Management of Dysarthria in Amyotrophic Lateral Sclerosis.

Amyotrophic lateral sclerosis (ALS) stands as the leading neurodegenerative disorder affecting the motor system. One of the hallmarks of ALS, especially its bulbar form, is dysarthria, which significantly impairs the quality of life of ALS patients. This review provides a comprehensive overview of the current knowledge on the clinical manifestations, diagnostic differentiation, underlying mechanisms, diagnostic tools, and therapeutic strategies for the treatment of dysarthria in ALS. We update on the most promising digital speech biomarkers of ALS that are critical for early and differential d

Stronger accent following a stroke: the case of a trilingual with aphasia.

This study documents patterns of change in speech production in a multilingual with aphasia following a cerebrovascular accident (CVA). EC, a right-handed Hebrew-English-French trilingual man, had a left fronto-temporo-parietal CVA, after which he reported that his (native) Hebrew accent became stronger in his (second language) English. Recordings of his pre- and post-CVA speech permitted an investigation of changes in his accent. In sentence- and segment-listening tasks, native American English listeners (n = 13 and 15, respectively) judged EC's pre- and post-CVA speech. EC's speech was perce

[How to examine a patient with higher cerebral dysfunction].

The mysterious maneuvers of the neuropsychological examination actually conceal the very intellectual process by which diagnosis is obtained. Five features that are usually sufficient to diagnose aphasia are 1) spontaneous speech, 2) speech comprehension, 3) repetition, 4) reading aloud with comprehension, and 5) writing. In spontaneous speech, fluent aphasics have normal rhythm, melody, and well-articulated sentence, but the speech is filled with paraphasias. The speech of non-fluent aphasics is slow, labored, and poorly articulated. When patients with spatial neglect are asked to perform a v

A Case Study on Vocal Loudness With a Young Adult With Autism Spectrum Disorder and Developmental Delay.

PURPOSE: This clinical focus article describes an exploratory case study addressing reduction of vocal loudness in a young adult with a history of autism spectrum disorder and developmental delay. The need for a short-term pullout individual intervention arose from his participation in the Advancing Language and Literacy group, a program that provides support for enhancement of speech, language, and literacy skills for young adults with developmental disabilities. METHOD: The participant attended individual treatment sessions weekly for 9 sessions, all of which were digitally audio-recorded.

[A management for severe acquired stuttering in a case of pure akinesia syndrome].

We reported a 73-year-old man with pure akinesia syndrome who showed severe acquired stuttering and paradoxical kinesia on speech. He was evaluated in another hospital for bradykinesia and frozen gait at age of 67 when his cranial MRI disclosed ischemic changes in bilateral basal ganglia and periventricular deep white matter. The treatment with L-dopa and L-threo DOPS was not effective. His symptoms were slowly progressive and got worse gradually. At age of 72, he began to have difficulty in speech due to severe acquired stuttering, and one year later, he visited our hospital. The diagnosis of

Interarticulator coordination in children with and without cerebral palsy.

The current study investigates how interarticulator coordination changes across speaking tasks varying in articulatory and linguistic demands for children with CP and their typically-developing peers. Articulatory movements from 12 children with spastic CP (7M, 5F, 4-15 years of age) and 12 typically-developing age- and sex-matched peers were cross-correlated to determine the degree of spatial and temporal coupling between the upper lip and jaw, lower lip and jaw, and upper and lower lips. Spatial and temporal coupling were also correlated with intelligibility. Results indicated that children

Relationship between speech rate and speech disruption in cluttering.

PURPOSE: To correlate speech rate and speech disruptions in individuals who clutter, and to compare with individuals who do not clutter. METHODS: Participants were 14 individuals with ages between 8 and 40 years and 11 months, of both genders, divided into two groups paired by age and gender. GI was composed by seven individuals who clutter, and GII by seven individuals who do not clutter. The Speech Fluency Assessment Protocol was used to gather and analyze the speech samples, considering the frequency of speech disruptions and the speech rate. RESULTS: Data indicated that the greater the r

Pattern-based approaches to phonological therapy.

Pattern-based approaches to phonological disorders emerged in the 1970s and 1980s after researchers realized that phonological "rules" used to describe speech patterns of adults could be used in the analysis of children's speech. In this article, a pattern-based approach to assessment and treatment of phonological disorders is described, based on identification and treatment of phonological processes. Means of identifying and quantifying phonological processes are provided and assessment tests are described. Principles underlying the use of phonological processes in intervention programs are p

Selection of pediatric patients for use of the Passy-Muir valve for speech production.

A potential side effect of tracheotomy in the pediatric population is poor speech development. It has been well documented that children with tracheotomies have delays in expressive and receptive language out of proportion to the child's degree of intellectual functioning. While numerous methods of augmentative devices have been proposed, none are ideal for the child with a tracheotomy. Of the choices available, the Passy-Muir valve is best suited for use in the pediatric population. We present a method of selection of patients for Passy-Muir valve placement currently in use at Blythedale Chil

[Improved measurement of vocal penetration].

By measuring the vocal penetration a characteristic feature of the efficiency is covered which is essential for all three parts of the communication chain of speech with special emphasis on the recipient. Results are expressed in decibel (AS) of a defined masking noise, at which 50% of the logatomes spoken by the test persons are recognized. The vocal efficiency is indicated by the sound pressure of the masking noise, subtracting the sound level of speech used. Individuals with a healthy voice lie within the range of 12-24 dB (AS), average value 20 dB(AS), patients affected with a vocal disord

Speech rehabilitation in dysarthria.

Although the usefulness of speech rehabilitation in disorders such as asphasia is now recognized, many clinicians estimate that speech treatments for dysarthrics are of limited value. The conceptual approach to dysarthrias is a physiological approach that emphasizes the component-by-component analysis of the peripheral speech mechanism. The selection and sequencing of treatment procedures follow directly from the physiological nature and severity of involvement in each component.

Visual training and correction of articulation disorders by use of dynamic palatography: serial observation in a case of cleft palate.

The dynamic palatograph is an electrical apparatus that generates a visual display of constantly changing palatolingual contact as a function of time, using an artificial palatal plate with affixed electrodes. This paper describes a technique of speech therapy incorporating dynamic palatography for a cleft palate patient. The patient, a 6-year-old Japanese girl with a repaired unilateral cleft lip and palate, had been judged to demonstrate articulation disorders involving contact of the tongue with the hard palate or alveolus following surgical improvement of velopharyngeal function. Prior to

Predicting failure to speak after laryngectomy.

Postlaryngectomy speech rehabilitation involves interactive psychosocial and physiological considerations. Progress may be facilitated by early identification and timely management of counterproductive factors. A retrospective review of patient records was conducted to identify salient factors in the rehabilitative processes of these patients. A method for using these factors predictively is proposed. Application of this procedure may result in improved efficiency of treatment and management of clinical resources.

Stapedectomy versus stapedotomy: a comparison.

Two hundred and fifty consecutively operated stapes procedures operated by the author for otosclerosis are presented. A fat-wire stapedectomy prostheses (Schuknecht) was used in 152 cases and a Fisch teflon-wire piston was used in 98 ears. Of the 250 procedures 33 patients had bilateral surgery. The operations were done in local anaesthesia in most cases and with endomeatal incision. The fat-wire prostheses gave in 95% a closure within 10 dB and the rest 5% closed within 11-20 dB. The Fisch teflon-wire piston gave a closure within 10 dB in 87%, within 11-20 dB in 12% and within 21-30 dB in 1%.

ChatGPT for speech-impaired assistance.

BACKGROUND: Speech and language impairments, though often used interchangeably, are two very distinct types of challenges. A speech impairment may lead to impaired ability to produce speech sounds whilst communication may be affected due to lack of fluency or articulation of words. Consequently this may affect a person's ability to articulate may affect academic achievement, social development and progress in life. ChatGPT (Generative Pretrained Transformer) is an open access AI (Artificial Intelligence) tool developed by Open AI® based on Large language models (LLMs) with the ability to respo

An Imitation-Based Treatment for Ataxic Dysarthria: A Retrospective Multiple Single-Case Study.

Background/Objectives: Ataxic dysarthria is a speech disorder characterized by the impaired coordination of movement due to cerebellar dysfunction. Despite its clinical relevance, few studies have explored its rehabilitation. This study aimed to evaluate the applicability of IMITAF, an adaptive computer-based clinical treatment protocol originally developed to target aphasia with a novel population comprising individuals with ataxic dysarthria. The approach leverages principles of procedural motor learning. Methods: Ten patients with ataxic dysarthria due to neurodegenerative disease were retr
