Nonsurgical Treatment of Radiculopathy From a Lumbar Synovial Cyst Using the Minimally Invasive Lumbar Decompression (MILD) Procedure: A Case Report.

Lumbar synovial cysts are fluid-filled lesions arising from the facet joints that can cause lateral recess stenosis and neurogenic claudication. While surgical decompression is the gold standard for symptomatic cysts, patients with significant comorbidities, such as chronic obstructive pulmonary disease (COPD) and emphysema, face elevated perioperative risks under general anesthesia and delayed wound healing. This case report describes the successful application of the minimally invasive lumbar decompression (MILD) procedure as a safe alternative for a high-risk surgical candidate. A 68-year-o

Split cord malformations in adults.

PURPOSE: Split cord malformations represent a small group among tethered cord syndromes. This paper presents the largest series of adult patients with this disorder reported to date. Neuroradiological features, clinical symptoms, surgical management and data on short- and long-term outcomes are analyzed. METHODS: 94 adults (mean age 54.7 ± 15 years) presented with split cord malformations between 1991 and 2024 and were evaluated (follow-up of 64 ± 84 months). Radiological features, intraoperative findings, and neurological examinations before and after surgery were analyzed. Long-term outcome

A comparison of piezoelectric surgery and conventional techniques in the enucleation of cysts and tumors in the jaws: a systematic review and meta-analysis.

BACKGROUND: Despite the comprehensive classifications provided by the WHO, the most common lesions include radicular cysts, dentigerous cysts, odontogenic keratocysts, ameloblastomas, and odontomas. The piezoelectric technique has shown effectiveness in removing intraosseous pathologies by relying on ultrasonic microvibrations, which help preserve soft and vascular tissues. Precision in manipulating intraosseous pathology can impact the prognosis and improve the surgical procedure by controlling hemorrhage and promoting microscopic benefits. While previous research has compared the advantages …

Benign Pediatric Jawbone Lesions: A 10-Year Clinical and Radiological Pilot Study.

This study aimed at presenting a retrospective longitudinal analysis of the pediatric jaw lesions treated at the Oral and Maxillofacial Surg … luded. The ratio of men to women was 1.15:1, with the mandible predominating by 64.4%. Inflammatory radicular cysts predominated in 31.7% of cases. A total of 42.68% of the patients were asymptomatic. Enucleation was the most frequent surgical technique (45.1%), followed by cystectomies (28%) and marsupialization (14.6%). The overall recurrence rate was 7.3%; th …

Clinical update--the teeth and the maxillary sinus: the mutual impact of clinical procedures, disease conditions and their treatment implications. Part 2. Odontogenic sinus disease and elective clinical procedures involving the maxillary antrum: diagnosis and management.

Although odontogenic sinusitis is a rare entity when compared to sinus disease of rhinogenic origin, it is extremely important to identify a … as cysts and tumours may involve the jaws and hence the maxillary antrum; some of these, such as a radicular cyst are quite common, but the rarer ones are included for completeness. Surgical techniques are continuously evolving to optimise form and function of the jaws, and when applied to the maxilla there may be some impact on nasal and sinus function. The ad …

Oral and masticatory rehabilitation using osseointegrated dental implants after resective treatment of multicystic ameloblastoma in the lower jaw with a fibula graft.

Ameloblastoma is an aggressive odontogenic tumor which typically occurs between third and fourth decade of life that often needs resective approach. Immediate reconstruction may show better results. The treatment of multicystic ameloblastoma in the mandible being a rare case that occurred in the late second decade of life, which was surgically removed along with the affected teeth with safety margins, and the region was immediately reconstructed using a vascularized graft, removed from the fibula. Its integration, in combination with osseointegrated dental implants and fixed implant-supported

Computer-aided diagnosis of periapical cyst and keratocystic odontogenic tumor on cone beam computed tomography.

BACKGROUND AND OBJECTIVES: In this article, we propose a decision support system for effective classification of dental periapical cyst and keratocystic odontogenic tumor (KCOT) lesions obtained via cone beam computed tomography (CBCT). CBCT has been effectively used in recent years for diagnosing dental pathologies and determining their boundaries and content. Unlike other imaging techniques, CBCT provides detailed and distinctive information about the pathologies by enabling a three-dimensional (3D) image of the region to be displayed. METHODS: We employed 50 CBCT 3D image dataset files as

Extranodal large B cell lymphoma of the anterior maxilla. Case report and review of literature.

In the oral cavity, lymphoproliferative disorders can manifest in various ways, often as an extranodal externalization. In the case presented here, it was a B cell lymphoma originating in the periapical bone of the anterior maxilla. X-ray revealed a periapical radiolucency associated with an intact tooth with no decay, fillings or history of trauma. The tooth tested non-vital. After root canal treatment, an apicoectomy was performed with a biopsy. The most common diagnosis would be of dental etiology. The pathology report revealed a non-Hodgkin's B cell lymphoma. Most often, this disease appea

A bacteriological and histological evaluation of 58 periapical lesions.

Periapical tissue from 58 cases requiring periapical surgery was examined histologically and cultured for the presence of microbes. Twenty-n … n particulate matter thought to be root canal sealer. Bacteria were found in periapical granulomas, radicular cysts, and a periapical abscess. According to our data, bacteria, foreign material, missed canals, vertical root fractures, and periodontal disease may all contribute to the chronic, non-healing periradicular lesion.

[Clinico-radiological picture of periradicular cysts in the lumbosacral region].

After a review of the modern views on the problem of the so called Tarlov's cysts the authors present a group of 20 patients with chronic low back pains in whom periradicular cysts were diagnosed demonstrating them by means of Dimer-X radiculography. In the light of an analysis of this material the authors reach the conclusion that there are no specific symptoms and signs which could make possible the diagnosis of Tarlov's cysts without contrast radiography of the dural sac. There are, moreover, no reliable data demonstrating that the clinical manifestations in patients with Tarlov's cysts are…

Periapical surgery.

A retrospective study of 572 teeth treated with periapical surgery was undertaken in order to evaluate the influence of preoperative, operative and postoperative factors in the healing process as well as the operation method used. It was also the aim to assess whether the histophatological diagnosis of biopsies taken at the operation could be correlated to healing. Statistical analyses by the AID method and conventional cross-classification methods were performed. Factors of importance were found to be the extent of the destrucion, the operation method used, the bone surrounding the destructio

[A retrospective study of 32 lumbar synovial cysts. Clinical aspect and surgical management].

We report a retrospective analysis of a series of 32 cases of lumbar synovial cysts. Clinically, two populations were identified: patients who presented radicular pain mimicking the clinical diagnosis of disk herniation (group I) and patients who presented neurogenic claudication consistent with a diagnosis of lumbar spinal stenosis (group II). The radiological work-up consisted in standard X-rays and CT-scan for all patients. MRI (magnetic resonance imaging) was performed in 14 patients. All the patients were operated on. The surgical technique consisted in foraminotomy for patients in group
