BACKGROUND: Distal pancreatic carcinoma is one of the most lethal cancers largely due to its high incidence of distant metastasis. This study aims to assess the prognostic value of splenic-vasculature involvement in resected distal pancreatic carcinoma.
METHODS: In this retrospective study, we collected the clinicopathologic information of 454 patients with pancreatic cancer and performed univariate and multivariate analyses to identify factors associated with progression-free survival (PFS) and overall survival (OS), with an emphasis on the prognostic value of splenic-artery and -vein involv

BACKGROUND: Urachal inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal neoplasm characterized by non-specific clinical and imaging features, which renders it susceptible to clinical misdiagnosis. Complete surgical resection is the preferred treatment method.
CASE PRESENTATION: We herein report a case of urachal IMT in a 12-year-old male child who presented with dysuria for 4 days and abnormal urine color for 2 days. Preoperative ultrasonography and contrast-enhanced computed tomography (CT) both suggested an infected urachal cyst. Laparoscopic resection of the urachal mass was ini

Basal cell carcinoma (BCC) is the most common skin cancer. Skin cancers may present either as a non-invasive tumor or an invasive malignancy. The terminology of carcinoma in situ is used when the tumor is either just limited to epidermis or not present as single cells or nests in the dermis. However, currently the terminology superficial BCC is inappropriately used instead of BCC in situ when the skin cancer is limited to epidermis. In this study we compare the pathologic changes of superficial, nodular, and infiltrative BCCs using electron microscopy to identify the ultrastructural characteri

Currently, there is no reliable method of discerning between low-risk and high-risk intraductal papillary mucinous neoplasms (IPMNs). Operative resection is utilized in an effort to resect those lesions with high-grade dysplasia (HGD) prior to the development of invasive disease. The current guidelines recommend resection for IPMN that involve the main pancreatic duct. Resecting lesions with HGD before their progression to invasive disease and the avoidance of resection in those patients with low-grade dysplasia is the optimal clinical scenario. Therefore, the importance of developing preopera

Myofibroblastoma (MFB) of the breast is a rare benign neoplasm of the mammary stroma. Several morphologic variants have been described in the literature, which can create diagnostic challenges for pathologists, in particular the epithelioid variant of MFB, which can mimic invasive lobular carcinoma. We report a case of a 72-year-old female who presented for a painless breast lump and was later found to have 2 lesions on imaging, with 1 lesion corresponding to the palpable lump and the other lying in a different quadrant. Core-needle biopsies demonstrated ductal carcinoma in-situ at both lesion

Mucinous cystic neoplasms (MCNs) of the pancreas are slow-growing tumours but carry a risk of malignant transformation and massive enlargement if left untreated. We present a rare case of a 49-year-old female initially diagnosed in 2022 with a 14-cm pancreatic cyst. After declining surgery, she presented four years later with acute bowel obstruction and a distended abdomen. Imaging revealed dramatic progression to a giant 31 × 22.7 cm mass displacing most abdominal viscera. An emergency distal pancreatectomy and splenectomy were performed, followed by a staged second-look laparotomy to ensure

Primary pancreatic leiomyosarcoma (PLMS) is a rare malignant tumor accounting for less than 0.1% of all pancreatic neoplasms and often presents at an advanced stage with distant metastases or local invasion. Clinical symptoms are nonspecific, including gastrointestinal discomfort, abdominal pain, or distension, which frequently leads to delayed diagnosis. We report a 53-year-old woman with persistent abdominal distension, bloating, and central discomfort, in whom imaging revealed a 7.5 cm mass in the pancreatic tail with multiple liver metastases, encasement of the splenic arteries, and no dir

CYSTIC LESIONS OF THE PANCREAS. The practice of every gastroenterologist is daily punctuated by the management of patients with pancreatic cystic lesions (PCL). The discovery is usually fortuitous and the lesions in majority are small (less than 10mm). The prevalence of PCL in the general population of adults is very high and probably greater than 40% if we consider lesions of 2 mm. Some PCL are precancerous lesions, (as intraductal papillary mucinous neoplasm [IPMN]) with a real risk of pancreatic adenocarcinoma and others cystic lesions have a risk of metastases such as pseudopapillary and s

An 83-year-old man with main pancreatic duct (MPD) stenosis in the pancreatic body had undergone surveillance with semiannual imaging studies for 3 years. During surveillance, magnetic resonance cholangiopancreatography revealed gradual enlargement of a small cyst near the MPD stenosis and contrast-enhanced computed tomography revealed locally progressive atrophic parenchyma in the pancreatic body. On endoscopic retrograde pancreatography, the MPD stenosis was more severe than it had been at diagnosis 3 years earlier. Endoscopic ultrasonography (EUS) showed a 10-mm hypoechoic mass adjacent to

Papilloma associated with recurrent respiratory papillomatosis (RRP), caused by human papilloma virus (HPV) infection types 6 and 11, is the most common benign neoplasm of the larynx. The clinical features of RRP vary widely from mild to aggressive forms. RRP in children is known as juvenile-onset recurrent respiratory papillomatosis (JORRP). Its outcome may be poor or even fatal due to the high rate of recurrence and eventual spread to the entire respiratory tract. Pulmonary invasion is reported to occur in 3.3% of patients with RRP, and malignant transformation in 0.5% of patients. We report

Determinants of hepatocellular carcinoma recurrence posttransplant include hepatic tumor burden, presence of vascular invasion, and serum alpha-fetoprotein level. However, the significance of marked alpha-fetoprotein elevation in cirrhosis, in the absence of a hepatic mass lesion on imaging studies, is unclear and no longer qualifies for a Model for End-Stage Liver disease exception for transplant listing in the United States. We report a case of posttransplant metastatic recurrent hepatocellular carcinoma in a patient with marked elevation of alpha-fetoprotein pretransplant without imaging ev

Primary intracranial myxofibrosarcoma is exceedingly rare, with less than 10 cases published. We present a case of a 23-year-old man with previous history of a primary low grade myxofibrosarcoma of the left parietal-occipital convexity resected in March 1999. He subsequently underwent several interventions for multiple local recurrent disease until March 2004. At that time, complete remission was documented. About 8 years later, in February 2012, the patient was admitted to the emergency room with refractory acute pulmonary oedema. On work up, sustained monomorphic ventricular tachycardia and

The expression of metalloproteinases and their inhibitors has been related to different invasive and metastatic potentials in cancer. This study aims to investigate the immunohistochemical expression of TIMP-3 and MMP-9 in samples of basal cell carcinoma (BCC), squamous cell carcinoma of the skin (SCC), and actinic keratosis (AK). Immunohistochemistry was performed to evaluate the expression of TIMP-3 and MMP-9 in samples of BCC (n=22), SCC (n=10), and AK (n=15). Ten fields of both tumor parenchyma and tumor stroma were photographed and counted in image software. The ratio of positive cells to

Low grade appendiceal mucinous neoplasms can spread to the peritoneum as pseudomyxoma peritonei even though they are not obviously invasive in the appendix. During the past several decades, several problematic issues surrounding this enigmatic tumor have been debated in the literature, including appropriate nomenclature for the appendiceal tumors and their peritoneal metastases. In this article, the most contentious issues in the area of appendiceal mucinous tumors are examined. First, the classification systems that have been proposed for these tumors are compared in the context of whether th

Malignant mesothelioma is a neoplasm that can arise from the mesothelial surface of the pleura and tends to be locally invasive. Various techniques have been used for diagnosis and include thoracentesis with pleural fluid cytology, closed pleural biopsy, and video-assisted thoracoscopic surgery. Thoracentesis and closed pleural biopsy can often establish the presence of malignancy, but frequently do not provide sufficient tissue to definitively diagnose malignant mesothelioma. Video-assisted thoracoscopic surgery biopsy has higher sensitivity for establishing the diagnosis, but is a more invas

Cholangiocarcinomas are epithelial neoplasms that originate from cholangiocytes and can occur at any level of the biliary tree. They are broadly classified into intrahepatic tumours, (extrahepatic) hilar tumours and (extrahepatic) distal bile duct tumours. In spite of well-understood predispositions, most cholangiocarcinomas arise in the absence of risk factors. In suspected cases, the diagnosis can be established with non-invasive imaging studies. Biliary invasion should be reserved for patients with obstruction. In high-risk patients, advanced cytological tests of aneuploidy (digital image a

Middle ear adenocarcinoma is a very rare, locally invasive neoplasm assumed to arise from the middle ear mucosa. Although endolymphatic sac tumor (aggressive papillary middle ear tumor) and jugulotympanic paraganglioma may show brain invasion, intracranial extension of histologically confirmed middle ear adenocarcinoma has not been previously reported. The authors describe a 53-year-old man who suffered from otalgia and tinnitus for more than 10 years and from neurological deficits for 1 year due to a large temporal bone tumor that invaded the temporal lobe. We reviewed 45 cases of transitional cell carcinoma of the renal pelvis and ureter with reference to the coexistence or subsequent development of bladder cancer. Bladder cancer was associated with an upper urinary tract neoplasm in 26 of the 45 cases. The 5-year survival rate for coexistence of bladder cancer was 56% and that for subsequent bladder cancer was 65.6%. The 5-year survival rate for 19 cases unrelated with bladder cancer was 46.7%. Therefore, there was no significant difference among the three groups. As to degree of the malignancy of the renal pelvis and ureter, the 5-year surviva
