Application of Flow Cytometry in Primary Immunodeficiencies: Experience From India.

Primary immunodeficiency diseases (PID) are a clinically and immunologically heterogeneous group of disorders of immune system. Diagnosis of these disorders is often challenging and requires identification of underlying genetic defects, complemented by a comprehensive evaluation of immune system. Flow cytometry, with its advances in the last few decades, has emerged as an indispensable tool for enumeration as well as characterization of immune cells. Flow cytometric evaluation of the immune system not only provides clues to underlying genetic defects in certain PIDs and helps in functional val

Endosomal signaling disorders: a new category for immune dysregulation.

Disorders involving endosomal signaling pathways are gaining attention due to their roles in immune regulation. This review proposes a new disease category, endosomal signaling disorders (ESDs), to describe disorders caused by dysfunction of endosomal molecules. Dysregulated endosomal signaling leads to hyperactivation of immune responses, contributing to immune dysregulation disorders such as systemic lupus erythematosus and systemic autoinflammatory disorders. The identification of pathogenic variants in endosomal genes such as TLR3, TLR7, TLR8, UNC93B1, and PLD4 emphasizes the importance of

DRESS syndrome presenting with acute hypoxic respiratory failure and delayed eosinophilia.

Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a rare, yet life-threatening hypersensitivity reaction. It typically presents with fever, lymphadenopathy, eosinophilia and systemic organ dysfunction. Atypical presentations of DRESS syndrome, including elevated liver enzymes and acute respiratory distress syndrome, have previously been reported in rare cases. Here, we describe an otherwise healthy male patient in his mid-50s who presented with a rash, myalgia, shortness of breath and delayed eosinophilia that rapidly progressed to acute respiratory failure with hypoxia

Recommendations for evaluation and diagnosis of extra-glandular manifestations of primary sjogren syndrome: results of an epidemiologic systematic review/meta-analysis and a consensus guideline from the Brazilian Society of Rheumatology (articular, pulmonary and renal).

Sjogren's Syndrome (SS) is an autoimmune disease characterized by lymphocytic infiltration of the exocrine glands and other organs, associated with sicca syndrome but also with systemic involvement with varying degrees of severity. Despite their importance, these systemic manifestations are not routinely evaluated and there is no homogenous approach to their diagnosis or evaluation. To close this gap, a panel of experts from the Brazilian Society of Rheumatology conducted a systematic review and meta-analysis on the identification of epidemiologic and clinical features of these manifestations

Hyperinflammation and derangement of renin-angiotensin-aldosterone system in COVID-19: A novel hypothesis for clinically suspected hypercoagulopathy and microvascular immunothrombosis.

Early clinical evidence suggests that severe cases of coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), are frequently characterized by hyperinflammation, imbalance of renin-angiotensin-aldosterone system, and a particular form of vasculopathy, thrombotic microangiopathy, and intravascular coagulopathy. In this paper, we present an immunothrombosis model of COVID-19. We discuss the underlying pathogenesis and the interaction between multiple systems, resulting in propagation of immunothrombosis, which through investigation in the c

Complications of long-term home total parenteral nutrition: their identification, prevention and treatment.

The purpose of this review is to describe the most common complications of home total parenteral nutrition, their identification, treatment and prevention. Data sources were manuscripts and abstracts published in the English literature since 1968. Studies were selected for summarization in this review on the basis of clinical relevance to the practicing clinician. Home total parenteral nutrition is a relatively safe, life-saving method for nutrient delivery in patients with compromised gastrointestinal function. However, numerous complications, with associated morbidity and mortality, involvin

Novel illuminoss photodynamic bone stabilization system: normal and post-operative complication imaging findings in the emergency setting.

The mainstay orthopedic surgical technique for fracture fixation involves metal plates, screws, and rods. While these methods are effective, they exhibit high rates of complications within specific populations, particularly among patients with pathologic and insufficiency fractures. IlluminOss represents a novel photodynamic bone stabilization system, approved for use in multiple countries, that serves as an alternative to traditional fracture fixation approaches for patients experiencing pathologic, traumatic, and fragility fractures. Despite the initial success of the system in fostering fra

A rare cause of ischemic colitis: polyarteritis nodosa.

This paper details the first case report of a patient with fulminant, gangrenous, ischemic colitis caused by polyarteritis nodosa which was successfully treated surgically. Ischemic colitis is a rare complication of polyarteritis nodosa. It should be suspected in patients with a past history of polyarteritis nodosa who develop abdominal pain and rectal bleeding. The identification of cutaneous lesions preoperatively and or mesenteric or serosal vascular beading at operation are helpful in establishing this diagnosis. Prognosis is generally determined by the extent of systemic involvement by po

Vasculitis. A practical approach to management.

Vasculitis encompasses a wide variety of diseases. Because diagnosis may be difficult, a careful evaluation is essential, including a detailed patient history, thorough physical examination, and appropriate laboratory studies. Diagnosis is based on clinicopathologic features that permit identification of the condition. Biopsies are often necessary to confirm a diagnosis. It is important to accurately categorize the vasculitic disorders, since prompt, aggressive therapy with potentially toxic drugs is necessary to avoid irreversible organ system dysfunction.

Oral lesions as an initial manifestation of dermatomyositis with occult malignancy.

Dermatomyositis (DM) is an inflammatory disease of skeletal muscle with characteristic cutaneous features. Oral manifestations of DM have been very rarely described. This case describes a patient in whom the oral lesions were among the initial manifestations of DM. The lesions were very painful and the most distressing component of the disease in this patient. High dose systemic steroids were required to control the lesions. Investigations for an occult malignancy in this patient resulted in the identification of a non-Hodgkin's lymphoma in her right kidney. Commencement of chemotherapy rapidl

Effectiveness of fracture liaison services in osteoporosis.

BACKGROUND: In response to the gradual decline in the number of prescriptions for anti-osteoporosis medication (AOM) following fragility fractures, fracture liaison services (FLSs) have been set up around the world with the aim of filling this treatment gap. Several studies have already reported the benefits of such organizations, particularly in reducing fracture risk, mortality rates and healthcare costs, and literature on FLSs has increased at a steady pace over time. METHODS: A narrative review was conducted on the latest available findings on the effectiveness of FLSs. Various approaches

Improving Care Transitions to Drive Patient Outcomes: The Triple Aim Meets the Four Pillars.

PURPOSE: The purpose of this article is to examine how case managers can support positive outcomes during care transitions by focusing on the goals of the Triple Aim () and Coleman's Four Pillars (). Case managers can play a pivotal role to ensure high-quality transitions by assessing patients and identifying those who are at high risk; coordinating care and services among providers and settings; reconciling medications; and facilitating education of patients and their support systems to improve self-management. These activities are congruent with an underlying value of case management as defi

Profound olfactory dysfunction in myasthenia gravis.

In this study we demonstrate that myasthenia gravis, an autoimmune disease strongly identified with deficient acetylcholine receptor transmission at the post-synaptic neuromuscular junction, is accompanied by a profound loss of olfactory function. Twenty-seven MG patients, 27 matched healthy controls, and 11 patients with polymiositis, a disease with peripheral neuromuscular symptoms analogous to myasthenia gravis with no known central nervous system involvement, were tested. All were administered the University of Pennsylvania Smell Identification Test (UPSIT) and the Picture Identification T

Predictors of postpartum coparenting relationship among primiparous couples in Karachi, Pakistan.

INTRODUCTION: The postpartum coparenting relationship is an important determinant of parental well-being, relationship functioning, and child development during the transition to parenthood. However, most studies combine mothers' and partner's data, limiting the identification of parent-specific predictors of the postpartum coparenting relationship. This study examined predictors of the postpartum coparenting relationship among primiparous Pakistani couples in Karachi by analyzing mothers' and partners' data separately. METHODS: The full study recruited primiparous mothers and their partners

Development and validation of a machine learning-based diagnostic system for 22 pediatric respiratory pathogens: a large-scale multicenter study.

Respiratory tract infections (RTIs) are a significant cause of morbidity in children, caused by a wide range of pathogens. As treatment strategies depend on the causative pathogen, early and accurate diagnosis is crucial. We developed and validated an interpretable Pathogen Diagnostic System for Pediatric Respiratory Infections (Pathog-PDx), conducting a multicenter study involving 134,500 hospitalized children across three clinical centers and two databases. The model integrated 42 clinical and laboratory features from electronic health records (EHR) to enable early pathogen identification. P
