[Examination of the acute central retinal artery occlusion (CRAO) by means of optical coherence tomography (OCT3)].

PURPOSE: Ischemic edema of the retina develops after CRAO and a regressive phase follows usually without any possibility of objectification. The aim of the study is to determine dynamics of edematous changes in the central retina using the OCT (Optical Coherence Tomography). SETTING: Department of Ophthalmology, Regional Hospital in Pardubice, Czech Republic, E.U. METHODS: During the period between June 2004 and January 2005, ten patients with the diagnosis of CRAO were examined by means of Stratus OCT3 (Zeiss). A protocol designed for analysis of the thickness and volume of the macula (Fast

Central retinal artery occlusion following transcatheter balloon aortic valvuloplasty in an adolescent with aortic valvular stenosis.

A 12-year-old girl with aortic valvular stenosis underwent transcatheter balloon aortic valvuloplasty (BAV) using a femoral artery approach. Anticoagulation with heparin during the procedure was used. The patient noted sudden onset of concentric constriction of the visual field in the right eye 40 min after BAV. Brain magnetic resonance imaging and cervical ultrasound revealed no abnormality. Funduscopic examination showed white swelling around the macular region, indicating ischemia, consistent with central retinal artery occlusion (CRAO). CRAO should be recognized as a rare and serious compl…

Cerebro-retinal ischemia after bilateral occlusion of internal carotid artery. A study with prospective follow-up.

Seven patients with occlusion of internal carotid arteries (ICAs) were prospectively followed during a mean period of 14 months. Prior to demonstration of occlusions, four patients suffered a mild stroke, and three isolated transient ischemic attacks (TIAs) or amaurosis fugax. All patients remained alive and with an unchanged functional ability. During follow-up, one patient suffered amaurosis fugax and TIAs followed by a mild stroke, three suffered isolated TIAs or amaurosis fugax, two suffered reversible cerebro-retinal ischemia of more than 24 hours, and one remained symptom free. In three

Correction of arterial input-function saturation for extraction fraction estimation in retinal fluorescein videoangiography.

OBJECTIVE: Quantitative retinal fluorescein videoangiography (FVA) can estimate extraction fraction (E), but the 8-bit intensity resolution of clinical systems creates a dynamic-range tradeoff: preserving arterial input function (AIF) linearity can restrict tissue signal, particularly for pixel-level analysis. We developed a single-bolus correction method to broaden the usable acquisition range while maintaining E estimation accuracy. APPROACH: The method combined truncated-mean reconstruction when sufficient unsaturated arterial pixels remained with gamma-variate interpolation when saturatio

Macular sub-ILM hemorrhage as a favorable prognostic factor in ruptured retinal arterial macroaneurysm.

PURPOSE: To examine the relationship between macular sub-internal limiting membrane (sub-ILM) hemorrhage, foveal subretinal hemorrhage (SRH), and final visual acuity in ruptured retinal arterial macroaneurysm (RAM). METHODS: This retrospective study included 32 consecutive eyes with ruptured RAM. SRH beneath the ruptured RAM caused visual loss requiring treatment. Patients underwent pars plana vitrectomy (PPV) with sulfur hexafluoride (SF₆), PPV alone, or SF₆ alone. Foveal SRH and macular sub-ILM hemorrhage were recorded. Multiple linear regression evaluated associations of foveal SRH and mac

Retrobulbar Hemodynamic Alterations in Diabetes Mellitus: Doppler Evaluation of the Ophthalmic and Central Retinal Arteries.

BACKGROUND: Diabetes mellitus causes microvascular alterations that contribute to the development of diabetic retinopathy. Hemodynamic changes in retrobulbar circulation may occur before clinically detectable retinal damage. PURPOSE: This study aimed to compare Doppler hemodynamic indices of the ophthalmic artery (OA) and central retinal artery (CRA) between diabetic participants and non-diabetic controls. MATERIALS AND METHODS: This cross-sectional observational study included 76 participants (38 diabetic participants and 38 non-diabetic controls), comprising 152 eyes analyzed at the eye le…

Clinical Guidelines for the Diagnosis and Management of Retinal Artery Occlusions by the Spanish Society of Retina and Vitreous.

Retinal artery occlusions (RAOs) constitute a rare but serious ophthalmological emergency with a poor prognosis for visual recovery, for which there is currently no adequate therapeutic strategy. However, there is substantial evidence highlighting the importance of systemic (non-ophthalmological) management of patients with RAO. Since RAOs are the ocular analogous of a cerebral stroke, they require a similar urgent, multidisciplinary approach involving laboratory tests and imaging studies that go beyond the typical scope of ophthalmologists. This calls for cooperation and understanding from al…

Coronary artery and retinal vascularization by optical coherence tomography angiography: are eyes the window to the heart?

PURPOSE: Coronary artery disease (CAD) is the leading cause of death in the United States and its assessment still relies on invasive diagnostic procedures requiring contrast, such as coronary angiography (CAG) or computed tomography angiography (CCTA). The retinal vasculature is the only microvascular site in the human body that can be assessed non-invasively, and it has been described as a promising method for predicting cardiovascular risk since the classification of hypertensive retinopathy in the 19th century. Unfortunately, most classifications still rely on qualitative findings, which e

Membrane Permeability Is Required for the Vasodilatory Effect of Carbonic Anhydrase Inhibitors in Porcine Retinal Arteries.

It has been demonstrated previously that a variety of carbonic anhydrase inhibitors (CAIs) can induce vasodilation in pre-contracted retinal arteriolar segments although with different efficacy and potency. Since the CAIs tested so far are able to permeate cell membranes and inhibit both intracellular and extracellular isoforms of the enzyme, it is not clear whether extra- or intracellular isoforms or mechanisms are mediating their vasodilatory effects. By means of small wire myography, we have tested the effects of four new CAIs on wall tension in pre-contracted retinal arteriolar segments th

Early vascular damage in retinal microcirculation in arterial hypertension: the Czech post-MONICA study.

UNLABELLED: Retinal microcirculation reflects retinal perfusion abnormalities and retinal arterial structural changes at relatively early stages of various cardiovascular diseases. Wall-to-lumen ratio (WLR) may represent the earliest step in hypertension-mediated organ damage. Our objective was to compare functional and structural parameters of retinal microcirculation in a randomly selected urban population sample, in hypertensive and normotensive individuals. DESIGN AND METHOD: A total of 398 randomly selected individuals from an urban population aged 25-65 years, residing in Pilsen, Czech R

Intravenous Thrombolysis for Central Retinal Artery Occlusion: A Look at the Literature for the Emergency Medicine Physician.

Central retinal artery occlusion (CRAO) is a subtype of ischemic stroke and true ocular emergency presenting with acute, painless, monocular vision loss. Typical findings include poor visual acuity (VA), impaired color vision, relative afferent pupillary defect, and on fundoscopic evaluation, retinal edema, cherry red spot, and occasionally visualization of retinal artery emboli. While there are no proven treatments for CRAO, options include orbital massage, hyperbaric oxygen therapy, and intra-arterial or intravenous thrombolysis (IVT). This study reviews the current literature on the efficac…

Retinal arterial-venous pulse delay: a new specific marker for a carotid-cavernous fistula.

PURPOSE: The purpose of the study was to describe ocular blood flow changes in eyes affected by a carotid-cavernous fistula (CCF) using laser speckle flowgraphy. We hypothesized that imaging blood flow velocity waveforms in the retinal arterioles and venules simultaneously would reveal specific characteristics of an arteriovenous (AV) connection. DESIGN: The study was an observational case series, with a retrospective case-control analysis. METHODS: Five patients with a CCF underwent measurement of ocular blood flow using laser speckle flowgraphy. The blood flow was compared retrospectively

To treat or not to treat: a clinical series of retinal arterial macroaneurysms: A single-center retrospective study.

Retinal arterial macroaneurysms (RAMs) develop as outpouchings of the arterial wall that is weakened by arteriosclerosis. The traditional treatment of RAMs comprises observation, focal laser photocoagulation, or surgery. Recently, intravitreal injection of anti-vascular endothelial growth factor (VEGF) drugs has been announced as an effective therapy for fovea-threatening RAMs and quickly improve visual acuity and central retinal thickness (CRT).In the retrospective series, medical charts and ocular images of 24 patients diagnosed as having RAM between May 2011 and November 2018 in our facilit
