[Idiopathic hypokalemic periodic paralysis presenting peculiar insulin secretion].

A 24-year-old male was admitted to our hospital because of the paralytic attack. He was well until he went to bed the day before, and he found his limbs unmovable in the morning. The initial attack occurred at age 11 and subsequently he had two episodes of the reversible generalized weakness which always appeared in the morning and continued for about one day at age 14 and 21, respectively. The provocative factors were uncertain. There was no family history of paralytic attacks nor thyroid diseases. On the neurologic examination he presented flaccid tetraparesis without the facial and respirat

Anesthetic adaptations for robotic-assisted pediatric surgery in a resource-constrained setting: a case report.

INTRODUCTION: Robotic-assisted pediatric surgery enhances visualization and dexterity but poses anesthetic challenges in low-resource settings. These challenges are compounded by prolonged operative times, pneumoperitoneum, and restricted patient access. This report describes the first pediatric robotic-assisted choledochal cyst excision performed at a Philippine tertiary hospital, highlighting anesthetic considerations and adaptive strategies in a resource-constrained environment. PRESENTATION OF THE CASE: A female child with a 2-year history of intermittent epigastric pain and no comorbidit

Genotypic variations and clinical implications of JEV-associated peripheral nerve injury: a commentary on multicenter findings from high-endemic regions.

We read with great interest the recent article by Wang et al. on peripheral nerve injury (PNI) associated with Japanese encephalitis virus (JEV) infection in high-endemic regions of China. The study provides important insights into the significant relationship between JEV infection and PNI, particularly highlighting clinical manifestations such as acute flaccid paralysis and respiratory muscle paralysis. While we commend the authors' work, we suggest caution in interpreting the findings due to several limitations. First, genotype-specific differences, notably between GIb and GIII strains, may

Nerve pathology is prevented by linker proteins in mouse models for LAMA2-related muscular dystrophy.

LAMA2-related muscular dystrophy (LAMA2 MD or MDC1A) is a devastating congenital muscular dystrophy that is caused by mutations in the LAMA2 gene encoding laminin-α2, the long chain of several heterotrimeric laminins. Laminins are essential components of the extracellular matrix that interface with underlying cells. The pathology of LAMA2 MD patients is dominated by an early-onset, severe muscular dystrophy that ultimately leads to death by respiratory insufficiency. However, pathology in nonmuscle tissues has been described. Prior work in the dyW /dyW mouse model for LAMA2 MD has shown that t

Acute transverse myelitis progressing to permanent quadriplegia following COVID-19 infection.

As of January 2022, there have been over 350 million confirmed cases of COVID-19 in the world. The most common symptoms in those infected are fever, cough, malaise, and myalgia, however pulmonary, hematologic, gastrointestinal, renal, and neurologic complications have also been reported. Acute transverse myelitis (ATM) is an uncommon neurological syndrome characterized by acute or subacute spinal cord dysfunction that can lead to paresthesias, sensory and autonomic impairment, and even paralysis. Etiologies are often unclear; however, potential causes include infection, neoplastic, drug or tox

Expanding the phenotypic spectrum of TRIM2-associated Charcot-Marie-Tooth disease.

Charcot-Marie-Tooth disease (CMT) is a clinically and genetically heterogeneous group of distal symmetric polyneuropathies due to progressive and length-dependent degeneration of peripheral nerves. Cranial nerve involvement has been described in association with various CMT-genes mutations, such as GDAP1, TRPV4, MFN2, MTMR2 and EGR2. Compound heterozygous mutations in the TRIM2 gene, encoding an E3 ubiquitin ligase, were previously identified in two patients with early-onset axonal CMT (CMT2). One of them also had bilateral vocal cord paralysis. The aim of this study is to further delineate th

Lymphatic imaging and intervention in a pediatric population: Anesthetic considerations.

The recent adoption of an improved lymphatic access technique coupled with Dynamic Contrast-enhanced Magnetic Resonance Lymphangiography has introduced the ability to diagnose and treat severe lymphatic disorders unresponsive to other therapies. All pediatric patients presenting for lymphatic procedures require general anesthesia presenting challenges in managing highly morbid and comorbid conditions both from logistical as well as medical aspects. General anesthesia is used because of the procedural requirement for immobility to accurately place needles and catheters, treat pain secondary to

Pediatric Acute Flaccid Paralysis: Enterovirus D68-Associated Anterior Myelitis.

BACKGROUND: Enteroviral infections can cause acute flaccid paralysis secondary to anterior myelitis. Magnetic resonance imaging (MRI) is important in the diagnosis of this potentially devastating pediatric disease. Before the 2014 outbreak of Enterovirus D68 (EV-D68), the virus was considered a relatively benign disease. CASE REPORT: A fully immunized 8-year-old boy was brought to the emergency department complaining of a cough, headache, neck pain, and right arm pain and weakness. Deep tendon reflexes in the weak arm could not be elicited. MRI of the brain and cervical spine revealed anterio

Contractile abnormalities of mouse muscles expressing hyperkalemic periodic paralysis mutant NaV1.4 channels do not correlate with Na+ influx or channel content.

Hyperkalemic periodic paralysis (HyperKPP) is characterized by myotonic discharges that occur between episodic attacks of paralysis. Individuals with HyperKPP rarely suffer respiratory distress even though diaphragm muscle expresses the same defective Na(+) channel isoform (NaV1.4) that causes symptoms in limb muscles. We tested the hypothesis that the extent of the HyperKPP phenotype (low force generation and shift toward oxidative type I and IIA fibers) in muscle is a function of 1) the NaV1.4 channel content and 2) the Na(+) influx through the defective channels [i.e., the tetrodotoxin (TTX

Medicinal plants and formulations used by the Soren clan of the Santal tribe in Rajshahi district, Bangladesh for treatment of various ailments.

The Santals form the largest tribal community in northern Bangladesh reside primarily in Rajshahi and Rangpur Divisions, where they live in the districts of Rajshahi, Rangpur, Thakurgaon, Dinajpur, and Panchagarh. Although they are fast losing their traditional medicinal practices, they still have their own medicinal practitioners who rely mostly on medicinal plants for treatment of a variety of ailments. The traditional medicinal practices vary quite extensively between the twelve clans of the Santals. The objective of the present study was to conduct an ethnomedicinal survey amongst the Sore

Type I thyroplasty revision 1 year after a window was mistakenly created on the cricoid cartilage.

Unilateral vocal fold paralysis causes glottic incompetence and can result in significant morbidity. To prevent such morbidity, surgeons treat affected patients with vocal fold medialization techniques; type I thyroplasty medialization surgery is widely used for this purpose. In this procedure, a window is opened on the thyroid cartilage to allow for placement of a silicon prosthesis to medialize the vocal fold. A 38-year-old woman presented to our clinic for evaluation of hoarseness and a low-pitched voice, which we diagnosed as being caused by left vocal fold paralysis. Two years earlier, sh

Puffer fish poisoning in Bangladesh: clinical and toxicological results from large outbreaks in 2008.

Poisoning after eating puffer fish containing highly lethal tetrodotoxin (TTX) is widespread in Asia. In 2008, naïve inland populations in Bangladesh were exposed to cheap puffer fish sold on markets. In three outbreaks, 141 patients with history of puffer fish consumption were hospitalized. Symptoms of poisoning included perioral paraesthesia, tingling over the entire body, nausea and vomiting, dizziness, headache, abdominal pain and muscular paralysis of the limbs. Seventeen patients (12%) died from rapidly developing respiratory arrest. Blood and urine samples from 38 patients were analyzed

Supraclavicular regional anesthesia is a safer alternative in the immediate postoperative period after shoulder arthroscopy: A systematic review & meta-analysis.

INTRODUCTION: Interscalene nerve blocks (ISB) are commonly used for arthroscopic shoulder surgeries due to their efficacy but carry a risk of hemidiaphragmatic paralysis (HDP). Supraclavicular nerve blocks (SCB) have been proposed as an alternative, offering similar analgesic benefits with potentially lower respiratory complication rates. The purpose of this systematic review and meta-analysis was to compare ISB and SCB respiratory outcomes following arthroscopic shoulder surgery. METHODS: A systematic review was conducted per PRISMA guidelines, with articles from Medline, Embase, and the Coc

Active Cough Sputum Clearance Grading System: A Novel Predictor of Early Tracheostomy in Patients With Acute Traumatic Cervical Spinal Cord Injury.

BACKGROUND: Respiratory failure is the leading cause of death in patients with traumatic cervical spinal cord injury (TCSCI). Early tracheostomy can effectively reduce the risk of secondary respiratory failure in TCSCI patients. However, there is currently a lack of robust indicators to predict which patients will require early tracheostomy. Paralysis of respiratory muscles leads to weakened cough and impaired sputum clearance, which is a major cause of secondary respiratory failure. Whether cough and sputum clearance capacity can serve as a predictor for early tracheostomy remains unclear. M

Clinical characteristics of children infected with enterovirus D68 in an outpatient clinic and the association with bronchial asthma.

BACKGROUND: All reports of increases in severe respiratory disease associated with human enterovirus D68 (EV-D68) are from hospital settings. However, there are few reports describing clinical characteristics in less severely affected populations. METHODS: We conducted a retrospective observational study from January 2010 to December 2015 in Yamagata, Japan. Using regional passive surveillance, 5794 respiratory specimens were collected from children who initially presented to an outpatient clinic with acute respiratory symptoms. The collected samples were tested for EV-D68 by reverse transcri

Effects of ischemic phrenic nerve root ganglion injury on respiratory disturbances in subarachnoid hemorrhage: an experimental study.

INTRODUCTION: Phrenic nerves have important roles on the management of respiration rhythm. Diaphragm paralysis is possible in phrenic nerve roots ischemia in subarachnoid hemorrhage (SAH). We examined whether there is a relationship between phrenic nerve root ischemia and respiratory disturbances in SAH. MATERIAL AND METHODS: This study was conducted on 5 healthy control and 14 rabbits with experimentally induced SAH by injecting autologous blood into their cisterna magna. Animals were followed up via monitors for detecting the heart and respiration rhythms for 20 days and then decapitaed by
