CAPD-associated peritonitis caused by Alcaligenes xylosoxidans sp. xylosoxidans.

Alcaligenes xylosoxidans is an uncommon cause of peritonitis in patients on maintenance continuous ambulatory peritoneal dialysis (CAPD). Peritonitis caused by A. xylosoxidans usually carries a poor prognosis because of the pathogen's virulence and its universal resistance to most antimicrobial agents. Even after early Tenckhoff catheter removal, the transport property of the peritoneum is often irreversibly damaged, leading to permanent technique failure. We report 2 patients with CAPD-associated peritonitis due to A. xylosoxidans sp. xylosoxidans who were successfully cured with a combinatio…

Spontaneous biliary peritonitis in acalculous cholecystitis: fact or misdiagnosis?

It is often speculated that an inflamed gallbladder weeps bile to produce bile peritonitis. This may be so, but more likely the problem is a peritoneal effusion in a jaundiced patient which thus resembles bile. So-called "spontaneous or idiopathic biliary peritonitis" in acute acalculous cholecystitis without a proven cause is a further example of this very rare condition. Spontaneous perforations of the extrahepatic biliary ductal system associated with acalculous cholecystitis are uncommon albeit reported in adults. Most patients present with an acute abdomen and are operated upon urgently w…

A rare case of primary group A streptococcal peritonitis.

Primary Group A streptococcal peritonitis is a rare clinical entity that is almost always associated with underlying disease. Group A streptococcus commonly causes upper respiratory tract infections and cutaneous infections such as impetigo and erysipelas. However, Group A streptococcus has rarely been associated with gastrointestinal infections. This is a case report describing a previously healthy adult male who developed primary Group A streptococcal peritonitis. Diagnostic laparoscopy resulted in identification of peritonitis without an identifiable intra-abdominal source. Appropriate anti…

[Role of prolonged intraarterial catheter therapy in general peritonitis].

The analyses of treatment results in 82 patients with general purulent peritonitis are presented. In these patients long-term intraarterial catheter therapy (IACT) was included in the complex of postoperative treatment after elimination of infection source. Postoperative peritonitis was in 66 patients, 52 patients of them underwent reoperations for persisting general peritonitis. Use of IACT promoted arrest of abdominal inflammatory process and polyorganic insufficiency due to high concentration of antibiotics in the affection focus, improvement of regional circulation, reduced number of posto…

Microbiological spectrum of septicemia and peritonitis in nephrotic children.

From April 1993 to December 1997, 452 admissions of 231 children with nephrotic syndrome to Chang Gung Children's Hospital were retrospectively reviewed. There were 10 episodes of sepsis and 8 episodes of peritonitis in 18 children, and 14 microorganisms were cultured. Two children died due to Streptococcus pneumoniae sepsis. Gram-positive microorganisms (n=7) and Gram-negative microorganisms (n=7) were found in equal numbers. Enterococcus (1), Streptococcus pneumoniae (4), group D streptococcus (1), and Streptococcus viridans (1) were the Gram-positive microorganisms cultured. Two of 4 cases …

Acute lupus peritonitis successfully treated with steroid pulse therapy.

A 21-year-old man with systemic lupus erythematosus (SLE) who developed acute lupus peritonitis is described. Acute lupus peritonitis appeared during generalized lupus flare, with nausea, vomiting, frequent diarrhea, and abdominal tenderness with rebound and guarding. The patient was afebrile and had decreased bowel sounds. Abdominal ultrasonography and computed tomography revealed marked thickening of the gastric, duodenal, and jejunal walls, massive intraluminal fluid collection, and increasing ascites. Gastrointestinal endoscopy showed edematous mucosa with multiple erosions of the stomach …

The influence of age on the severity of peritonitis.

OBJECTIVE: To evaluate the influence of age on the evolution and severity of peritonitis. DESIGN: A chart review. SETTING: An adult university hospital. PATIENTS: One hundred and twenty-two patients with acute appendicitis and 100 patients with acute colonic diverticulitis requiring operation or percutaneous drainage. MAIN OUTCOME MEASURES: Patient age and sex, presence of perforation or gangrene (appendicitis), extent of peritonitis (diverticulitis); duration of symptoms prior to admission; admission leukocyte count; duration of hospitalization before surgery; length of hospital stay; and…

The natural history of meconium peritonitis diagnosed in utero.

The authors reviewed their experience with meconium peritonitis (MP) diagnosed in utero to define criteria for prenatal and postnatal management. Prenatal diagnosis was made by identifying abdominal calcification on serial ultrasound examinations in nine fetuses, between 18 and 37 weeks' gestation. Cases without associated bowel abnormalities were considered "simple MP" and those with bowel abnormalities were considered "complex MP." Five cases of simple MP were identified at 18, 23, 30, 34, and 37 weeks' gestation. These five fetuses were delivered at term and had normal abdominal examination…

Campylobacter jejuni enterocolitis causing peritonitis, ileitis and intestinal obstruction.

Patients with Campylobacter enterocolitis may come to laparotomy due to the severity of abdominal symptoms and signs, although only two patients with intestinal inflammation have been described and in neither was the histopathology documented. A case of a 52-year-old male who had a typical diarrhoeal illness of Campylobacter enterocolitis diagnosed on stool culture is reported. Despite appropriate treatment he developed signs and symptoms consistent with small intestinal obstruction. Laparotomy revealed peritonitis and thickened distal ileum with transmural inflammatory changes on histopatholo…

[Acute peritonitis as a clinical manifestation of xanthogranulomatous pyelonephritis].

Xanthogranulomatous pyelonephritis can become clinically apparent in a variety of manners. The paper contributes a very infrequent presentation; a picture of acute peritonitis following non-focal generalized toxic syndrome and haematuria episodes.

Ruptured mesothelial cyst of the spleen causing acute peritonitis.

Congenital splenic cysts are rare (only 40% of all true cysts). A case report is presented of a young male adolescent patient with a ruptured mesothelial splenic cyst causing acute peritonitis. Abdominal ultrasound and CT-scan are very useful tools to obtain the preoperative diagnosis. Spleen saving resections, if possible, are preferred to splenectomy because of the risk of severe post-splenectomy sepsis.

[A method of peritoneostomy for treating diffuse purulent peritonitis].

For improvement of exudate outflow and prevention of the development of abscesses, a modified method of peritoneostomy has been suggested and successfully used in treatment of 59 patients. After elimination of a source of peritonitis and sanitation of the abdominal cavity, the greater omentum is covered with a hydrocellulose film of corresponding sizes, rolled up as a rouleau, and placed in the epigastrium. The intestinal loops are covered with a perforated polyethylene film sutured to the ends of a laparotomy wound.

[An improvement in the combined treatment of acute peritonitis].

The object of the research was to study regional lymph circulation in abdominal organs in experimental acute peritonitis and determination of the possibility of including lymphotropic therapy in the complex of measures for the management of patients with acute peritonitis. Regional lymph circulation in the abdominal organs was studied in 45 unbred rats, the effect of various lymphostimulators of regional lymph circulation was studied in 35 rats. Lymphotropic therapy was applied in the complex treatment of 106 patients with acute peritonitis in the clinic. The results of the study showed that l…
