Gerbil bite anaphylaxis.

Household pets are well known to cause allergic symptoms in susceptible individuals, most commonly conjunctivitis, rhinitis, bronchospasm or urticaria. The increasing prevalence of exotic pets, including rodents, may introduce novel allergens into the household setting. We describe the case of a 16-year-old female who presented to the emergency department (ED) with an immediate systemic reaction consistent with anaphylaxis following a bite injury from a pet Mongolian gerbil. Although rare, gerbil bite injury represents another possible allergen source for precipitating a severe allergic reacti

[Bitten by an exotic venomous snake].

BACKGROUND: Patients who have been bitten by an exotic venomous snake are at risk of severe morbidity and a fatal outcome following an incorrect risk-assessment. Treatment with an antivenom can be necessary and can turn out to be lifesaving. In the Netherlands there are only a few cases of bites from exotic venomous snakes each year. CASE DESCRIPTION: A 28-year-old man presented at the emergency department (ED) after having been bitten by a monocled cobra (Naja kaouthia). Here he developed severe neurotoxicity with respiratory insufficiency requiring intubation and mechanical ventilation. The

Radiology of foreign bodies: how do we image them?

To assess the sensitivity of detecting the most commonly encountered foreign bodies in Emergency Radiology using all imaging modalities (conventional radiography, computed tomography, ultrasound, and magnetic resonance imaging). The following materials were inserted into a pig-leg phantom and imaged using conventional radiography, computed tomography, ultrasound, and magnetic resonance imaging: Plastics #1, 2, 3, 5, and compostable plastic; dry and wet wood, aluminum, gravel, glass (tinted and non-tinted), and Salmon and Halibut fish bones. The visibility of plastic is variable on both convent

Is vitamin D supplementation a new hope for the therapy of the septic shock?

Vitamin D is mainly known for its traditional role in the bone mineralization and calcium homeostasis. Recent studies have shown that vitamin D receptors (VDR) are present in almost all the tissues and cells in the human body. In addition, several studies have revealed that vitamin D is important in immunomodulation, regulation of inflammation and cytokines, cell proliferation, cell differentiation, apoptosis, angiogenesis, muscle strength, and muscle contraction. Patients with sepsis have high mortality rate and high deficiency in vitamin D. In addition, septic patients have decreased vitamin

Intestinal anisakiasis as a rare cause of small bowel obstruction.

Anisakiasis, a parasitic infection by larvae of the nematode Anisakis found in raw or undercooked saltwater fish, mostly involves stomach but rarely small intestine. We report a rare case of a 61-year-old man who presented with abdominal pain and developed small bowel obstruction caused by intestinal anisakiasis. Abdominal computed tomography revealed segmental edema of the intestinal wall with proximal dilatation. The patient underwent urgent laparotomy because strangulated small bowel obstruction was suspected. A localized portion of the intestine around jejunoileal junction was found to be

Human ECGs corrupted with real CPR artefacts in an animal model: generating a database to evaluate and refine algorithms for eliminating CPR artefacts.

AIM: For the analysis of ECG rhythms during ongoing CPR, single- or two-channel methods have been proposed to eliminate artefacts from the CPR-corrupted ECG. To refine, test and evaluate these algorithms with a realistic data set, we introduce an animal model with which we created an extended database of human ECGs with real CPR artefacts. MATERIAL AND METHODS: In a pig model real CPR-related artefacts were added to annotated human emergency ECGs. Via a special catheter placed in the oesophagus, ECG sequences (duration>10s) were fed in close to the dead pig's heart. The resulting surface pote

Commissioned article: management of exotic snakebites.

Exotic (foreign or non-native) snakes, including venomous species, are becoming increasingly popular pets in Western countries. Some of them are kept illegally (as defined by the UK Dangerous Wild Animals Act of 1976). There is a large international market for such animals, with contraventions of the Convention on International Trade in Endangered Species of Wild Fauna and Flora (CITES). In the UK, several other European countries and the USA the reported numbers of bites by venomous exotic snakes, although small, are increasing but still underestimate the occurrence of these occasionally fata

The role of case containment centers in the eradication of dracunculiasis in Togo and Ghana.

As part of the global effort to eradicate dracunculiasis (Guinea worm disease), several endemic countries established case containment centers to provide treatment and support to patients with emerging Guinea worms to keep them from contaminating water sources. To assess the functioning, effectiveness, and public perception of this intervention, we visited eight centers and conducted surveys in 32 villages in Togo and Ghana. In the areas served by these centers, incidence dropped by 71% in Togo and 42% in Ghana from 2003 to 2004. Among persons with emerging worms, admission to the centers was

Epidemiology of pediatric bite/sting injuries. One-year study of a pediatric emergency department in Israel.

Animal bite/sting injuries are a known source of morbidity with a significantly higher incidence among children who are most often bitten in the face, head, and neck. The objective of this study was to provide a better understanding of bite/sting injuries treated at the pediatric emergency department in order to guide preventive efforts. The sociodemographic, epidemiological, and clinical data on all bite/sting injuries treated in one representative pediatric emergency department in Israel over a 1-year period were retrieved and analyzed. Two hundred of the 9,309 pediatric trauma cases treated

Preoperative evaluation and management of the emergency surgical small animal patient.

The initial management and stabilization of the emergency surgical patient is challenging. A consistent systematic evaluation of the four major body systems, determination of the underlying cause, and aggressive stabilization before surgery can optimize the outcome.

Elimination of bancroftian filariasis (Wuchereria bancrofti) in Santa Catarina state, Brazil.

During the 1950s, three foci of Wuchereria bancrofti transmission were identified in the State of Santa Catarina, Brazil. In Florianópolis, São José da Ponta Grossa and Barra da Laguna community treatment of bancroftian filariasis with diethylcarbamazine (DEC) was performed using two distinct approaches, without vector control or improvements in sanitation. In two of the three communities only microfilaraemic individuals were treated, while in Barra da Laguna the entire population received DEC treatment regardless of their infection status. In both cases, transmission of the parasite was block

Statistical data on occupational contact urticaria.

Data on occupational contact urticaria (protein contact dermatitis included) in Finland during 1990-1994 were analyzed. Altogether 815 cases were reported to the Finnish Register of Occupational Diseases during this period, compared with 1944 cases of occupational allergic contact dermatitis. Accordingly, the total number of occupational allergic contact dermatoses was 2759, 29.5% being contact urticaria and 70.5% being allergic contact dermatitis. Occupational contact urticaria was much more common in women (70%) than in men (30%). The 6 most common causes of contact urticaria were (1) cow da

Comparing vaccination coverage and dog population demographics among four pilot dog rabies vaccination strategies in Uganda.

INTRODUCTION: The zero by 30 initiative aims to eliminate dog-mediated human rabies by 2030, for which dog vaccination is a crucial pillar. This study piloted four different dog vaccination campaign strategies in Kyegegwa, a rural district in Uganda, where rabies is endemic, and compared the vaccination coverages achieved by the strategies. METHODS: Four vaccination strategies were rolled out, each in three parishes from different sub-counties: (i) static point vaccination (SP), (ii) school-based (SB, i.e., information campaigns were mainly conducted at schools and vaccination was done at the

Efficacy of equine-assisted psychotherapy in veterans with posttraumatic stress disorder.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a common and debilitating disorder among war veterans. Although complementary and alternative therapies are gaining acceptance in the treatment of PTSD, the efficacy of animal-based therapies in this disorder is unknown. The goal of equine-assisted psychotherapy (EAP) is to improve the social, emotional, and/or cognitive functions of individuals with PTSD. OBJECTIVE: This study aims to explore the effects of EAP on PTSD symptoms. We hypothesized that veterans with PTSD who participate in a standardized EAP program for 1 h per week for 6 week

Metritis vaccination in Holstein dairy heifers using a herd-specific multivalent vaccine - Effects on uterine health and fertility in first lactation.

In cattle, acute puerperal metritis (APM) compromises animal welfare and has an economic impact to the farmer because of the detrimental effects on reproductive performance, milk production, and survivability. The aim of this randomized clinical study was to investigate the effects of a prepartum immunization using a herd-specific multivalent vaccine on incidence of APM, prevalence of clinical endometritis (CE), and selected measures of reproductive performance in primiparous Holstein cows. Pregnant nulliparous cows (230±4days of gestation) assigned to the vaccinated group (VG; n=142) received
