Chronic nonbacterial osteomyelitis: the role of whole-body MRI.

BACKGROUND: Chronic nonbacterial osteomyelitis (CNO), also known as chronic recurrent multifocal osteomyelitis, is a noninfectious autoinflammatory disorder that occurs primarily in children and adolescents and is characterized by episodic musculoskeletal pain with a protracted course. MAIN BODY: Traditionally, the diagnosis of CNO is made by exclusion and commonly requires bone biopsy to rule out infection and malignancy. However, bone biopsy may be avoided when imaging and clinical characteristic features are present, such as multifocal bone lesions at typical sites, no constitutional sympt…

Mono-Lateral External Fixation for Treatment of Femoral Osteomyelitis.

BACKGROUND AND AIM: Maintenance of stability using external fixation devices is an important principle to ensure successful treatment of osteomyelitis (OM). In this study, we report our experience with femoral OM treated with acute compression and bone transport using the Orthofix limb reconstruction system (LRS). PATIENTS AND METHODS: This prospective study included 30 consecutive patients with femoral OM. LRS insertion and corticotomy were done according to the standard technique. Radiographic evaluation was performed every 2 weeks during the distraction phase and every 2-4 weeks during the…

Fungal Osteomyelitis due to Phaeoacremonium venezuelense: A Case Report.

CASE: A 56-year-old immunosuppressed man presented with pain and swelling in the medial and anterior right foot with accompanied numbness in the second and third toes 1 month after a puncture wound by a Sylvester palm tree thorn. An intraoperative culture/biopsy returned positive for septic arthritis of the naviculocuneiform joint and fungal osteomyelitis of the navicular, medial, and intermediate cuneiforms due to Phaeoacremonium venezuelense. CONCLUSION: Fungal osteomyelitis is extremely rare. Only 5 cases by Phaeoacremonium venezuelense have been reported previously in the literature. To t…

Classification Model for Diabetic Foot, Necrotizing Fasciitis, and Osteomyelitis.

Diabetic foot ulcers (DFUs) and their life-threatening complications, such as necrotizing fasciitis (NF) and osteomyelitis (OM), increase the healthcare cost, morbidity and mortality in patients with diabetes mellitus. While the early recognition of these complications could improve the clinical outcome of diabetic patients, it is not straightforward to achieve in the usual clinical settings. In this study, we proposed a classification model for diabetic foot, NF and OM. To select features for the classification model, multidisciplinary teams were organized and data were collected based on a l…

Are Surrogate Markers for Diabetic Foot Osteomyelitis Remission Reliable?

BACKGROUND: We aimed to evaluate surrogate markers commonly used in the literature for diabetic foot osteomyelitis remission after initial treatment for diabetic foot infections (DFIs). METHODS: Thirty-five patients with DFIs were prospectively enrolled and followed for 12 months. Osteomyelitis was determined from bone culture and histologic analysis initially and for recurrence. Fisher exact and χ2 tests were used for dichotomous variables and Student t and Mann-Whitney U tests for continuous variables (α = .05). RESULTS: Twenty-four patients were diagnosed as having osteomyelitis and 11 as…

Mucor Osteomyelitis of the Distal Radius Necessitating Ulnocarpal Fusion.

This is a case report of a 60-year-old female who developed distal radius osteomyelitis secondary to Mucor infection from likely hematogenous spread that was managed with ulnocarpal wrist fusion. Following serial debridement and systemic antifungal therapy, ulnocarpal wrist fusion offered functional limb salvage rather than amputation in this patient with significant operative risk and comorbidities.

Bacillus subtilis revives conventional antibiotics against Staphylococcus aureus osteomyelitis.

As treatment of Staphylococcus aureus (S. aureus) osteomyelitis is often hindered by the development of antibiotic tolerance, novel antibacterial therapeutics are required. Here we found that the cell-free supernatant of Bacillus subtilis (B. subtilis CFS) killed planktonic and biofilm S. aureus, and increased S. aureus susceptibility to penicillin and gentamicin as well. Further study showed that B. subtilis CFS suppressed the expression of the genes involved in adhesive molecules (Cna and ClfA), virulence factor Hla, quorum sensing (argA, argB and RNAIII) and biofilm formation (Ica and sarA)…

Mind the Localized Skeletal Pain: Chronic Recurrent Multifocal Osteomyelitis.

Chronic recurrent multifocal osteomyelitis (CRMO) is a rare idiopathic aseptic inflammatory bone disorder affecting primarily children and adolescents characterized by an insidious onset of pain, swelling, and tenderness over the affected bones. The clinical signs and symptoms of CRMO are nonspecific, radiological and histopathological tests are essential for its diagnosis. We present a case of an 18-year-old young man who was diagnosed with CRMO by a combination of clinical data, laboratory results, radiological imaging, and bone biopsy. The patient started anti-inflammatory and immunosuppres…

Chronic Nonbacterial Osteomyelitis: Insights into Pathogenesis, Assessment, and Treatment.

Chronic nonbacterial osteomyelitis, or its most severe form, chronic recurrent multifocal osteomyelitis, is an autoinflammatory bone disease that causes skeletal inflammation characterized by bone pain and swelling that primarily affects children. It is a diagnosis of exclusion and its clinical presentation may mimic underlying infectious processes and malignancy. Clinical suspicion for this diagnosis and timely referral to pediatric rheumatology is crucial to achieve earlier diagnosis, appropriate treatment, and improved quality of life of affected patients and families. This article focuses …

Case of the Season: Asymmetric Chronic Recurrent Multifocal Osteomyelitis.

Chronic recurrent multifocal osteomyelitis is a rare noninfectious inflammatory bone disease diagnosed based on the synthesis of clinical, radiological, and pathological findings. The differential diagnostic considerations are led by multifocal infectious osteomyelitis and multifocal neoplasms. We report a case of a 9-year-old girl who emergently presented with worsening back pain, inability to walk, and normal vital signs. C-reactive protein and erythrocyte sedimentation rate were elevated, whereas the white blood cell count was normal. Initial radiographs and MRI of the spine showed multiple…

Treatment of femoral defective osteomyelitis with minimal invasive plates.

Although recent treatment modalities reduced chronic osteomyelitis, it is still a challenging problem for both orthopaedic surgeons and patients. Especially treatment of femoral osteomyelitis with defective nonunion is reasonably difficult. Most accepted treatment option is external fixation. But the duration needed for external fixation may be too long for patient compliance. To reduce external fixation duration several methods were described. In this study we described a method for reconstruction of infected femoral defective nonunions using minimal invasive plates.

Management of Deep Sternal Wound Infection: Complete Sternal Osteomyelitis.

Deep sternal wound infection (DSWI) after cardiac surgery is a challenging complication that affects the outcome of surgery. The worst type of DSWI is mediastinitis and sternal osteomyelitis, which dramatically increase morbidity, mortality, and cost of care. This case report describes successful treatment of sternal osteomyelitis after open heart surgery with combined negative pressure wound therapy and rectus abdominis flap. This combination of negative pressure wound therapy with rectus abdominis flap in treating sternal osteomyelitis after open cardiac surgery is not well studied.

Prediction of Adverse Outcomes in Pediatric Acute Hematogenous Osteomyelitis.

BACKGROUND: Clinicians cannot reliably predict complications of acute hematogenous osteomyelitis (AHO). METHODS: Consecutive cases of AHO from 2 pediatric centers in the United States were analyzed retrospectively to develop clinical tools from data obtained within 96 hours of hospitalization to predict acute and chronic complications of AHO. Two novel composite prediction scores derived from multivariable logistic regression modeling were compared with a previously published severity of illness (SOI) score, C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR) using area under t…

Contribution of radiobiphosphonates bone scan in multifocal tuberculous osteomyelitis.

We share the case of a 86-year-old male patient, followed up for recent left hip pain. Interrogation revealed a fever, nocturnal shivering, … e cervical and dorsal spine. Biopsy of the left humeral head confirmed the diagnosis of tuberculous osteomyelitis, showing an aspect of caseo-follicular tuberculosis. Bone scan with radiobiphosphonates can be used in our context to search other localizations falling within the framework of a polystotic form, and also to guide nonrisk biopsy in such group of pat …
