[Adult form of acid maltase deficiency presenting as progressive spinal muscular atrophy].

As far as could be elucidated, a sporadic manifestation of a slowly progressing muscular weakness and atrophy had commenced in a female patient symmetrically in the pelvic girdle and thigh region at the age of about 22 years. Only at the age of 55 years a precise neurological examination was done. On the basis of clinical and electromyographic data spinal muscular atrophy of the Kugelberg-Welander type was diagnosed. Muscular biopsy demonstrated severe far-progressed neurogenic muscular atrophy. In addition, increased storage of PAS-positive material was noticed. This was particularly true in …

[Electromyographic analysis on juvenile muscular atrophy of unilateral upper extremity (author's transl)].

In 1959, this disorder was first reported by one of authors (K.H.) (6). Since then, many Japanese cases have been reported and recently some cases are also reported in other countries. Its cause and pathology are still unknown, because no autopsy case was obtained. In this report, some electromyographic findings are described and discussed. This series contain 26 cases (male 23, female 3). Age of onset: 13-33 years old (mean 19.4 years old). Clinical duration: 2-32 years old (mean 9.0 years). Only 6 cases (23%) showed trauma as the past history (Table 2). We classified 26 cases in three groups

Utility of Far-Field Potentials as a Biomarker of Neurodegeneration in Spinal Muscular Atrophy.

INTRODUCTION/AIMS: Far field potentials (FFP) have been proposed as a reliable neurophysiological prognostic biomarker in amyotrophic lateral sclerosis (ALS). This study evaluated the utility of ulnar nerve FFP as a robust research biomarker of lower motor neuron degeneration in spinal muscular atrophy (SMA). METHODS: Peripheral neurophysiological assessments were performed in 13 participants with SMA, 19 with amyotrophic lateral sclerosis (ALS), and 19 healthy controls. The ulnar nerve was stimulated at the wrist, and motor responses were recorded over the abductor digiti minimi (ADM) muscle…

Case Report: Spinal muscular atrophy with IgA nephropathy: a coincidence or association?

BACKGROUND: Spinal muscular atrophy (SMA) is an autosomal recessive neuromuscular disorder caused by biallelic loss-of-function variants of the survival motor neuron 1 (SMN1) gene on chromosome 5q13. It has been reported that SMA may affect the function of the kidneys. Here, we report a patient with co-occurrence of SMA and IgA nephropathy (IgAN). CASE PRESENTATION: A 14-year-old girl presented with six months of limb weakness, progressive exacerbation of symptoms of left lower limb muscle weakness; her left lower limb muscle strength decreased, and bilateral knee tendon reflexes and Achilles…

A Gene, A Breakthrough, A Challenge: Lessons From the History of Spinal Muscular Atrophy.

The discovery of the SMN1 gene on chromosome 5q in 1995, and later, identification of SMN2 as a modifier gene was the breakthrough in the history of spinal muscular atrophy (SMA). It was the discovery of this gene that led to the discovery of 3 disease-modifying drugs that were approved for use by the US Food and Drug Administration and the European Medicines Agency. This brings to one's mind the fact that the earliest description of this disease happened a century ago. The persistent efforts of a few scientists have rewritten the destiny of children with SMA. With the discovery of the new dru…

Spinal and bulbar muscular atrophy in a patient with Parkinson's disease - Case report.

Spinal and bulbar muscular atrophy and Parkinson's disease rarely coexist. We describe a male patient presenting with parkinsonism, hyposmia, with DAT-SPECT deficits, responsive to dopaminergic therapy, preceding recognition of SBMA confirmed genetically. Dopamine agonist-associated dropped head syndrome unmasked underlying lower motor neuronopathy, highlighting diagnostic pitfalls and overlap between synucleinopathy and polyglutamine disease.

Molecular diagnosis of spinal muscular atrophy: Experience in a pediatric hospital in Argentina.

Introduction. Spinal muscular atrophy (SMA) is an autosomal recessive neuromuscular disease caused by the loss of the SMN1 gene, with a variable clinical spectrum determined primarily by the number of copies of the SMN2 gene. The development of new therapies underscores the importance of early molecular diagnosis and genotype-phenotype characterization. Objective. To describe 27 years of experience in the molecular diagnosis of SMA at a pediatric referral hospital in Argentina and to evaluate the correlation between SMN2 copy number and the SMA types.Population and methods. A retrospective des…

The systemic complexity of a monogenic disease: the molecular network of spinal muscular atrophy.

Monogenic diseases are well-suited paradigms for the causal analysis of disease-driving molecular patterns. Spinal muscular atrophy (SMA) is one such monogenic model, caused by mutation or deletion of the survival of motor neuron 1 (SMN1) gene. Although several functions of the SMN protein have been studied, single functions and pathways alone do not allow the identification of crucial disease-driving molecules. Here, we analysed the systemic characteristics of SMA, using proteomics, phosphoproteomics, translatomics and interactomics, from two mouse models with different disease severities and…

Pilocytic astrocytoma in a child with spinal muscular atrophy treated with onasemnogene abeparvovec.

Spinal muscular atrophy (SMA) is a severe neuromuscular disease, leading to progressive muscle weakness and potentially early mortality if untreated. Onasemnogene abeparvovec is a recombinant adeno-associated virus serotype 9 (rAAV9)-based gene therapy that has demonstrated improvements in survival and motor function for SMA patients. Here, we present a case of a patient diagnosed with a grade 1 pilocytic astrocytoma at the age of 2 years, approximately 8 months after onasemnogene abeparvovec treatment. Although vector genomes delivered by rAAVs persist primarily as episomes, rare integration …

Clinical Application of Risdiplam in 5q Spinal Muscular Atrophy: A Narrative Review.

5q spinal muscular atrophy (SMA) is caused by mutations in the survival motor neuron (SMN) gene located on chromosome 5, leading to insufficient SMN protein levels. Risdiplam is an RNA splicing modifier that modifies pre-mRNA splicing of the SMN2 gene, thereby promoting the production of functional survival motor neuron protein (SMN-fl). This article reviews the drug trials of Risdiplam, summarizes the actual clinical data, and systematically evaluates the effectiveness and safety of this drug. By discussing the mechanism of action and economic cost of this drug and comparing it to other SMA d…
