We report a case of transitional cell carcinoma of a renal pelvis in a woman discovered after childbirth. A 38-year-old woman, who delivered an immature male infant in cesarean section 29 days prior to hospitalization, was admitted complaining of asymptomatic gross hematuria. Excretory urography and retrograde pyelography showed a filling defect of the right renal pelvis, Spontaneous urine cytology indicated class 5. Renal computed tomographic scan demonstrated a mass lesion in the right kidney. Right total nephroureterectomy and partial cystectomy was performed for diagnosis and treatment. Pa…

Intermittent drainage (ID) of the renal pelvis is probably due to episodes of aperistalsis in the pelvo-calyceal system. The incidence of ID in adult patients referred for renography for various nephro-urological conditions was found to be 4.5%. Presence of ID was not related to sex or age but more common at a low urine flow. It was then usually bilateral and interpreted as a normal phenomenon. If the urine flow at the renographic examination exceeded 2 ml min-1, ID was typically unilateral and there was often a history of obstructive uropathy and dilatation of the renal pelvis. However, there…

A prospective study comprised operative specimens from 11 patients with transitional cell carcinoma of the renal pelvis or ureter. DNA analysis of the primary tumor and of multiple biopsy specimens from preselected sites of the surrounding urothelium was performed with flow cytometry. All Grade 3 tumors and 50% of the Grade 2 tumors were aneuploid, and the remainder were Grade 2 and diploid. All invasive tumors were aneuploid. Carcinoma in situ was found in some of the preselected biopsy specimens, all of which were aneuploid, from two patients. Close correlation thus was observed between aneu…

A small mass was detected in the renal pelvis during investigation for hypertension and diminished renal function. Its presence was confirmed by computed tomography. Since the patient was not a candidate for nephrectomy, interventional percutaneous techniques were used to confirm the diagnosis of transitional cell carcinoma and ultimately permit endourologic access for resection and cure.

Among 3540 trauma patients received at the A.B.U. Hospital, Zaria between 1972 and 1982, 350 had fractures of the pelvis, and 127 of these had associated injuries of the urinary bladder and urethra. They comprised 90 men and 37 women. Bladder and urethral contusion were the commonest injuries in 56 patients who required only catheter drainage. Surgical explorations and repairs were necessary in 71. Of the anatomical categories, pelvic posterior fractures occurred in 55 patients and were associated with a larger proportion of major non-urinary injuries, ruptured bladder and a greater fatality r…

Use of a rigid ureteroscope for extraction of 19 ureteric and 6 renal pelvic stones is reported. One of the 24 patients had bilateral ureteric stone. The extraction was successful in 22 cases, including 9 after ultrasonic disintegration of stone. Surgical removal of stone was required because of failure of the ultrasound apparatus in one case and ureteric perforation in another. One stone was extracted "blind". The method has many advantages over more established procedures. It should be the preferred technique for management of ureteric calculi, and may be an option for some renal pelvic ston

A case of rare inverted papilloma of the renal pelvis with cytologic study of ureteral washings is described. Cytologic findings included increased numbers of urothelial cell clusters with mild degenerative changes. Although cytology was helpful in establishing the presence of a papillary transitional-cell lesion, histologic analysis following resection was required to distinguish between the possibilities of transitional-cell carcinoma and inverted papilloma.

The cytologic features of a primary adenocarcinoma of the renal pelvis in a 64-year-old woman are reported. Cytologic examination of voided urine revealed an ordinary papillary pattern, characterized by clumping of cells with nuclear overlapping, eccentric position of nuclei and cytoplasmic vacuolization due to mucus production. Leukocytes and micro-organisms were also present. Multiple lesions of papillary adenocarcinoma were subsequently found in the right renal pelvis and ureter. The histogenesis of the primary adenocarcinoma seemed to be closely related to columnar-cell metaplasia of linin…
