Artificial Intelligence in Hair and Nail Disorders.

Artificial intelligence (AI), a field of computer science, aims at simulating human intelligence with computers. Though AI has surpassed dermatologists in skin cancer detection, it still lags behind various other specialties like radiologists in broader adoption. Newer AI applications are becoming increasingly accessible. AI plays a role in various areas, such as medical image recognition, auxiliary diagnosis, and drug research and development. Dermatology has a prime position in implementation of AI in medical research due to its larger clinical, dermoscopic, and histopathological image datab

Dysbiosis of nail microbiome in patients with psoriasis.

Shifts in skin microbiome are considered to be involved in the pathogenesis of psoriasis. However, data on the microbial dysbiosis of nail p … ns in the bacterial microbiome were found in the nail samples of psoriatic patients. The unaffected nails in psoriatic patients were associated with higher bacterial diversity, and a higher relative abundance of Enhydrobacter, whereas nail psoriasis was correlated with a decreased relative abundance of Anaerococcus. Shifts in fungal community compositio …

Alopecia affects men and women and can result in significant distress for patients. Alopecias can be categorized as nonscarring or scarring. Nonscarring alopecias include male and female pattern alopecias, alopecia areata, telogen effluvium, traction alopecia, trichotillomania, and tinea capitis. Scarring alopecias include central centrifugal cicatricial alopecia, lichen planopilaris, frontal fibrosing alopecia, discoid lupus erythematosus, dissecting cellulitis of the scalp, folliculitis decalvans, and acne keloidalis nuchae. Evaluation of patients with alopecia involves assessment of the dur

Surgical repair for deformities of the nail unit.

The nail apparatus is the largest and most complex skin appendage. Defects in this unit can result in significant functional insufficiency and cosmetic disfigurement. Common nail deformities include split nail, short nail, onycholysis, nail malalignment, hooked nail, and absent nail. Currently, surgical repair is the primary treatment for such deformities. Based on the etiological and anatomical classifications, one or more appropriate operations can be selected to repair nail unit deformities. These include autologous fat grafting, longitudinal cicatrectomy, Z-plasties, nail bed elongation, s

Dermoscopic Features of Nail Psoriasis: Revisited.

INTRODUCTION: Nail psoriasis poses a diagnostic challenge in a context of clinical findings that are not evident. We aimed at exploring dermoscopic findings of nail psoriasis and assessing the relationship between these features with disease severity and the risk of arthritis. METHODS: We enrolled 35 patients (26 men, 9 women) with severe nail psoriasis (Nail Psoriasis Severity Index [NAPSI] score of more than 50). A complete clinical evaluation, including musculoskeletal assessment and videodermoscopic examination of all nails, was carried out for each patient. RESULTS: Splinter haemorrhage…

Nail Changes Associated With Thyroid Disease.

Nail changes with thyroid disease have not been well studied. Nail findings are helpful in early diagnosis of thyroid disorders and therefore are important for dermatologist education. We reviewed the literature on nail changes in thyroid patients and found that onycholysis and slow-growing, thin nails are associated with hypothyroidism and that onycholysis, koilonychia, and brittle nails changes are associated with hyperthyroidism.

Hair and nail manifestations of COVID-19.

Emerging literature evidence shows that the manifestations of the coronavirus disease 2019 (COVID-19), which is the disease caused by SARS-CoV-2, encompass alterations of the pulmonary, cardiovascular, gastrointestinal, and neurological system. Moreover, hematologic and dermatologic manifestations have been documented. The aim of this review is to summarize the dermatologic manifestations of COVID-19 involving the hair and nails in a narrative way. A total of 1136 patients have been reported to have de novo hair loss following COVID-19. Notably, 958 patients experienced telogen effluvium (TE) …

Nail Surgery: Six Essential Techniques.

Successful nail surgery requires an understanding of specific disease processes, the anatomy of the nail unit, and fluency with only a few key techniques. This article focuses on 6 high-yield procedures, facility with which will allow the clinician to approach most of the clinical scenarios requiring surgical intervention. These encompass surgical approaches to inflammatory nail diseases, melanonychia, erythronychia, and nail melanoma in situ.

Concepts, Role, and Advances on Nail Imaging.

This review discusses, from a practical point of view, the most common imaging techniques and their applications in nail pathologies with some technical considerations. There are several imaging techniques for studying the nail, and all of them require proper devices and trained operators. The highest axial resolution and a more extensive range of applications are provided by ultrasound, which currently is the first-choice imaging technique for evaluating nail conditions. A correlation of state-of-the-art clinical and imaging figures supports the review of this topic.

Treatment of nail diseases in the emergency department.

BACKGROUND: Although Americans seek care for their nail diseases on an outpatient basis, they also seek nail care in the setting of the emergency department (ED). PURPOSE: This study aims to characterize the treatment of nail diseases at ED visits in the United States from 2009-2018. METHODS: Data from 2009-2018 was collected from the National Hospital Ambulatory Medical Care Survey for EDs. RESULTS: There was an estimated 1.54 (95% CI [1.35, 1.74]) million nail visits to the ED from 2009-2018. Whites accounted for the most visits (57%), followed by African Americans (21%). 25-44-year-olds

Retrospective Diagnosis of Nail-patella Syndrome.

A 37 years old female presented with asymptomatic nephrotic range proteinuria due to focal segmental glomerulosclerosis (FSGS). She was treated with steroids and mycophenolate mofetil to which there was no response and progressed to advanced chronic kidney disease. When her brother who was being evaluated as a potential donor, for renal transplant, was found to have proteinuria and a genetic study for the steroid-resistant nephrotic syndrome was done. This revealed mutation in the LMX1B gene. It is then that a diagnosis of nail-patella syndrome (NPS) was made. She underwent a successful renal

Implant fracture of the TFNA femoral nail.

BACKGROUND: The TFNA (Trochanteric Fixation Nail Advanced) Proximal Femoral Nailing System (DePuy Synthes) is frequently used for intramedullary fixation of proximal femoral fractures. The aim of this study was to evaluate all TFNA implant fractures at a UK trauma unit to ascertain any patient or surgical factors associated with implant failure. METHODS: A retrospective study was carried out identifying all patients that sustained a TFNA implant fracture over a five-year period. Data was collected on demographic information, ASA, co-morbidities, mechanism of injury, fracture pattern according

Psoriatic transversal nail grooves on biologics.

We noticed transverse grooves on the nails of a psoriasis vulgaris patient being treated with ixekizumab. The indentation might have appeared during the period when the effective concentration of ixekizumab was low and psoriasis activity in the nail matrix had increased shortly before the monthly dose.
