inappropriate use of fluids may occur in up to 20% of patients receiving fluid therapy. The delegates of the 12th Acute Dialysis Quality Initiative (ADQI) Conference sought to obtain consensus on the use of i.v. fluid

Load Monitoring System for Partial Weight Bearing Therapy for rehabilitation of lower extremity fractures.

Partial Weight Bearing Therapy (PWB) is used during in rehabilitation of lower-extremity fractures, but optimal outcomes are limited by the paucity of data supporting the current standard of care, the inability of clinicians to assess patients' compliance to the prescribed therapy, and by the patient's inability to apply clinical loading guidelines. To address these needs a Load Monitoring System (LMS) was developed using a novel long-term load measuring technology coupled with consumer electronics for storing and reporting of patient limb loading data. The project was planned as a staged effo

Surgical treatment and intraoperative spinal cord monitoring in scoliosis associated with chronic inflammatory demyelinating polyneuropathy: a case report.

There has been only one reported case of neuromuscular scoliosis following chronic inflammatory demyelinating polyneuropathy (CIDP). However, no cases of scoliosis that were treated with surgery secondary to CIDP have been previously described. A 16-year-old boy with CIDP was consultant due to the progression of scoliosis with the coronal curve of 86° from T8 to T12. Posterior correction and fusion with segmental pedicle screws were performed under intraoperative spinal cord monitoring with transcranial electric motor-evoked potentials. Although the latency period was prolonged and amplitude w

A Kinect-based system for cognitive rehabilitation exercises monitoring.

In this paper, a 3D computer vision system for cognitive assessment and rehabilitation based on the Kinect device is presented. It is intended for individuals with body scheme dysfunctions and left-right confusion. The system processes depth information to overcome the shortcomings of a previously presented 2D vision system for the same application. It achieves left and right-hand tracking, and face and facial feature detection (eye, nose, and ears) detection. The system is easily implemented with a consumer-grade computer and an affordable Kinect device and is robust to drastic background and

Hemolacria: case report.

Hemolacria is a very rare entity which literally means "bloody tears". Conjunctivitis, capillary hemangioma, conjunctival telangiectasia, trauma, retrograde epistaxis and nasal and paranasal neoplasms may play a role in its etiology. In our case report, a 11-year-old female patient who presented with the complaint of bilateral epistaxis accompanied by bloody tears that had been present for 2 years and whose endoscopic examination revealed hyperemia and increased vascularity in the nasal cavity mucosa was discussed together with a review of the literature.

[Management of primary open angle glaucoma by paying attention to monitor intraocular pressure fluctuations].

Higher intraocular pressure (IOP) is a risk factor for the development and progression of glaucomatous neuropathy. Increasing attention has been brought to IOP fluctuation in patients with primary open angle glaucoma (POAG) in recent years. IOP fluctuations include short-term (24-hour IOP fluctuations) and long-term fluctuations, which may contribute to the progression of glaucoma. It is a big challenge for ophthalmologists in their busy daily clinical practice to deal with IOP fluctuation in patients with POAG. In this paper, the research progress in IOP fluctuation of POAG and its relationsh

Defining the boundaries of bedside pulse contour analysis: dynamic arterial elastance.

Assessment of vasomotor tone is essential in defining appropriate resuscitation strategies for the hypotensive patient. Although changes in mean arterial pressure to cardiac output define arterial resistance, resistance is only one component of vasomotor tone. Compliance is the other component. The reciprocal of compliance is arterial elastance (Ea). Importantly, dynamic Ea can be estimated by the pulse pressure variation to stroke volume variation relation. Dynamic Ea is only one component of vasomotor tone, however, and increases in pulse pressure may not be proportional to increases in mean

EEG monitoring during therapeutic hypothermia in neonates, children, and adults.

Therapeutic hypothermia is being utilized as a neuroprotective strategy in neonates, children, and adults. The most common indications are hypoxic ischemic encephalopathy in neonates and post cardiac arrest in adults. Electroencephalographic monitoring use is increasing in critical care units, and is sometimes a component of therapeutic hypothermia clinical pathways. Monitoring may detect non-convulsive seizures or non-convulsive status epilepticus, and it may provide prognostic information. We review data regarding indications for therapeutic hypothermia and electroencephalographic monitoring

[News in hemodynamic monitoring, resuscitation and intensive care of patients after cardiac surgery: "Guidelines for resuscitation in cardiac arrest after cardiac surgery" of the European Association for Cardio-Thoracic Surgery].

In order to simplify and to standardize procedures during cardiac arrest in patients after cardiac surgery and for professional medical staff education, working group of the European Association for Cardio-Thoracic Surgery issued in 2009 "Guideline for resuscitation in cardiac arest after cardiac surgery". There are several differences between these guidelines and guidelines for general population: in ventricular fibrillation, three sequential attempts at defibrillation should precede external cardiac massage; in asystole or extreme bradycardia, pacing should precede external cardiac massage.
