[The value of ultrasound-guided multiple systematic biopsies in the early diagnosis of cancer of the prostate].

Endorectal ultrasonography using a 7 mHZ probe was performed in 99 patients with a negative history for surgery of the prostate and either findings on rectal examination suggestive of cancer (T3 stage excluded) or a serum level of prostate-specific antigen of 2.5 mg/ml or more. A biopsy gun was used to harvest 1.5 centimetre long specimens. Ultrasound-guided biopsies were performed in peripheral hypoechogeneic areas; in addition, regardless of the result of the endorectal ultrasonography, routine multiple ultrasound-guided biopsies were performed in both lobes of the prostate (3 per lobe). Mor…

Evaluation of Prostate Cancer Screening Strategies in a Low-Resource, High-risk Population in the Bahamas.

IMPORTANCE: The benefit of prostate-specific antigen screening may be greatest in high-risk populations, including men of African descent in the Caribbean. However, organized screening may not be sustainable in low- and middle-income countries. OBJECTIVE: To evaluate the expected population outcomes and resource use of conservative prostate-specific antigen screening programs in the Bahamas. DESIGN SETTING AND PARTICIPANTS: Prostate cancer incidence from GLOBOCAN and prostate-specific antigen screening data for 4300 men from the Bahamas were used to recalibrate 2 decision analytical models p…

Targeting the tumour cell surface in advanced prostate cancer.

Prostate cancer remains a substantial health challenge, with >375,000 annual deaths amongst men worldwide. Most prostate cancer-related deaths are attributable to the development of resistance to standard-of-care treatments. Characterization of the diverse and complex surfaceome of treatment-resistant prostate cancer, combined with advances in drug development that leverage cell-surface proteins to enhance drug delivery or activate the immune system, have provided novel therapeutic opportunities to target advanced prostate cancer. The prostate cancer surfaceome, including proteins such as pros

Origin of carcinomas causing bladder neck obstruction demonstrated by immunoperoxidase localisation of specific antigens.

The immunohistological demonstration of prostate specific acid phosphatase, prostate specific antigen, epithelial membrane antigen and keratin was used to ascertain the origin of five advanced carcinomas involving the bladder neck and causing obstruction. Five prostatic carcinomas, 10 transitional cell and 10 squamous cell carcinomas with schistosomiasis of the bladder were studied for comparison. The concomitant localisation of prostate specific antigen and keratin was most useful in ascertaining the origin of tumour invading the bladder in contrast to the multiple use of other epithelial mar

Prostate cancer and chronic prostatitis.

Prostate cancer and chronic prostatitis are prevalent disorders in men. The cause of prostate cancer and chronic prostatitis is multifocal and diverse. Both disorders exhibit characteristic elevation of serum prostate-specific antigen, currently the primary screening test for prostate cancer. Prostate inflammation, regardless of cause, is the histopathologic hallmark of chronic prostatitis. In general, inflammation is associated with multiple cancers, and prostate inflammation, in particular, is a suggested factor in the development and progression of prostate cancer. This review addresses the…

Results of a screening program for prostate cancer in patients scheduled for abdominoperineal resection for colorectal pathologic findings.

OBJECTIVES: Because of the difficulty of accessing the prostate for tissue sampling after surgical removal of the rectum and obliteration of the anus, we started an early detection program for prostate cancer in all men scheduled for abdominoperineal resection. METHODS: Twenty consecutive men were screened for prostatic adenocarcinoma before planned abdominoperineal resection for colorectal pathologic findings. Patients were 48 to 77 years old (mean 66.9). Screening included serum prostate-specific antigen determination and digital rectal examination. Those patients with suspicious findings u…

Hepatic failure-induced hypogonadism in a prostate cancer patient.

Hypogonadism owing to systemic diseases in prostate cancer is rare. Here, we present a patient with metastatic prostate cancer to the pericardium who had low serum testosterone level due to hepatic failure. The patient had cardiac tamponade, and pericardiocentesis revealed sanguineous exudate. Cytology revealed adenocarcinoma. High serum prostate-specific antigen level of 244 ng/mL was detected. The patient experienced complications of stress gastric and duodenal ulcer perforation and underwent subtotal gastrectomy. Perioperative intra-abdominal inflammatory process caused subsequent cholestas…

Seizure Disorders in Adults: Primary Care for Patients With Epilepsy.

Prostate cancer is the second most common cancer in US men. Guidelines for screening vary, underscoring the need to engage patients in shared decision-making and counsel them about potential benefits and harms. When screening is undertaken, it is based on the prostate-specific antigen test, with digital rectal examination used selectively as an adjunct. Men with an abnormal prostate-specific antigen level should have a repeat test within a few months, before proceeding to biomarker testing, referral to urology, biopsy, and potentially imaging. Patients with newly diagnosed prostate cancer shou…

A castrate-resistant metastatic prostate cancer patient with severe pancytopenia, successfully treated with docetaxel chemotherapy.

INTRODUCTION: Prognosis of metastatic castrate-resistant prostate cancer is poor with a median survival of 12 to 36 months. Bone metastasis is common, and bone marrow metastasis occurs later in the disease course. The median survival in these patients after bone marrow involvement is less than six months. We report a case of castrate-resistant prostate cancer patient presented with severe pancytopenia due to bone marrow involvement of prostate cancer, treated successfully with docetaxel chemotherapy. Post chemotherapy, the patient became transfusion independent and prostate-specific antigen im…

Prostate Cancer Screening: Common Questions and Answers.

Prostate cancer is the most diagnosed noncutaneous malignancy and the second most common cause of cancer death among men in the United States. Risk factors include older age, family history of prostate cancer, and Black race. Screening via prostate-specific antigen testing may lead to a small reduction in prostate cancer-specific mortality, with no reduction in all-cause mortality, but it can cause significant harms related to false-positive test results, unnecessary biopsies, overdiagnosis, and overtreatment. Shared decision-making is strongly recommended by all national guidelines before ini…

Clinical Outcomes and Testosterone Levels Following Continuous Androgen Deprivation in Patients with Relapsing or Locally Advanced Prostate Cancer: A Post Hoc Analysis of the ICELAND Study.

PURPOSE: Lower serum testosterone levels correlate with improved cause specific survival and longer time to progression in year 1 of continuous androgen deprivation in men with prostate cancer. ICELAND was a large European study demonstrating the efficacy of leuprorelin (Eligard®) during continuous androgen deprivation. In this post hoc analysis we investigated serum testosterone levels within year 1 of continuous androgen deprivation to determine survival and time to progression. MATERIALS AND METHODS: In ICELAND (ClinicalTrials.gov NCT00378690) patients with locally advanced or relapsing no

Prostatic carcinoma with elevated carcinoembryonic antigen: a case report.

Carcinoembryonic antigen is a glycoprotein often associated with colorectal carcinoma but can also increase in other malignancies. We reported a rare case of prostate mucinous adenocarcinoma in a 69-year-old male with elevated carcinoembryonic antigen and normal prostate-specific antigen. The patient, with a history of benign prostatic hyperplasia, was initially asymptomatic apart from an elevated carcinoembryonic antigen detected during routine examination. Comprehensive evaluations, including colonoscopy, CT and PET/CT, yielded no conclusive findings. However, a pelvic MRI later revealed an …

Radiopharmaceutical Combination Therapies in Advanced Prostate Cancer.

Prostate-specific membrane antigen is a transmembrane glycoprotein that is highly specific and overexpressed in prostate cancer cells, making it an effective target for theranostics. Despite significant progress in recent years with prostate-specific membrane antigen-targeting radiopharmaceutical therapies that show promising anticancer activity and safety profiles, resistance is inevitable and underpins the need for combinatorial approaches. In this article, we discuss the progress made toward radiopharmaceutical therapy combination approaches, including those involving androgen receptor path

Risk Stratification of Suspected Clinically Localized Prostate Cancer Using Multi-Indicator Functional Differentiation: A Predictive Model With fPSA/tPSA, PHI, PCA3, and MRI.

BACKGROUND: Accurate risk stratification is crucial for managing men with suspected clinically localized prostate cancer, particularly those with total prostate-specific antigen (tPSA) in the diagnostic grey zone (4-10 ng/mL). This study aimed to develop and validate a predictive model integrating multi-dimensional indicators to distinguish clinically insignificant prostate cancer from significant disease. METHODS: This retrospective cohort study analysed 242 patients with suspected clinically localized prostate cancer who underwent biopsy from January 2020-December 2021. Patients were strati…
