Gasless laparoscopic varicocele ligation: experience with new instrumentation and technique for retroperitoneal and intraperitoneal approaches.

Laparoscopic access to the retroperitoneum without abdominal insufflation has recently been made possible by devices designed to create a dilated laparoscopic cavity maintained by a fixed retraction system. This technique was applied in 10 patients undergoing laparoscopic ligation of the internal spermatic vein via extraperitoneal and intraperitoneal approaches. Gasless laparoscopic varicocelectomy was completed in 7 of 8 men by the extraperitoneal route and in neither of the 2 men approached intraperitoneally. The mean operative time was 170+/-55 min and postoperative pain exceeded the norm f

[Chilaiditi's syndrome-A rare anatomical variant of the colon].

Chilaiditi's sign is a rare, usually asymptomatic anatomical variant in which a segment of the colon, most commonly the transverse colon, is interposed between the diaphragm and the liver. This interposition can be misinterpreted as a pneumoperitoneum on radiographic imaging, which therefore poses diagnostic challenges. While many cases are discovered incidentally and require no treatment, symptoms such as abdominal pain, nausea or constipation can occur. In such cases, the condition is referred to as Chilaiditi's syndrome. The causes are multifactorial and include liver atrophy, colonic redun…

[Pyloric stenosis in infants. Contribution of ultrasonography and video-surgery].

Hypertrophic pyloric stenosis is the commonest condition requiring abdominal surgery in infancy. The Fredet-Ramstedt pyloromyotomy gives a v … py in 19 infants. This new surgical technique using laparoscopy with precautionary measures for the pneumoperitoneum appears to be very promising and should become a widespread technique in the future.

Pneumothorax, Pneumomediastinum, and Cervical and Facial Massive Emphysema Secondary to Colonoscopy: A Rare Complication of Colonoscopy.

BACKGROUND: Colonoscopy is a resource used for the diagnosis, treatment, and monitoring of benign and malignant colorectal pathologies. The incidence of perforation is 0.03%-0.65% in diagnostic colonoscopy; however, the incidence can be up to 10 times higher in therapeutic interventions, such as polypectomies, increasing the risk of complications up to 0.07-2.1%. Materials and methods. Case report of a 71-year-old female who presents a rare complication due to a perforation in the sigmoid which developed pneumoperitoneum, pneumomediastinum, pneumothorax, and massive subcutaneous emphysema as a…

Pneumatosis intestinalis presenting with acute abdomen.

A woman in her 70s attended the emergency department with sudden-onset severe epigastric pain, abdominal distension and sinus bradycardia of 37 beats per minute. Her relevant past medical history included irritable bowel syndrome, axillary node clearance for breast cancer and surgical removal of an ovarian cyst as a young woman. A CT scan showed pneumoperitoneum and air in the small bowel wall and findings suggestive of volvulus and bowel obstruction. Emergency laparotomy revealed extensive small gas-filled blisters in the submucosa of the mid-small bowel, consistent with pneumatosis intestina…

Small intestine necrosis in catastrophic antiphospholipid syndrome: A rare and severe case.

Catastrophic antiphospholipid syndrome (CAPS) is a multisystem autoimmune disease with widespread thrombotic events. In this case report, we present a young man with primary antiphospholipid syndrome (PAPS) admitted to the hospital with abdominal pain and vomiting. Abdominal computed tomography showed pneumoperitoneum and acute explorative laparotomy revealed small intestinal necrosis indicating small vessel thrombosis without involvement of large intestine. "Triple therapy" was initiated after surgery and the patient was treated in an intensive care unit for 72 days before being discharged to…

Perforated Meckel's diverticulum in a very preterm baby revealed at birth.

Perforated Meckel's diverticulum (MD) in a preterm baby is very rare. We report a case of a very preterm baby, born at 29-week gestation, with a birth weight of 1400 g, admitted in the third hour of life to our intensive care unit (ICU) for respiratory distress syndrome with abdominal distention. An abdominal radiograph showed a pneumoperitoneum. Laparotomy revealed Meckel's perforation. The baby was discharged healthy at the age of 16 days. MD should be kept in mind as one cause of an acute abdomen in preterm neonates mimicking necrotizing enterocolitis. To our knowledge, our patient is the t…

Spontaneous perforation of Meckel's diverticulum without peritonitis in a newborn: report of a case.

A case of a newborn male with a perforation of Meckel's diverticulum is reported. The clinical course consisted of progressive abdominal distention and pneumoperitoneum that formed within 29 h after birth. The perforation of Meckel's diverticulum was not associated with peritonitis because meconium did not contaminate the abdominal cavity. The histology of the diverticulum showed a nearly intact muscular layer but a focal muscular defect. Neither any inflammatory phenomena nor ectopic mucosa was found. A congenital focal muscular defect of the diverticulum and a sudden elevation of intralumina…

Pneumopericardium in blunt chest trauma after high-speed motor vehicle accidents.

Pneumopericardium is the presence of air in the pericardial space. In adults, it may be seen in the context with severe blunt chest trauma, pneumothorax, pneumoperitoneum, or other causes of pneumomediastinum. The diagnosis is made by computed tomography scan of the thorax and abdomen that allows the additional detection of concomitant injuries. Possible causes of the pneumopericardium such as tracheobronchial or oesophageal tears have to be excluded by bronchoscopy or esophagogastroduodenoscopy. Usually, pneumopericardium is self-limiting requiring no specific therapy. However, a continuous m…

Approach to a giant inguinoscrotal hernia.

We present an extremely huge and longstanding, giant inguinoscrotal hernia extending to below the knee with an ulcer at its base. Though hernias of this magnitude are rare, their management can be demanding and challenging. Nevertheless, in an emergency situation, the repair of the hernial defect is not essential, especially in a compromised patient. In fact, the most important step is excision of the devitalised tissue, and the final surgery can be delayed. In nonemergency management, definitive surgery can be planned either by a period of preoperative staged pneumoperitoneum, repairing after…

[Laparoscopic cholecystectomy in pregnancy. A description of 2 cases and review of the literature].

Although pregnancy is still widely considered to be a contraindication to laparoscopic cholecystectomy, several successful cases have been published so far. This paper reports on two cases of laparoscopic cholecystectomy performed during the second trimester for recurrent biliary pancreatitis and acute cholecystitis. There were no intra- or postoperative complications, the obstetric course was also uneventful in both cases. Important aspects to be considered are the placement of trocars and the pressure of the pneumoperitoneum, which should not exceed 12 mmHg. The need for intraoperative monit…

Alternative laparoscopic technique for cholecystectomy during pregnancy.

Laparoscopic cholecystectomy had been considered contraindicated during pregnancy. A few brief clinical reports have appeared in the literature, but no trials attesting to the safety to the fetus (and mother) have been published. Laparoscopic cholecystectomy during pregnancy has advantages for the mother in that it speeds her recovery, giving her less pain, and decreasing the chance of incisional hernias from large incisions. Using an open, Hasson technique combined with low-pressure pneumoperitoneum via an "abdominal wall lift," safety to the fetus is enhanced. Herein is described a laparosco…
