Relationship between parathyroid hormone levels and sarcopenia and nutritional status in patients undergoing hemodialysis.

INTRODUCTION: The potential effect of elevated parathyroid hormone (PTH) levels on nutritional status and sarcopenia in chronic kidney disease (CKD) patients undergoing hemodialysis (HD) has not yet been well established. OBJECTIVE: The aim of this study was to evaluate the status of sarcopenia and nutritional aspects according to PTH levels in HD patients. METHODS: This cross-sectional study evaluated consecutive patients attending three HD clinics in Northeast Brazil. Patients were stratified based on PTH levels: (1) 0-199 pg/mL, (2) 200-599 pg/mL, and (3) ≥ 600 pg/mL. Group 1 was excluded…

Intraoperative Parathyroid Hormone in Minimally Invasive Parathyroidectomy (MIP) for Primary Hyperparathyroidism With Concordant Localisation.

OBJECTIVE: There is increasing use of intraoperative parathyroid hormone (ioPTH) in minimally invasive parathyroidectomy (MIP). While ioPTH has improved cure rate, there is little evidence to suggest effectiveness in primary hyperparathyroidism (PHP) with concordant preoperative localisation. This study aims to determine if ioPTH improves cure rates in such cases. DATA SOURCES: A search of PubMed, Embase, and Cochrane databases identified studies that compared MIP with and without ioPTH, in patients with concordant preoperative imaging of ultrasonography and technetium-99m sestamibi (MIBI) sc…

Blockade of Gi Signaling Enhances the Anabolic Effect of Parathyroid Hormone in Female Mice.

Intermittent administration of parathyroid hormone (PTH) produces potent anabolic effects on bone. These anabolic effects appear to be mediated by activation of the Gs-cyclic AMP system initiated by the PTH1 receptor (PTHR1) in osteoblast lineage cells. Gs-mediated cyclic AMP production can be inhibited by activation of Gi signaling, and we have previously demonstrated that Gi signaling in osteoblasts suppresses bone formation and thereby plays a prominent role in the bone loss associated with aging in female mice. It is not clear whether this increased Gi signaling with aging dampens the anab…

Predictors of persistently elevated parathyroid hormone levels after parathyroidectomy: experience from a reference center.

OBJECTIVE: Primary hyperparathyroidism is considered the third most common endocrine disorder. Moreover, persistently elevated levels of parathyroid hormone occur in up to 30% of patients undergoing parathyroidectomy. The aim of this study was to determine the prevalence of persistently elevated parathyroid hormone levels after parathyroidectomy and analyze its associated factors. METHODS: An anonymized medical record query was conducted, filtering patients who underwent parathyroidectomy at a tertiary hospital center in Southern Brazil from 2010 to 2019. Clinical, laboratory, and sociodemogr…

Challenging Medical and Surgical Management of Ectopic Parathyroid Hormone (PTH) Adenomas in the Elderly.

Ectopic parathyroid adenomas (EPAs) are rare and difficult to localize, making surgical cures challenging on the first attempt. However, the … howed only one 1.6cm nodule in the thyroid, and no PA was detected. Further, fine needle aspiration parathyroid hormone (PTH) wash-out from that thyroid nodule showed very high PTH levels, referring to the thyroid's EPA. Meanwhile, she was screened for complications of PHPT, and nephrolitiasis and kyphosis due to severe osteoporosis were found. She underwent minimall …

Tubular functional capacity and maladaptive parathyroid hormone response in early-stage chronic kidney disease.

Clinical data regarding the interaction between tubular functional capacity (TFC) and maladaptive parathyroid gland response in early-stage chronic kidney disease (CKD) are limited. This study aimed to evaluate the association between parathyroid gland response, measured as intact parathyroid hormone (iPTH) serum concentration (pg/mL) using chemiluminescent microparticle immunoassay, and the dissociation between the decline in glomerular filtration rate (GFR) and TFC, assessed through radionuclide clearances. TFC was evaluated by measuring effective renal plasma flow (mERPF, ml/min/1.73m²) usi…

Skeletal parathyroid hormone hyporesponsiveness: a neglected, but clinically relevant reality in chronic kidney disease.

PURPOSE OF REVIEW: Defining the optimal parathyroid hormone (PTH) target in chronic kidney disease (CKD) is challenging, especially for bone outcomes, due to the substantial variability in the skeleton's response to PTH. Although PTH hyporesponsiveness is as integral a component of CKD-mineral bone disorder as elevated PTH levels, clinical awareness of this condition is limited. In this review, we will discuss factors and mechanisms contributing to PTH hyporesponsiveness in CKD. This knowledge may provide clues towards a personalized approach to treating secondary hyperparathyroidism in CKD. …

Diagnosing Primary Hyperparathyroidism in Pregnancy: A Case of Altered Parathyroid Hormone Degradation in Pregnancy.

Diagnosing primary hyperparathyroidism in pregnancy is difficult due to pregnancy-related changes in parathyroid hormone (PTH); calcium; 1,25 vitamin D; and renal calcium excretion. Parathyroid hormone-related peptide (PTHrP) produced by the placenta adds additional complexity. Our case is the first to demonstrate an increased rate of PTH degradation within a pregnant individual who returned unexpectedly low PTH levels. We describe a 27-year-old female patient who presented at 25 weeks gestation with pancreatitis and hypercalcemia. Primary hyperparathyroidism was suspected but variable PTH res…

Persistent elevation of parathyroid hormone after curative parathyroidectomy: A risk factor for recurrent hyperparathyroidism.

BACKGROUND: Up to 45% of patients may have persistently elevated parathyroid hormone (PTH) levels after curative parathyroidectomy for primary hyperparathyroidism (PHPT), although the clinical significance is unclear. We aimed to assess the long-term clinical significance of persistently elevated PTH early after parathyroidectomy. METHODS: A prospectively collected institutional database was queried for patients who underwent parathyroidectomy for sporadic PHPT between 12/99 and 6/22 and had normal serum calcium levels at 6 months postoperatively. Demographic and clinical data were collected,…

Case report: Ectopic production of intact parathyroid hormone (iPTH) by malignoma mimicking primary hyperparathyroidism.

BACKGROUND: Malignant hypercalcemia is usually caused by osteolytic processes of metastases, production of parathormone-related peptide, or secretion of 1,25-dihydroxyvitamin D. Ectopic PTH (parathyroid hormone) production by malignancy is very unusual. METHODS: Case report and review of the literature. RESULTS: We present a case of a malignant hypercalcemia with a presentation that mimicked primary hyperparathyroidism in a patient with endometrial carcinoma. Finally, ectopic production of PTH by a rapidly progressive neuroendocrine tumor was proven. Systematic literature review revealed ect…

Case report: Familial hypoparathyroidism with elevated parathyroid hormone due to an inactivating PTH mutation.

INTRODUCTION: So far, only 11 PTH mutations have been described as causes of familial isolated hypoparathyroidism (FIH). In this report, we describe a family with FIH but with significant elevation of functionally inactive PTH due to a PTH mutation. We also show a positive therapeutic outcome of recombinant human PTH (teriparatide) therapy in one of the siblings who was not well controlled on large doses of calcitriol and calcium replacement therapy. CASE DESCRIPTION: The proband is a 34-year-old woman who has a history of chronic severe hypocalcemia (HypoCa) since birth. She and her three br

Parathyroid Hormone (PTH)-Related Peptides Family: An Intriguing Role in the Central Nervous System.

Parathyroid Hormone (PTH) plays a crucial role in the maintenance of calcium homeostasis directly acting on bone and kidneys and indirectly on the intestine. However, a large family of PTH-related peptides exists that exerts other physiological effects on different tissues and organs, such as the Central Nervous System (CNS). In humans, PTH-related peptides are Parathyroid Hormone (PTH), PTH-like hormones (PTHrP and PTHLH), and tuberoinfundibular peptide of 39 (TIP39 or PTH2). With different affinities, these ligands can bind parathyroid receptor type 1 (PTH1R) and type 2 (PTH2R), which are pa…

Parathyroid hormone stimulates bone regeneration in an atrophic non-union model in aged mice.

BACKGROUND: Non-union formation still represents a major burden in trauma and orthopedic surgery. Moreover, aged patients are at an increased risk for bone healing failure. Parathyroid hormone (PTH) has been shown to accelerate fracture healing in young adult animals. However, there is no information whether PTH also stimulates bone regeneration in atrophic non-unions in the aged. Therefore, the aim of the present study was to analyze the effect of PTH on bone regeneration in an atrophic non-union model in aged CD-1 mice. METHODS: After creation of a 1.8 mm segmental defect, mice femora were …

Density-Dependent Differentiation of Tonsil-Derived Mesenchymal Stem Cells into Parathyroid-Hormone-Releasing Cells.

Mesenchymal stem cells (MSCs) can differentiate into endoderm lineages, especially parathyroid-hormone (PTH)-releasing cells. We have previously reported that tonsil-derived MSC (T-MSC) can differentiate into PTH-releasing cells (T-MSC-PTHCs), which restored the parathyroid functions in parathyroidectomy (PTX) rats. In this study, we demonstrate quality optimization by standardizing the differentiation rate for a better clinical application of T-MSC-PTHCs to overcome donor-dependent variation of T-MSCs. Quantitation results of PTH mRNA copy number in the differentiated cells and the PTH concen

The Stability of Intact Parathyroid Hormone (PTH) in Different Types of Blood Collection Tubes.

BACKGROUND: Over past decades, the instability of parathyroid hormone (PTH) causes great interference for the clinical laboratory. Contradictory results were reported in many reports about storage conditions and suitable blood collection tubes to ensure PTH stability in the pretreatment phase. METHODS: This study recruited 30 participants including 10 healthy persons, 10 hemodialysis, and 10 hyperparathyroidism patients. Five types of blood collection tubes (EDTA-K3 tube, coagulant tube, heparin anticoagulant tube, gel separating tube, and plain tube) were included to determine whether they w…

Parathyroid Hormone Regulates Circulating Levels of Sclerostin and FGF23 in a Primary Hyperparathyroidism Model.

Parathyroid hormone (PTH) increases fibroblast growth factor 23 (FGF23), mediated both by protein kinase A (PKA) and Wnt signaling, and decreases expression of sclerostin, a Wnt antagonist derived from osteocytes. Patients with primary hyperparathyroidism (PHPT) have lower serum sclerostin levels than healthy controls, consistent with the idea of SOST downregulation by PTH. Nevertheless, the relationship between FGF23 and sclerostin in PHPT is still unclear. We examined this issue in a mouse model of PHPT. PHPT mice had increased FGF23 and decreased sclerostin expression in calvaria and in the…
