Although the routine use of a patch for closure of the arteriotomy after carotid endarterectomy is still debated, patch angioplasty is frequently used to avoid residual stenosis and to lower the incidence of later restenosis. Several materials are widely used for carotid patching, notably, autologous vein, Dacron and polytetrafluorethylene (PTFE). During recent years a heterologous stabilized graft made from bovine pericardium has been introduced. This paper reports the authors' experience of 52 carotid endarterectomies in which a patch of bovine pericardium was used to close the arteriotomy.

A retrospective review of long-term follow-up of Niebauer trapeziometacarpal arthroplasty for treatment of disabling arthritis of the basal joint was performed. Thirty implants in 27 patients were reviewed, with an average follow-up of 9 years (minimum, 4 years). All surgical procedures were performed by the senior author. Eighty-eight percent of the patients were subjectively pleased and would undergo the procedure again. Postoperative subluxation occurred in 83% of the patients. Sixty-five Dacron cuffed, dual lumen, silicone central venous dialysis catheters (Quinton PermCath, Seattle, WA) were inserted in 51 patients as the sole form of permanent access for chronic hemodialysis. Six and 12 month actuarial survival rates of patients for all catheters were 53% and 35%, respectively. When calculations included revisions, 6 and 12 month actuarial catheter survival rates were 61% and 43%, respectively. Between March 1986 and September 1988, 38 patients underwent extended aortic resection (aortic valve, ascending aorta, and arch) for acute type-A aortic dissection with aortic valve insufficiency; deep hypothermia and circulatory arrest were used. Double lumen subclavian venous hemodialysis catheters are in wide use in the United States to provide temporary vascular access. The disadvantages of these catheters include a high infection rate and short use-life (2 to 3 weeks). We report three cases of ureteral obstruction producing hydronephrosis following aortofemoral bypass graft. In each case, the obstruction occurred in the area where in the ureter crossed over the limb of a Dacron graft. Ureterolysis relieved the obstruction. Most of the previously reported cases of this complication of bypass surgery occurred when the ureter was situated between the limb of a graft and the patient's own iliac artery. We report an unusual complication of a Dacron graft in a 31-year-old woman who had undergone extra-anatomical bypass for coarctation of the aorta aged 11 years. She presented with haemoptysis, and imaging confirmed an aneurysm of the Dacron graft at the proximal anastomosis with the aorta, eroding into the right upper lobe, as well as a smaller aneurysm at the anastomosis with the descending aorta. She underwent a median sternotomy and refashioning of the bypass, and a re-do right thoracotomy with removal of the Dacron graft and a right upper and middle lobectomy.

The best surgical approach for patients presenting with coarctation of the aorta and additional surgical cardiovascular disorders is uncertain. We describe the case of a young man with an aneurysm of the ascending aorta and a bicuspid aortic valve with a moderate insufficiency associated with a coarctation of the aorta. We describe a modified surgical technique to treat patients with a previous history of isolated aortic valve replacement who now require aortic root replacement for an aneurysmal or dissected aorta. This technique consists of replacing the aortic root with a Dacron conduit, leaving intact the previously implanted prosthesis, and re-implanting the coronary arteries in the Dacron graft.
