CNS imaging findings associated with Parry-Romberg syndrome and en coup de sabre: correlation to dermatologic and neurologic abnormalities.

INTRODUCTION: Parry-Romberg syndrome (PRS) and en coup de sabre (ECS) are variants of morphea. Although numerous findings on central nervous system (CNS) imaging of PRS and ECS have been reported, the spectrum and frequency of CNS imaging findings and relation to cutaneous and neurologic abnormalities have not been fully characterized. METHODS: We retrospectively reviewed patients younger than 50 years at our institution over a 16-year interval who had clinical diagnosis of PRS and ECS by a skin or facial subspecialist. Two neuroradiologists evaluated available imaging and characterized CNS i

Linear scleroderma in an adolescent woman treated with methotrexate and excimer laser.

A 17-year-old Caucasian woman presented for evaluation and treatment of a tender expanding linear plaque on her left flank. Biopsy findings were consistent with morphea. Treatment initially included intralesional steroid injections and topical calcipotriene ointment, followed by methotrexate and excimer laser. The lesion decreased in size considerably with relief of symptomatic discomfort by 7 months. An excisional biopsy of a persistent eroded papule on the superior aspect of the morphea plaque revealed dermal thickening and sclerosis with superimposed perforation of a calcified nodule. Local

Connective tissue panniculitis: lupus panniculitis, dermatomyositis, morphea/scleroderma.

Panniculitis is an uncommon cutaneous manifestation of connective tissue diseases. Our discussion will include panniculitis occurring in the setting of lupus erythematosus, dermatomyositis, and scleroderma/morphea. These subtypes of panniculitis are unified by an active inflammatory stage of the disease that can progress to develop scarring, atrophy, and calcifications. Treatment is most effective if initiated during the active phase of the disease and often requires systemic therapy because of the location of the inflammation. Antimalarials are the initial treatment of choice for most cases o

Autosomal recessive plasma cell panniculitis with morphea-like clinical manifestation.

Two siblings had hyperpigmented indurations over the inner aspects of both thighs extending to the lower abdomen. Skin biopsy showed plasma cell panniculitis favoring a diagnosis of morphea profundus. Family history of consanguinity was present, but both parents were unaffected. To our knowledge this is the first report of autosomal recessive plasma cell panniculitis with the clinical manifestations of morphea.

Linear scleroderma: a Neuman nursing perspective.

Although quite a rare pediatric disease, linear scleroderma (LS), a chronic disease, may leave scarring and may have devastating effects on a child and family. This article provides an overview of the disease, presents a case history of a boy with LS, and discusses how nurses and other health-team members assisted the child and family in managing the illness. The description of the nursing care is based on the Neuman systems model and on the nursing diagnoses of fear, disturbed self-concept, and impaired physical mobility.

Carpal tunnel syndrome presenting as sclerodactylia, nail dystrophy and acro-osteolysis in a 60-year-old woman.

Skin changes associated with carpal tunnel syndrome have rarely been reported. A 60-year-old patient was referred to our Department of Dermatology for evaluation of an aseptic gangrene of the distal phalanx of the third finger of her left hand, progressive nail dystrophy, sclerodactylia and spontaneous ulcerations. These features were seen on the digits with sensory supply from the median nerve. The patient had hand paresthesias for at least 10 years. No abnormal results were found in biochemical, hematological or immunological determinations. An echo-Doppler study of the upper limbs was norma

Neurologic abnormalities in two patients with facial hemiatrophy and sclerosis coexisting with morphea.

Progressive facial hemiatrophy or Parry-Romberg syndrome is a rare entity characterized by unilateral atrophy of the skin, subcutaneous tissue, and the underlying bony structures. This syndrome has many features of linear scleroderma en coup de sabre but is distinguished by more extensive involvement of the lower face and by only slight cutaneous sclerosis. We describe two unusual children with both atrophic and sclerotic changes of half of the face coexisting with multiple plaques of typical morphea. Both children developed neurologic disturbances with cranial magnetic resonance imaging (MRI)

[Infantile sclero-atrophic lichen and infection caused by Borrelia burgdorferi].

The authors describe a study of 8 girls suffering from genital and perianal lichen sclerosus et atrophicus. Following the most recent reports in literature, antibodies against Borrelia burgdorferi have been looked for, and the patients have been treated with propicillin. The results are discussed.

Severe subcutaneous calcification in the 'CREST' syndrome: evidence of high turnover of calcific deposits.

Four patients with mild CREST syndrome and extensive subcutaneous calcification have been studied over a 5-year period. There was no correlation between mineral deposition and other features of the disease and no obvious clinical or metabolic cause of the calcification. Mineral deposits, identified as crystalline carbonated apatite, were very labile. A clear pattern of radiographic evolution was apparent. Bone scans showed a higher uptake of isotope in new, developing deposits than in older lesions. Spontaneous formation and disappearance of adjacent deposits occurred in some patients. Loss of

[Anesthesiologic management in scleroderma patients. Presentation of a clinical case].

The scleroderma (literally, hard skin), or progressive systemic sclerosis (PSS), is a multisystem disease and can present various anaesthetic problems. The anaesthesist should be aware of the difficulty in opening mouth wide enough for laryngoscopy and intubation, the possibility that cardiopulmonary changes may be present and the probability of lesions in oesophagus, bowel, kidneys, skin and joints. In this article the authors describe the anesthetic management of a patient with Thibierge-Weissenbach syndrome (scleroderma with calcinosis cutis) and explore potential problems that should be an
