Comparison of Long-Term Tunnel Widening in Anterior Cruciate Ligament Reconstruction Using Allograft and Hamstring Autograft: More Than 10 Years' Mean Follow-Up.

Bone tunnel widening following anterior cruciate ligament reconstruction (ACLR) is a well-recognized radiological phenomenon with multifactorial etiology. While concerns have been raised regarding potentially greater tunnel enlargement with allografts due to immune-mediated responses, evidence remains inconsistent. This study compared long-term femoral and tibial tunnel widening between hamstring autograft and allograft reconstruction at a mean follow-up exceeding 10 years. A retrospective analysis was conducted on 60 patients who underwent primary ACLR between 2011 and 2016 by a single surgeo

Comparative study of different intraoperative 3-D image intensifiers in orthopedic trauma care.

INTRODUCTION: Recently, isocentric C-arm fluoroscopy (Iso-C 3D) has been introduced as a precise imaging modality for intraoperative evaluation and management of fractures and osteosyntheses. The Siemens Iso-C 3D collects multiple fluoroscopic images during a 190-degree arc of rotation around the anatomic region of interest and reconstructs them into sagittal, axial, and coronal planes. Like the Iso-C 3D, the new Ziehm Vario 3D imaging system reconstructs images in multiple planes, but only requires a 136-degree arc of rotation. The purpose of this study was to compare the image quality and ra

Alkaline biodegradable implants for osteoporotic bone defects--importance of microenvironment pH.

UNLABELLED: Change of microenvironment pH by biodegradable implants may ameliorate unbalanced osteoporotic bone remodeling. The present work demonstrated that a weak alkaline condition stimulated osteoblasts differentiation while suppressed osteoclast generation. In vivo, implants with an alkaline microenvironment pH (monitored by a pH microelectrode) exhibited a promising healing effect for the repair of osteoporotic bone defects. INTRODUCTION: Under osteoporotic conditions, the response of the bone microenvironment to an endosseous implant is significantly impaired, and this substantially i

Child pectus excavatum: correction by minimally invasive surgery.

INTRODUCTION: Pectus excavatum (PE) is a congenital deformity essentially responsible for an unattractive aspect, much more rarely for compression problems. The classical treatments consist either in filling the excavation or in open thoracic reconstruction (the Ravitch technique). Alternatively, the treatment described by Nuss raises the sternum with a retrosternal metallic bar placed under thoracoscopic guidance. We present the preliminary results of a series of 25 children operated on using this technique. HYPOTHESIS: The minimally invasive procedure described by Nuss is a valid surgical s

Bone replacement by ionomer cement in osteoplastic frontal sinus operations.

Ionomer-based cement is a new bone replacement material. After promising results in middle ear and skull base surgery, we started to use Ionocap as an alternative to metal osteosynthesis material. Between 1992 and 1995, 20 men and 9 women were treated with an osteoplastic approach for complicated frontal sinus diseases at Johann Wolfgang Goethe University. Ionocap as bonding material for bone was used in 22 of the patients. The indications for osteoplastic surgery were recurrent frontal rhinosinusitis, mucoceles, skull base fractures, osteomas and meningocele. The surgical procedure using bone

Prolonged Jackson-Pratt drainage in the management of lumbar cerebrospinal fluid leaks.

BACKGROUND: Cerebrospinal fluid (CSF) leak is a complication of spinal surgery. Intraoperative or postoperative identification of a CSF leak often results in wound healing complications, lumbar drain placement, and/or reoperation. These complications usually extend a patient's hospital stay, can be painful, and have their own associated risks. The authors describe a technique that may improve on traditional interventions by managing postoperative CSF leaks after lumbar instrumentation without an additional procedure or extended hospitalization. METHODS: A retrospective review of lumbar instru

Mid-term clinical results of Graf stabilization for lumbar degenerative pathologies. a minimum 2-year follow-up.

BACKGROUND CONTEXT: Spinal fusion has some adverse effects, such as nonunion and pain at the site of grafted bone, and fusion with rigid spinal instrumentation especially may have the possibility of increasing mechanical stress on the segments adjacent to the site of fusion. The theory of the Graf system is that it will decrease adjacent disc deterioration because of maintenance of regional lordosis with flexibility and restriction of the motion of unstable segments without rigid spinal fusion. PURPOSE: To assess the clinical and radiologic results of Graf stabilization for lumbar degenerativ
