The authors present a statistics on 32 cases of proctosigmoid junction cancer and discuss diagnostic and management aspects. This localization of the colorectal cancer has certain characteristic features, among which most important are its severe complications. The treatment of choice is the high anterior proctosigmoid resection with restoration of digestive passage, with or without colostomy. In the complicated cases the interventions will be performed in two or even three stages. Hartman's surgery is also indicated in the complicated cancers and in the compromised patients. The postoperative

Of 1,033 patients with colorectal carcinoma, 238 (23%) presented with obstruction. The majority of tumours (74%) were potentially curable (Dukes B/C) at presentation. Tumours situated at the splenic flexure, transverse or descending colon were most likely to obstruct. Perioperative mortality was high following either primary resection (31%) or fashioning of a defunctioning stoma alone (25%) but was twice as high (40%) in patients over 70 than those under 70 (20%). Cardiorespiratory complications accounted for 55% of this mortality, while fatal technical complications occurred in 19%. Sixty sev

Endometriosis is a common disease of women which occasionally causes serious symptoms in virtue of involvement of the distal large intestine. Two cases are reported, illustrating opposite ends of the spectrum of this disorder, one presenting with a complete large-bowel obstruction, and the other with periodical per-rectal bleeding. Endometriosis in these locations can closely mimic carcinoma, and from a consideration of our cases and the literature, those features which might help to distinguish one from the other are discussed. The treatment of endometriosis itself is briefly outlined. Either

In a 46-year-old patient lymphogranulomatosis was found localized exclusively in the rectosigmoid region. The stenosing tumour was removed by resection of the sigmoid colon. No further evidence of abdominal Hodgkin's disease was found at a second operation when splenectomy and careful searching for affected lymph nodes were performed.

The use of the pulsating water jet lavage (Water Pik) in the care of the perineal wound is discussed. The relative case with which debridement of a difficult-to-locate perineal wound can be efficiently debrided and cared for by a patient on an ambulatory basis is stressed. This facilitates early patient hospital discharge and offers effective self-care to the patient.

PURPOSE: Nearly one-third of patients diagnosed with colorectal cancer (CRC) will ultimately develop metastatic disease. While a small percentage of patients can be considered for curative resection, more patients have limited disease that can be considered for local therapy. Challenges remain in defining oligometastatic CRC as well as developing treatment strategies guided by high level evidence.
METHODS: In this review, we present the challenges in defining oligometastatic CRC and summarize the current literature on treatment and outcomes of local therapy in patients with metastatic CRC.

Colorectal cancer is the third most common cancer globally and nearly one fourth of distant metastases are found at the time of the primary diagnosis. Synchronous metastasis of colorectal cancer to the palatine tonsil is rare. To date, only 5 cases have been published in the English literature. In such cases, the prognosis is worse than in other common metastatic sites. Herein, we report a case of rectal adenocarcinoma who presented with a tonsillar mass initially.

BACKGROUND: Anorectal melanoma is a rare and aggressive mucosal cancer. There is usually a delay in diagnosis because about 30% of these cancers are amelanotic and are often mistaken for benign conditions. Herein, we report a case of amelanotic anorectal malignant melanoma with an unusual metastatic deposit in the vulva and also review the literature.
CASE REPORT: A 67-year-old woman presented with a history of prolapse of an anal tumour. Clinical examination showed a pedunculated and ulcerated amelanotic tumour associated with three other nodules, 1 cm in diameter, localized in the vulval mu

A family history of colorectal cancer is the basis for risk stratification and allows reliable screening recommendations for individual patients. It is also the starting point for diagnosing hereditary forms of colorectal cancer. The surgical implications of treating hereditary nonpolyposis colorectal cancer and familial adenomatous polyposis are briefly described. Laparoscopic colectomy and sentinel node biopsies are new techniques with potential application in the treatment of colon cancer. For rectal cancer, advances have resulted in better preoperative staging and improved oncologic and fu
