Orthoses play an important therapeutic role in the rehabilitation of children with cerebral palsy (CP). Studies examining the factors that physiotherapists consider when prescribing orthotics are limited in the literature. This study aimed to examine physiotherapists' perspectives and experiences with using walking orthoses in patients with CP. A total of 207 physiotherapists working with children with CP were included in this online cross-sectional study. The questionnaire consisted of examining the knowledge of the physiotherapist about walking orthoses, such as the types of orthoses that ar

This study aims to clarify the toilet seat height and inclination conditions that promote sit-to-stand motion for users with simulated kyphosis and reduced trunk mobility. The study was performed on healthy subjects with spinal orthosis to simulate a kyphotic posture and reduced trunk mobility (simulated kyphosis) in which the toilet seat was either not adjusted ("standard setting"), supplemented by 3 cm or 5 cm ("3 cm higher" or "5 cm higher"), or tilted forward by 5° or 15° ("5° tilt" or "15° tilt"). Subjective evaluation using sensory evaluation and objective evaluation using floor reaction

This study investigated the impact of varying the tension of the elastic band in a lower-limb orthosis with a trunk belt (hereinafter, lower-limb orthosis) on muscle activity in the lower limbs during walking in healthy adults. Within-subject repeated-measures pilot study. University motion analysis laboratory. The task involved walking 10 m on flat ground for 18 healthy adult males. Two conditions were assessed: walking without lower-limb orthosis and with the orthosis applied to the right lower limb. For the latter, the elastic-band tension in stationary standing position was set to 3 levels

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper extremity. Conservative treatments are effective for treating mild and moderate CTS. There is still a need for studies to investigate the superiority of conservative treatments over each other.
PURPOSE: The aim of our study was to compare the efficacy of orthosis treatment and Kinesio taping (KT) treatment on pain, functional status, and grip strength in patients with idiopathic CTS.
STUDY DESIGN: Randomized clinical trial.
METHODS: Sixty patients (90% women) who were diagnosed with mild or modera

BACKGROUND: Prior to the introduction of disease-modifying treatments (DMTs), children with type 1 spinal muscular atrophy (SMA) typically did not survive beyond the age of 2 years; management was mainly palliative. Novel therapies have made this a treatable condition, resulting in increased life expectancy and more time spent upright. Survival and improved function mean spinal asymmetry is a new complication with limited data on its prevalence and severity and no current guidelines on management and treatment. This study aimed to evaluate the spinal presentation and management of type 1 SMA c

BACKGROUND: The purpose of this study was to evaluate the rate of residual acetabular dysplasia (RAD), defined as an acetabular index (AI) of >90th percentile of age and sex-matched controls, in a cohort of infants successfully treated with the Pavlik harness (PH).
METHODS: We retrospectively studied typically developing infants at a single center, with at least 1 dislocated hip, that was successfully treated with a PH and had a minimum of 48 months follow-up. Hip dislocation was defined as <30% femoral head coverage at rest on pretreatment ultrasound or IHDI grade 3 or 4 on the pretreatment

PURPOSE: Late presentation of hip dysplasia persists despite robust screening methods. After 6 months of age, treatment with a hip abduction orthosis becomes challeng-ing, and all other treatment modalities have higher reported rates of complications.
METHODS: We performed a retrospective review of all patients from 2003 to 2012 who had the sole diagnosis of de-velopmental hip dysplasia, who presented before 18 months of age, and who had at least 2 years of follow-up. The cohort was then grouped based on their presentation before (BSM) or after (ASM) 6 months of age. The groups were compared

OBJECTIVE: Trigger finger is a common clinical disorder that often results in pain and functional limitations. This study aimed to determine whether joint-blocking orthosis was effective in treating adult idiopathic trigger fingers.
METHODS: Fifty-five participants were randomly assigned to either a hand therapy group or an orthosis group that immobilized the proximal interphalangeal joint for 6 weeks. The Green classification of triggering; short-form version of the Disabilities of the Arm, Shoulder, and Hand; and numeric pain rating scale were used to assess participants' data.
RESULTS: Th

Background The current widely applied Graf classification used on US images for developmental dysplasia of the hip in infants does not enable prediction of the development and outcome of well-centered stable dysplastic hips (Graf type II). Purpose To use statistical shape modeling on US images to identify acetabular shape characteristics of Graf type II hips, which enable prediction of the development of Graf type II hips, and to identify which hips benefit from Pavlik harness treatment. Materials and Methods In this secondary analysis of a prospective multicenter randomized trial on treatment

BACKGROUND: There is a lack of early (infant) mobility rehabilitation approaches that incorporate natural and complex environments and have the potential to concurrently advance motor, cognitive, and social development. The Grounded Early Adaptive Rehabilitation (GEAR) system is a pediatric learning environment designed to provide motor interventions that are grounded in social theory and can be applied in early life. Within a perceptively complex and behaviorally natural setting, GEAR utilizes novel body-weight support technology and socially-assistive robots to both ease and encourage mobili

STUDY DESIGN: Case report.
INTRODUCTION: Radial nerve injury can cause severe functional impairment due to paralysis of wrist and digit extensors. Various orthotic designs have been described, including static, dynamic, and tenodesis. All provide wrist stabilization or extension assistance. Some, but not all, also provide extension assistance to the wrist, thumb, and fingers.
PURPOSE AND METHODS: This article tells the story of Max, a 27-year-old male university student, who sustained a radial nerve injury after a left humeral shaft fracture. He was treated at a Brazilian tertiary hospital,

Spinal orthotic bracing is a common modality for treating nonoperative spinal fractures with risks. This study aimed to assess the effect of an intervention on critical care nurses to improve their clinical knowledge and comfort level of managing patients. A literature review was conducted regarding common complications associated with spinal orthotics. This information was compiled and used to create a questionnaire and spinal orthotic course for nurses. Pre- and postassessments of nurses' knowledge regarding spinal orthotics were conducted. A total of 197 nurses completed the presentation. T

The purpose of this study was to compare the effects of different ankle and knee supports on balance in early ambulation of post-stroke hemiplegic patients. This is a randomized experimental study. The study sample included 20 hemiplegic patients who were able to stand with support and who had been diagnosed with a cerebrovascular accident within the last month. A knee immobilization brace (KIB) was first placed on each individual (first application), followed by placement of knee immobilizer brace and Foot Lifter Orthosis ® (FLO) (second application), and lastly, placement of KIB and rigid ta

OBJECTIVES: The development of neurorehabilitation technologies requires the profound understanding of the mechanisms underlying an individual's motor ability and impairment. A major factor limiting this understanding is the difficulty of bridging between events taking place at the neurophysiologic level (i.e., motor neuron firings) with those emerging at the musculoskeletal level (i.e. joint actuation), in vivo in the intact moving human. This review presents emerging model-based methodologies for filling this gap that are promising for developing clinically viable technologies.

BACKGROUND: Shoulder subluxation is a common secondary impairment of the upper limb following stroke. A range of supportive devices are used in rehabilitation to prevent shoulder subluxation, including hemi-slings and firm supports, such as arm troughs, however, there is little evidence regarding their efficacy.
AIM: To determine whether a modified lap-tray during sitting and a triangular sling during standing is more effective than a hemi-sling in preventing shoulder subluxation, pain, contracture and upper limb activity limitation after stroke.
DESIGN: A prospective, randomized trial with

Neuromuscular scoliosis (NMS) is the second most prevalent spinal deformity (after idiopathic scoliosis) and is usually first identified during early childhood. Cerebral palsy (CP) is the most common cause of NMS, followed by Duchenne muscular dystrophy (DMD). Progressive spinal deformity causes difficulty with daily care, walking and sitting, and can lead to back and rib pain, cardiac and pulmonary complications, altered seizure thresholds, and skin compromise. Early referral to specialist spinal services and early diagnosis of NMS is essential to ensure appropriate multidisciplinary patient

As therapists and physicians, we often need to work with our patients to cater treatment accordingly. These authors describe how they modified a treatment protocol for a patient that sustained a proximal interphalangeal joint comminuted fracture and dorsal dislocation, but refused surgery. Their modification allowed the patient to return to full activities. - Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.
