The diagnosis, differentiation, and management of severe pancreaticoduodenal complications occurring after therapeutic endoscopic sphincterotomy and related procedures on the ampulla of Vater were reviewed for 254 patients. During the period 1982-1988, 8 patients were admitted with peripapillary carcinoma. Pancreatico-duodenectomy (PD) was successfully carried out on 7 occasions but only 3 patients are alive today. Early diagnosis of this disease is therefore mandatory. Endoscopic sphincterotomy is particularly useful for the diagnosis of the non-exposed type of carcinoma in the peripapillary region.

We present the case of a 50-year-old patient in whom an anastomotic biliary stricture after liver transplantation was treated endoscopically by sphincterotomy, dilatation and stenting using a plastic biliary stent. A distal migration of the stent caused a perforation of the rectum which was treated following stent extraction per anum -- conservatively with antibiotics and temporary bowel rest.

No controlled studies document either clinical or biochemical efficacy following balloon dilation of the sphincter of Oddi. Because of this and the unacceptable rates of pancreatitis and cholangitis associated with sphincter dilation when compared with the other available modalities of sphincterotomy, stent placement and surgery, this technology currently has little place when dealing with a spastic or stenotic sphincter mechanism.

Endoscopic sphincterotomy (EST) is a useful alternative to re-intervention of the bile ducts in cases of choledocholithiasis in the aged patient. The rate of success of EST is 87.7%, the rate of complications 8.4%, and the rate of mortality 1.1%. Mortality from EST is lower than that from transduodenal papillotomy or reoperation of the bile ducts. Late complications after EST are unknown and not to be expected.

Acute pancreatitis is a rare complication of hydatidosis, and only few reports were published previously.
