Management of elderly hip fractures by an orthopaedic trauma surgeon reduces surgical delays but does not improve outcomes compared to non-trauma surgeons.

PURPOSE: Recent literature on hip fractures has focussed on the optimal environment for best outcomes. One factor that has not been studied is the managing surgeon's training background. Our study aims to examine if hip fracture patients managed by fellowship-trained orthopaedic trauma surgeons have better outcomes compared to non-trauma trained general orthopaedic surgeons. METHODS: This is a retrospective study performed at a tertiary hospital with an established orthogeriatric co-managed hip fracture care pathway. All surgically treated elderly hip fracture patients over a period of 2 year

Restoration of longitudinal forearm stability using a suture button construct.

PURPOSE: This study proposed a method of restoring the longitudinal stability of the forearm provided by the central band of the interosseous membrane (IOM) by using a percutaneously placed suture button construct. We hypothesized that supporting the forearm IOM with a suture button construct would restore longitudinal stability in a cadaveric model of the Essex-Lopresti lesion. METHODS: We assessed 7 adult cadaver upper extremities radiographically for evidence of previous elbow, forearm, or wrist fracture. Each limb was mounted onto a materials testing system with the elbow held at 90° and

Modified Nuss procedure in patients with recurrent pectus excavatum following the Ravitch procedure with a retained strut: report of two cases.

BACKGROUND: Pectus excavatum is a prevalent congenital chest wall deformity that is often treated using surgical methods such as the Ravitch or Nuss procedures. Although both techniques are effective, recurrence is possible. We present two cases of recurrent pectus excavatum in patients who had undergone Ravitch procedures during childhood, with retained struts identified during evaluation > 15 years later. These patients underwent repair using the Nuss procedure. CASE PRESENTATION: Two adult patients with a history of pectus excavatum treated using the Ravitch procedure in their childhood ex

Content versus correction: Analyzing hallux valgus surgery across social media platforms.

BACKGROUND: While social media use in medicine grows, patients increasingly encounter medical content at various stages of treatment; however, little is known about the nature or tone of posts related to bunion procedures. PURPOSE: This study aims to characterize the content, authorship, and sentiment of such posts to better understand patient exposure. STUDY DESIGN: An exhaustive cross-sectional content analysis queried Instagram®, Facebook®, and Reddit® for "bunion surgery," identifying the 100 most recent posts per platform. METHODS: Posts were coded for surgery type, author, topic, medi

Extensor Tendon Injuries: A New Classification, Strong Repairs, and Easier Therapy.

A new 5-zone classification of extensor tendons in the hand and forearm is proposed to regroup surgical methods and discuss outcomes. Similar to flexor tendon zones, the new classification unifies the zones of fingers and thumbs. Based on this classification, strong surgical repairs (6-strand repairs) are always used for zone 2. In zone 1c and 3, 6-strand, 4-strand, or other repair methods are used. The finger is protected with short splinting with early active motion or hand use. Many principles of flexor tendon repairs are applicable to these extensor tendon areas. A solid strong repair abol

Comparison of Costs and Energy Expenditure in Common Hand Surgery: Operating Theatre Versus Ward Procedure Room.

The health care sector is the fifth largest contributor to greenhouse emissions globally. Results of several studies have demonstrated that altering surgical practices from inpatient to outpatient service was associated with cost savings, reduction of medical waste and energy expenditure. The objective of this study was a comparison of costs and energy expenditure for hand surgeries performed in an operating theatre versus a ward procedure room. The analysis included the cost of materials, cost of sterilization of surgical instruments and energy expenditure for sterilization generated at stand

Criteria for medication reconciliation in major orthopedic surgery in high-risk patients: A consensus based on the Delphi method.

BACKGROUND: Medication reconciliation is relevant in transitional care, however, given limited resources, it is necessary to identify the patients who benefit most from this activity. AIM: To validate criteria to identify patients at high risk of medication errors undergoing major orthopedic surgery. METHOD: Delphi Method in 3 phases, April-June 2023, to obtain consensus on the inclusion criteria, previously defined. Each expert rated criteria according to a 5-point Likert scale. Consensus was assumed in round 1 if the rate average was ≥4 (inclusion) or <2 (exclusion) and in rounds 2 and 3 i

Management of the in-season athlete with an anterior shoulder dislocation.

BACKGROUND: Management of the in-season athlete presenting with an anterior shoulder dislocation is a nuanced process that continues to be refined. Options and pathways between nonoperative and operative treatment have undergone many iterations over a century of orthopedic research and advancement. It requires an understanding of sport-specific demands and the individual athlete's goals. The orthopedic surgeon must have mastery of the natural history, treatment options, and outcomes of anterior shoulder dislocations. Balance of these factors is delicate and highly individualized for each athle

Obesity and the Risk of Venous Thromboembolism after Major Lower Limb Orthopaedic Surgery: A Literature Review.

The risk of venous thromboembolism following total joint arthroplasty is significantly greater than those of other types of elective orthopaedic procedures. This risk is increased in obesity due to the associated prothrombotic physiological and hematological changes that predispose to embolic events. The prevalence of obesity is increasing in the aging population, which contributes to a further increase in the risk of postoperative thrombosis in the older patients. There is a lack of clear evidence regarding dosing information for thromboprophylaxis medications in patients with obesity. As a r

Lengthening Behavior of Magnetically Controlled Growing Rods in Early-Onset Scoliosis: A Multicenter Study.

BACKGROUND: The "law of diminishing returns" is described for traditional growing rods. Magnetically controlled growing rods (MCGRs) have become a preferred implant for the surgical treatment of early-onset scoliosis (EOS). We examined a large cohort of patients with EOS to determine whether the law of diminishing returns applies to MCGRs. METHODS: A prospectively collected, multicenter registry was queried for patients with EOS treated with MCGRs. Patients with only spine-based implants and a minimum of 2 years of follow-up were included; patients with congenital scoliosis, single rods, <3 l
