Radiographic and Advanced Imaging Evaluation of Posterior Shoulder Instability.

PURPOSE OF REVIEW: Posterior shoulder instability is an uncommon but important cause of shoulder dysfunction and pain which may occur as the result of seizure, high energy trauma, or repetitive stress related to occupational or sport-specific activities. This current review details the imaging approach to the patient with posterior shoulder instability and describes commonly associated soft tissue and bony pathologies identified by radiographs, CT, and MR imaging. RECENT FINDINGS: Advances in MR imaging technology and techniques allow for more accurate evaluation of bone and soft tissue patho

Intraarticular findings in the chronically painful shoulder. A study of 32 posttraumatic cases.

32 consecutive patients suffering from chronic shoulder pain for more than 6 months after a single, nondislocating shoulder trauma were examined clinically and by special radiographs, dynamic sonography, MRI and arthroscopy. Typical complaints were pain during loading, especially during over the head activities. Symptoms of a "dead arm" and instability were also present. Patients with previous dislocations, traumas or radiographic signs of degenerative shoulder lesions were excluded. The patients had a decreased active range of motion and positive signs of apprehension and impingement, but onl

Arthroscopic double-pulley remplissage technique for engaging Hill-Sachs lesions in anterior shoulder instability repairs.

We present a modified arthroscopic technique used to treat anterior shoulder instability associated with mild glenoid bone loss and a large Hill-Sachs lesion. The procedure aims to convert a bony intra-articular defect into an extra-articular defect by insetting the infraspinatus into the Hill-Sachs lesion. The arthroscopic procedure is performed with the patient in the lateral decubitus position, and the same portals used for anterior instability repair are used for this technique. The sequence of steps involves placing and passing the glenoid anchors and sutures and then waiting to tie the a

[Immobilization by external rotation after primary traumatic shoulder dislocation].

Immobilization of the shoulder following dislocation and reduction is traditionally carried out using internal rotation braces. However, high recurrence rates of up to 96% may result depending on several concomitant intrinsic and extrinsic prognostic factors, such as age and level of physical activity. Recently, different cadaveric and radiologic studies have suggested an improved tissue apposition of the affected labroligamentous structures in a better anatomic position during external rotation of the humeral head. Recent clinical outcome studies have supported the hypothetical benefit of thi

[Arthroscopic ventral shoulder stabilization].

Traumatic antero-inferior dislocation of the shoulder is a common injury among the young athletic population. The incidence of reluxation after conservative treatment in young patients is up to 80%. Arthroscopic antero-inferior shoulder stabilization has become the gold standard in recent years due to technical and material improvement. The recurrence rate after arthroscopic and open surgery is similar (approximately 6.5%), but morbidity after arthroscopic surgery is less. The authors of this paper advise the use of suture anchors for refixation of the capsulolabral complex. An antero-inferior

[Posterior shoulder joint instability. Classification, pathomechanism, diagnosis, conservative and surgical management].

The posterior instability of the shoulder is a more difficult diagnostic and therapeutic challenge than the anterior instability. There are many etiologies and causes of posterior instability. Most studies in the literature are retrospective and yield a great variation in therapeutic recommendations. Generally it has to be separated in traumatic and atraumatic instabilities. Most of the traumatic dislocations are impaction fractures of the humeral head against the dorsal glenoid. Therapy is depending on the size of the humeral defect, the duration of dislocation and the functional demand of th

Osteosynthesis using a Brodsky approach after scapular fractures: good clinical results after a mean follow-up of 6.3 years.

BACKGROUND: This study evaluated the long-term clinical and radiological outcomes of operatively treated scapular fractures using the tissue-sparing posterior Brodsky approach. We hypothesized that osteosynthesis using this approach would yield favorable functional outcomes. METHODS: This retrospective study included patients with acute scapular fractures treated operatively using the posterior Brodsky approach from January 2015 to December 2019. Radiologic evaluation included fracture classification and fracture union. Functional outcomes were assessed using the Constant-Murley Score (CMS),

Factors associated with pseudoparalysis in patients with extensive chronic and atraumatic rotator cuff injury.

BACKGROUND: Patients with massive chronic atraumatic rotator cuff injuries can be asymptomatic or present severe shoulder dysfunction, a condition known as pseudoparalysis. Our hypothesis is that integrity of the subscapularis, hypertrophy of the teres minor, and a complete tear of the long head of the biceps tendon are associated with improved active forward flexion range in such patients. Therefore, the objective of this study was to evaluate factors associated with pseudoparalysis. METHODS: This was a single-center cross-sectional study that included patients with massive chronic atraumati

SLAP lesion type IX: case series and a review of literature.

PURPOSE: A type IX SLAP (superior labrum anterior to posterior) lesion involves 360° of the glenohumeral labrum. Only rare reports have been published analyzing the risk factors of this lesion and the success of its arthroscopic management. The aim of our study is to evaluate predisposing factors that lead to SLAP IX and to assess the clinical outcome after arthroscopic treatment. Our treatment algorithm is also presented. METHODS: We report on a series of six patients treated in our institution between January 2014 and January 2019 who underwent shoulder arthroscopy and were intraoperatively

Review of the surgical anatomy of the axillary nerve and the anatomic basis of its iatrogenic and traumatic injury.

The axillary nerve is invariably reported to be one of the most commonly injured nerves during surgical procedures of the shoulder, and the importance of protecting it cannot be overemphasized. Many researchers have tried to identify safe regions, but the results vary among published studies. The axillary nerve may also be injured during acute trauma to the shoulder or by chronic repeated trauma as has been described in the quadrilateral space syndrome. The nerve injury may occur together with shoulder dislocation and rotator cuff tear, thus comprising the so-called "unhappy triad" of the shou

Arthroscopic bankart repair for unidirectional shoulder instability.

A successful arthroscopic Bankart repair for unidirectional shoulder instability requires careful patient selection and, to the extent possible, the restoration of normal anatomy. The patient's goals and anticipated demands are important considerations. A patient who participates in an overhead sport requires not only a stable shoulder but also a full range of shoulder motion. An athlete who engages in a contact or collision sport, however, may tolerate a mild loss of motion provided the shoulder is stable. Compared with an open procedure, an arthroscopic repair provides the opportunity to ret

A biomechanical study of fast throwing movements of the shoulder in baseball pitching.

OBJECTIVE: To define the biomechanic function of the shoulder in fast throwing movements in baseball pitching. METHODS: Using NAC high-speed video and PIAS LA-555 computer system, overhand pitching movements carried out by 12 healthy Japanese professional baseball pitchers were analyzed in three dimensions. The pitching motion was displayed as stick to sick figures dynamically. RESULTS: Pitching motion was a combination of translational and rotational movements. Acceleration and deceleration, being two typical motion patterns, worked sequentially through the anatomic link system from the piv

Arthroscopic evaluation of the unstable shoulder.

The arthroscopic evaluation of glenohumeral instability is based upon having a firm understanding of normal arthroscopic anatomy and knowledge of the structures responsible for the static and dynamic control of glenohumeral translation. Once this has been mastered, shoulder arthroscopy in combination with EUA can play an integral role in the management of shoulder instability. EUA's use in the dislocator is established in the decision of when to proceed with open or arthroscopic stabilization, and its role for prognostic purposes in this situation is less well-defined. For occult instability,

Current concepts in the rehabilitation of the athletic shoulder.

The rehabilitative process of the overhead athlete represents a significant challenge to the clinician. Overhead athletes (thrower, tennis player, swimmer) repetitively subject their shoulder joints to high microtraumatic stresses that, due to the accumulative effects, may lead to a variety of shoulder injuries. This type of athletic patient exhibits uniquely specific physical characteristics, such as hypermobility of the anterior shoulder capsule, excessive external rotation, hypomobility of the posterior capsule, limited internal rotation, and generalized ligamentous laxity of the glenohumer…

Lesions of the subscapular tendon regarding two cases in arthroscopic surgery.

Subscapularis tendon lesions in the shoulder are usually difficult to diagnose through clinical examination alone. Isolated subscapularis tendon lesions are infrequent, and most of these injuries are associated with biceps tendon pathology, rotator cuff tears, fractures to the tuberosities, or as a part of an instability complex of the shoulder. All of these other diagnoses over-shadow and complicate the diagnosis of the subscapularis tendon. Although magnetic resonance imaging is frequently effective in evaluating subscapularis tendon tears, arthroscopy is certainly more efficient and affords

[Radiographic signs of rotator cuff tears in the shoulder].

In 42 patients affected by disruption of the whole layer of the shoulder rotator collar the authors compared radiographic changes on a simple radiograph in anterior-posterior projection with operation findings. The proliferative changes on the large trochanter of humerus and the humeroacromial distance were decisive for prognosis of the operation therapy for the disrupted collar. The patients with a shorter humeroacromial distance were affected, after the operation, by a worse external rotation of the arm, worse extension and muscular strength of the arm in the extension position. Radiographic
