Acromioclavicular joint sprains: the post-injury recovery interval.

Acromioclavicular joint (ACJ) sprains occur after injuries to the shoulder girdle. Current practice is to treat such injuries conservatively. This study determines the recovery interval of ACJ sprains treated conservatively. Forty-seven patients with grades I or II ACJ sprains were evaluated in a shoulder clinic and treated conservatively. The average age was 37 years, with the injury being caused by a fall, contact sport or road-traffic accident. At an interval of 12 months or greater these patients were assessed by means of a questionnaire. Six months post-injury, pain was described as signi

Chronic subtalar instability due to insufficiency of the calcaneofibular ligament: a case report.

Calcaneofibular ligament insufficiency in isolation is an uncommon cause of chronic instability of the subtalar joint. We report one case of chronic subtalar instability due to calcaneofibular ligament insufficiency after an ankle sprain. It was diagnosed with clinical findings and stress radiograph, and successfully treated with proximal advancement of the elongated calcaneofibular ligament.

Exact moment of a gastrocnemius muscle strain captured on video.

A left gastrocnemius strain was sustained by an elite cricket batsman while he was taking off to run. The exact moment of injury, captured by a camera in the middle stump, appears to correspond to the sudden appearance of a deficit in the gastrocnemius muscle, seen through the player's trousers. The strain occurred when the entire body weight was on the left foot with the centre of mass well in front of the leg. The injury probably occurred close to the time when the gastrocnemius complex was moving from an eccentric to an isometric phase.

Acute myelogenous leukaemia presenting with mid-foot pain after an inversion injury.

The case is presented of a 5 year old boy who attended after an inversion injury. He had persistent pain despite treatment and was subsequently diagnosed with acute myelogenous leukaemia. It is well recognised that acute leukaemia commonly presents as non-traumatic limb pain. This case highlights the need to consider acute leukaemia in the differential diagnosis of any persisting bone or joint pain, even in the context of recent injury.

Peroneal tenosynovitis following ankle sprains.

Peroneal tenosynovitis has a variety of causes which include inversion injury of the ankle. It is rarely reported and frequently misdiagnosed. We report the operative findings and outcome of surgical decompression in 19 patients who clinically presented following inversion injury with features suggestive of peroneal tenosynovitis. A review of the literature of this pathology is presented.

Fracture of the hook of the hamate, often misdiagnosed as "wrist sprain".

A patient with fracture of the hook of the hamate is described, in whom the diagnosis was delayed for 1 year after the initial trauma. This delay in correct diagnosis is often seen, as routine anteroposterior and lateral roentgenograms of the wrist are unlikely to show the fracture. The trauma is often misinterpreted as a "wrist sprain." Nevertheless, with a careful trauma history and a detailed clinical examination and with minimal additional roentgenographic views, it should be possible to correctly diagnose the fracture. Relief of the disabling pain can then be offered by removal of the un-

Ankle injuries in the pediatric population.

Ankle injuries occur with considerable frequency in the pediatric population, although diagnosis is rarely straightforward. In this paper, we highlight the more common pitfalls in diagnosis and management, calling upon classic and contemporary literature and our own combined 30-year experience in managing pediatric lower extremity problems.

Strategy of exercise prescription using an unloading technique for functional rehabilitation of an athlete with an inversion ankle sprain.

STUDY DESIGN: Case study. OBJECTIVES: To demonstrate how an exercise program can be designed with specific sets, repetitions, and rest periods, and to enhance the healing process in early stages of rehabilitation when injured tissues cannot tolerate full body weight. Our goal was to enhance ankle tissue healing by reducing gravitational force through a prescriptive exercise and unloading program. BACKGROUND: This report describes a treatment method that we used to rehabilitate a collegiate soccer player with a Grade II inversion ankle sprain. This athlete sprained his ankle 6 weeks before th

Weight-bearing immobilization and early exercise treatment following a grade II lateral ankle sprain.

STUDY DESIGN: Case study. OBJECTIVES: To describe a protocol used in the rehabilitation of a grade II lateral ankle sprain, emphasizing brief immobilization with a removable boot, weight bearing as tolerated, and progression of early exercise. BACKGROUND: The optimum conservative treatment of severe grade II ankle sprains remains undefined. Short-term benefits of early mobilization have won favor over immobilization by casting; however, pain and ankle joint instability often linger. The timing of weight bearing as a variable that influences recovery has largely been ignored when either treat

[CBO-guideline for diagnosis and treatment of the acute ankle injury. National organization for quality assurance in hospitals].

Following an acute ankle inversion trauma, the aim of the diagnostic strategy is to rule out a fracture and/or lateral ankle ligament rupture. Plain X-rays are indicated when weightbearing is impossible (the patient cannot take at least four steps) and/or if there is pain on palpation on the posterior aspect of the lateral or medial malleolus. This applies to the period directly following the trauma as well as 4-5 days later. In the first forty-eight hours after trauma, physical examination cannot distinguish between a distortion and a lateral ligament rupture. In a patient with a painful swol

Low-level laser therapy in ankle sprains: a randomized clinical trial.

OBJECTIVE: To test the efficacy of low-level laser therapy on lateral ankle sprains as an addition to a standardized treatment regimen, a trial was conducted in which high-dose laser (5J/cm2), low-dose laser (0.5J/cm2), and placebo laser therapy (0J/cm2) at skin level were compared. DESIGN: Randomized, double-blind, controlled clinical trial with a follow-up of 1 year. Patients, therapists, assessors, and analysts were blinded to the assigned treatment. SETTING: An ambulatory care setting. PATIENTS: After informed consent and verification of exclusion criteria, 217 patients with acute later

Effect of cane use on tibial strain and strain rates.

The effect of cane ambulation on hip biomechanics has been well studied, but its effect on tibial strains and strain rates is not known. To test the hypothesis that cane use may lower tibial strain and strain rates during walking, percutaneous axial extensometers were mounted on the right medial cortex of the midtibial diaphysis in seven male volunteers. In vivo peak-to-peak axial tibial strains and strain rates were measured for ipsilateral and contralateral cane usage and compared with a no cane control. Cane-assisted ambulation was not found to significantly lower strain magnitudes; however

Strain magnitude related changes in whole bone architecture in growing rats.

Short daily periods of controlled dynamic loading were applied in vivo through the flexed carpus and olecranon to the intact ulna of 240 g male Sprague-Dawley rats. This technique involved neither surgical preparation, nor direct loading of the periosteum at a site close to the region of the bone in which adaptive modeling was subsequently assessed. The animals used their limbs normally between loading episodes, thus approximating to the natural situation, in which short periods of exercise are generally superimposed on longer periods of less strenuous activity. The strain patterns associated

Effects of vibration frequency and postural changes on human responses to seated whole-body vibration exposure.

The present investigation evaluated the effects of changes in pelvic orientation and vibration frequency on the seated human's response to wholebody vibration (WBV). Seat-to-trunk and seat-to-head acceleration transmissibility, peak-to-peak pelvic motion and erector spinae EMG and mean erector spinae EMG was collected across three pelvic orientations (9 degrees anterior pelvic tilt, neutral pelvis, and 9 degrees posterior pelvic tilt) and frequencies ranging from 4.5 to 16 Hz. Subjects included 30 healthy males between the ages of 18 and 35. Ensemble averages, two vibration cycles in length, w

Evaluation of kinesthetic deficits indicative of balance control in gymnasts with unilateral chronic ankle sprains.

If ankle proprioception can be determined to be impaired, then treatment can be more specifically directed toward correcting the proprioceptive deficit, thereby improving functional ability. The purpose of this study was to determine if collegiate level gymnasts with unilateral, multiple ankle sprains (ie., chronic ankle sprains) had decreased ability to detect passive plantar flexion of the ankle (ie., decreased ankle proprioception) and to determine if balance deficits existed during one-legged stance. Eleven gymnasts participated in 30 passive movement trials (15 movement and 15 nonmovement

Occupational disorders of the foot and ankle.

Occupational problems of the foot and ankle have received less attention than those of other body regions, namely the hand, neck, and lumbar spine. Functional requirements of the foot and ankle differ from other body regions, necessitating a different clinical approach to injuries than those indicated in the upper extremity or back. This article reviews the clinical characteristics of the foot and ankle pertinent to the worker and highlights the unique features of the management of foot and ankle problems encountered in occupational orthopedic practice.

Rock climbing injuries.

Rock climbing has become increasingly popular in the past decade. However, the increased participation exposes a greater number of climbers to potential injury. The risks involved with climbing increase in proportion to the skill-level of the climber: the higher the skill-level, the more hours are required for training and on more difficult routes. The hands are used as tools for the ascent, with much of the climber's weight placed upon the fingers and also distributed through the wrist, elbow and shoulders. The combination of repetitive climbing and the excessive weight-bearing demands of the

Arthroscopic lateral ankle stabilization.

The indications for arthroscopic stabilization include those patients with isolated ruptures or strains of the ATF. The procedure is particularly convenient for patients with ankle joint pathology that is already most commonly treated arthroscopically, such as chronic instability, in which inflamed synovium or meniscoid bodies are to be excised. Although significant stability can be gained with this technique, injuries that result in damage to the calcaneofibular ligament may require additional open surgery, since this ligament cannot be visualized arthroscopically. However, the anchoring tech

The enigmatic diagnosis of posterior tibialis tendon rupture.

Posterior tibialis tendon rupture is a diagnosis that is often missed. This is thought to be secondary to nonspecific clinical findings and the lack of any laboratory or radiographic test to reliably confirm the diagnosis. We report sixteen cases of surgically confirmed posterior tibialis tendon rupture. Based on our review of these patients, the diagnosis of posterior tibialis tendon rupture should be strongly suspected in the adult patient presenting with a history of a twisting ankle injury and generalized medial ankle pain and swelling. A flexible, asymmetric pes planus and forefoot pronat

Ankle sprain and postural sway in basketball players.

The present study compares postural ankle stability between previously injured basketball players, uninjured players and a control/group. Postural sway was recorded and analysed by stabilometry using a specially designed computer-assisted forceplate. Recordings were obtained for 60 s on each foot. The stabilometric results in the players with no previous injuries did not differ from those in the controls. Players with a previously injured ankle differed significantly from the control group. These players had a larger mean postural sway and used a larger sway area.
