Prevention of self-mutilation in patients with Lesch-Nyhan syndrome: review of literature.

There have been no standard methods established to prevent the self-mutilation seen in LNS. Preventive treatment must be developed, therefore, for each patient based on clinical findings. The first method to be utilized should be a nonsurgical splint or mouth guard. If it proves to be unsuccessful then the benefits of surgical procedures should be considered. In the future, the use of medication, such as carbamazepine, may prove to restructure the current approach advocated by the literature today.

Estrogen suppresses gastric motility response to thyrotropin-releasing hormone and stress in awake rats.

Symptoms associated with gastric motility alteration vary with stress and ovarian hormone status, most notably in women with irritable bowel syndrome. This study examines combined effects, comparing gastric motility during administration of a stress-related neuropeptide thyrotropin-releasing hormone (TRH) and restraint stress in conscious rats of varied ovarian hormone status. Adult rats were ovariectomized and implanted with estrogen, progesterone, or vehicle-releasing pellets. After 21 days, intracerebroventricular (i.c.) cannula and gastric tension transducer were implanted. After 25-27 day

Prenatal stress induces high anxiety and postnatal handling induces low anxiety in adult offspring: correlation with stress-induced corticosterone secretion.

It is well known that the hypothalamo-pituitary-adrenal (HPA) axis is altered by early environmental experiences, particularly in the perinatal period. This may be one mechanism by which the environment changes the physiology of the animal such that individual differences in adult adaptative capabilities, such as behavioral reactivity and memory performance, are observable. To determine the origin of these behavioral individual differences, we have investigated whether the long-term influence of prenatal and postnatal experiences on emotional and cognitive behaviors in adult rats are correlate

Catecholaminergic responses to rotational stress in rat brain stem: implications for amphetamine therapy of motion sickness.

The fact that amphetamine, a noradrenaline releaser, prevents motion sickness leads the hypothesis of Wood and Graybiel that the noradrenergic neuron system in the brain stem acts against the development of motion sickness. To evaluate the hypothesis, the effects of rotational stress on the turnovers of noradrenaline and dopamine in the rat brain stem were examined. Rats were rotated about two axes simultaneously (double rotational) or about one axis (single rotation) for 60 min. Measurement of kaolin intake (pica) induced by rotation, as an index of motion sickness, showed that double rotatio

Involvement of D1- and D2-like dopamine receptors in the hippocampal CA1 region in mediating the restraint stress-induced analgesia in the rats.

Acute stress exposure can elicit analgesic responses in multiple pain models. Research indicates that dopaminergic pathways in the hippocampus contribute to the analgesic responses elicited by forced swim stress, considered a combination of physical and psychological stress. Since psychological and physical stresses can trigger different brain pathways and areas, this study has examined how dopaminergic receptors in the CA1 region of the hippocampus affect the pain relief induced by restraint stress (RS), generally considered a psychological stress, in an acute pain model. [Results of and issues with a nation-wide survey of an additional system for patients with suspected or diagnosed dementia during hospitalization for physical illness].

AIM: The dementia support team (DST) is an initiative established based on the knowledge that, when individuals with dementia or older people at risk for delirium are hospitalized because of physical illnesses, there are often cases in which their cognitive symptoms rapidly deteriorate. However, the activities of each hospital are unknown. Therefore, a nation-wide survey of DSTs was conducted. METHODS: A questionnaire survey was conducted among 1,032 hospitals throughout Japan. RESULTS: A total of 422 responses were obtained, of which 292 were valid. The main results were as follows: number

Current State of Analgesia and Sedation in the Pediatric Intensive Care Unit.

Critically ill pediatric patients often require complex medical procedures as well as invasive testing and monitoring which tend to be painful and anxiety-provoking, necessitating the provision of analgesia and sedation to reduce stress response. Achieving the optimal combination of adequate analgesia and appropriate sedation can be quite challenging in a patient population with a wide spectrum of ages, sizes, and developmental stages. The added complexities of critical illness in the pediatric population such as evolving pathophysiology, impaired organ function, as well as altered pharmacodyn

All Unhappy Childhoods Are Unhappy in Their Own Way-Differential Impact of Dimensions of Adverse Childhood Experiences on Adult Mental Health and Health Behavior.

Adverse childhood experiences have consistently been linked with poor mental and somatic health in adulthood. However, due to methodological restraints of the main lines of research using cumulative or selective models, little is known about the differential impact of different dimensions of adverse childhood experiences. Therefore, we gathered data from 396 psychiatric in-patients on the Adverse Childhood Experiences (ACE) questionnaire, extracted dimensions using factor analysis and compared this dimensional model of adverse childhood experiences to cumulative and selective models. Household

[German practice of involuntary commitment at both federal and state level after introduction of the Guardianship law (1992-2009)].

BACKGROUND: Given the steady rise of psychiatric coercive measures in Germany, the question arises whether this development is significantly influenced by the corresponding legal basis or through epidemiological, socio-economic or socio-structural factors. METHODS: Based on full surveys of the Federal Ministry of Justice we examined the development and associations of 10 indicators of coercive psychiatric measures over a period of 18 years. Time trends of all indicators have been descriptively analysed. Statistical associations between time trends and between involuntary and admissions econom

Dimensions of care for dementia sufferers in long-term care institutions: are they related to outcomes?

OBJECTIVES: This study empirically examined whether dimensions of care cluster in special care units (SCUs) compared with non-SCUs. The relationship between SCU status plus separate measures of the dimensions of care and outcomes for dementia sufferers was then investigated. METHODS: Data were drawn from the Intermediate Care Facility Project. The sample (N = 510) included residents with dementia, aged 65 and older, in intermediate care facilities throughout the province of British Columbia. Canada. Longitudinal data included 6 outcomes: cognitive function, behavioral problems of agitation an

Health-Related Quality of Life, Everyday Executive Functioning, and Eating Behavior in Adults With Bardet-Biedl Syndrome.

Bardet-Biedl syndrome (BBS) is a rare genetic condition with a broad phenotypic spectrum. Knowledge about quality of life, executive functioning, and eating behavior in adults with BBS remains limited. This study aimed to assess health-related quality of life (HRQoL), everyday executive functioning, and eating behavior in adults with BBS and examine associations among these domains. Thirty adults with BBS (50% male, aged 20-69 years) participated in a cross-sectional study. HRQoL was measured using the Short Form Health Survey (SF-36), everyday executive functioning with the Behavior Rating In

Mistreatment of women during childbirth in Abuja, Nigeria: a qualitative study on perceptions and experiences of women and healthcare providers.

BACKGROUND: Global efforts have increased facility-based childbirth, but substantial barriers remain in some settings. In Nigeria, women report that poor provider attitudes influence their use of maternal health services. Evidence also suggests that women in Nigeria may experience mistreatment during childbirth; however, there is limited understanding of how and why mistreatment this occurs. This study uses qualitative methods to explore women and providers' experiences and perceptions of mistreatment during childbirth in two health facilities and catchment areas in Abuja, Nigeria. METHODS: I

Acute Agitation as an Initial Manifestation of Neuro-Behçet's Disease.

Managing acutely agitated or violent patients in the emergency department (ED) represents a significant challenge. Acute agitation as an initial manifestation of neuro-Behcet's disease (NBD) is an extremely rare clinical entity. A 44-year-old male, who had been complaining about a severe headache and fever for several days, was admitted to our ED due to acutely presented incontinence and agitation. On admission, physical restraint and sedation with sevoflurane and propofol were required for his combative and violent behavior. Cerebrospinal fluid examination revealed increased cell count. Fluid

The impact of domestication on fearfulness: a comparison of tonic immobility reactions in wild and domesticated finches.

We examined differences in the fear response between Bengalese finches and their wild ancestor, the white-backed munia, to explore the evolutionary mechanisms of behavioural changes due to domestication. The Bengalese finch (Lonchura striata var. domestica) was domesticated from the wild-living white-backed munia (L. striata) approximately 250 years ago. A previous study indicated that Bengalese finches sing much more complex songs than white-backed munias. We hypothesised that Bengalese finches are likely able to allocate more resources to reproduction in exchange for reduced survival effort.

Comparison of lumbar range of movement and lumbar lordosis in back pain patients and matched controls.

Inconclusive findings have been shown in previous studies comparing lumbar range of movement (LROM) and lumbar lordosis between back pain patients and healthy subjects. In these studies, confounding variables such as age, gender, height, obesity, and pain level were usually not well controlled. The present study aimed to compare LROM and lumbar lordosis between back pain patients and matched controls. Fifteen male back pain patients and 15 age-, height-, obesity-, and physical activity-matched male controls were investigated. To minimize the effect of pain on the measurements, only patients wi

[Life threatening embolism caused by central venous catheter fragments in psychiatric patients].

Central venous catheters are sometimes the cause of life-threatening complications. In two patients with underlying psychiatric disorders we observed an embolism as a result of catheter fragments. The first patient was a 30-year-old woman with a borderline personality disorder and several previous episodes of self-mutilation, psychogenic seizures and disturbances of consciousness. She cut her central venous line positioned in the external jugular vein when she was unattended. The intravasal fragment dislocated into the right ventricle and had to be removed by a forceps used for myocardial biop

Melanin as a Multifunctional Molecular System: Biosynthesis, Genetics, and Functional Roles in Livestock Species.

Melanin is the primary pigment found in the skin, hair, wool and feathers of many vertebrates and plays a crucial role in determining the physical appearance and physiological performance of livestock species. Melanin functions in thermoregulation as well as coloration, photoprotection, and adaptation to diverse environmental conditions. This review examines the biosynthesis of melanin and the genetics of melanogenesis in major livestock species such as cattle, sheep, goats, pigs, horses, and poultry, and the roles of melanin-based pigmentation within different systems of animal production. Strangulating left colon volvulus following nonsurgical castration in a 6-year-old donkey.

Colic is a clinical syndrome and has been defined as a visceral abdominal pain and/or acute abdominal disease. It is a common cause of morbidity and mortality in horses. The most common forms of colics are related to gastrointestinal tract in nature and most often linked to colonic disturbances. However, colics are not well understood in donkeys compared to those of in horses and the literature is poor regarding bowel strangulating obstruction in donkeys. This report described the clinical signs and post-mortem necropsy findings of an abdominal colic due to the left colon volvulus following a

Prior Out-of-Home Placement and Length of Stay Among Youths Receiving Mental Health Services in the ED.

IMPORTANCE: Youth mental health crises have been increasing over the last decade, and there is an urgent need for clinicians to be more knowledgeable about patients with high emergency department (ED) utilization. Several disparities in ED utilization and outcomes have already been identified; however, little data exist on disparities affecting youths with histories of out-of-home placement (OOHP). OBJECTIVE: To explore whether history of OOHP is associated with increased length of stay among child and adolescent patients who present to the emergency department with psychiatric symptoms. DES
