Quantitative assessment of structural and functional changes in temporal lobe epilepsy with hippocampal sclerosis.

BACKGROUND: Magnetic resonance imaging (MRI) changes in hippocampal sclerosis (HS) could be subtle in a significant proportion of mesial temporal lobe epilepsy (mTLE) patients. In this study, we aimed to document the structural and functional changes in the hippocampus and amygdala seen in HS patients. METHODS: Quantitative features of the hippocampus and amygdala were extracted from structural MRI data in 66 mTLE patients and 28 controls. Structural covariance analysis was undertaken using volumetric data from the amygdala and hippocampus. Functional connectivity (FC) measured using resting …

Memory retrieval in temporal lobe epilepsy is related to functional segregation of the mesiotemporal structures.

OBJECTIVE: We analyzed the impact of temporal lobe epilepsy (TLE) with hippocampal sclerosis (HS) on functional connectivity (FC) between mesiotemporal structures. Functional connectivity modifications related to word retrieval were investigated. METHODS: High-density EEG of 21 patients with TLE with HS (12 left TLE and 9 right TLE) and 10 healthy controls (HCs) were recorded during a verbal subsequent memory paradigm. Electroencephalography data were reconstructed into the source space and FC was calculated from the source activity of regions of interest. RESULTS: A significant decrease in …

Multi-omic strategies applied to the study of pharmacoresistance in mesial temporal lobe epilepsy.

Mesial temporal lobe epilepsy (MTLE) is the most common type of focal epilepsy in adults, and hippocampal sclerosis (HS) is a frequent histopathological feature in patients with MTLE. Pharmacoresistance is present in at least one-third of patients with MTLE with HS (MTLE+HS). Several hypotheses have been proposed to explain the mechanisms of pharmacoresistance in epilepsy, including the effect of genetic and molecular factors. In recent years, the increased knowledge generated by high-throughput omic technologies has significantly improved the power of molecular genetic studies to discover new…

Induction of the Nrf2 Pathway by Sulforaphane Is Neuroprotective in a Rat Temporal Lobe Epilepsy Model.

Epilepsy is a chronic disease of the brain that affects over 65 million people worldwide. Acquired epilepsy is initiated by neurological ins … of a naturally occurring Nrf2 activator sulforaphane, in in vitro epileptiform activity model and a temporal lobe epilepsy rat model. Sulforaphane significantly decreased ROS generation during epileptiform activity, restored glutathione levels, and prevented seizure-like activity-induced neuronal cell death. When given to rats after 2 h of kainic acid-induced s …

Cortical complexity alterations in the medial temporal lobe are associated with Alzheimer's disease psychosis.

Psychosis is frequent in Alzheimer's disease (AD) and it is associated with a worse disease course. AD psychosis may represent a distinct AD phenotype, though its specific neurobiological underpinnings have yet to be identified. This study investigated neural underpinnings of AD psychosis using surface-based-morphometry.Data from 32 AD patients, 17 with psychosis (AD-P) and 15 without were analyzed. Average cortical complexity (fractal dimension, FD) was estimated for each theoretically motivated ROI and patient. First, we compared regional FD in AD-P and AD patients. Then we calculated the co

Diagnostic challenges in patients with temporal lobe seizures and features of autoimmune limbic encephalitis.

BACKGROUND AND PURPOSE: Consensus criteria for autoimmune limbic encephalitis (ALE) allow for a diagnosis even without neuronal antibodies (Abs), but it remains unclear which clinical features should prompt neuronal Ab screening in temporal lobe epilepsy patients. The aim of the study was to investigate whether patients with temporal lobe seizures associated with additional symptoms or signs of limbic involvement may harbor neuronal Abs, and which clinical features should prompt neuronal Ab screening in these patients. METHODS: We identified 47 patients from a tertiary epilepsy center with me…

A case report of sinoatrial arrest caused by temporal lobe epilepsy in subclinical glioblastoma.

BACKGROUND: Atrial fibrillation with symptomatic bradycardia, higher grade atrioventricular block, and sinus node disease are all common indications for permanent pacemaker implantation. The most frequent causes of sinus node disease treated with pacemaker implantation involve degenerative structural changes of the sinus node; less often, extrinsic causes (such as damage due to myocardial infarction or heightened parasympathetic nervous system activity) lead to pacemaker implantation. CASE PRESENTATION: A 50-year-old patient with syncope and documented sinoatrial arrest was referred. Neurolog

Association of Piriform Cortex Resection With Surgical Outcomes in Patients With Temporal Lobe Epilepsy.

IMPORTANCE: A functional area associated with the piriform cortex, termed area tempestas, has been implicated in animal studies as having a crucial role in modulating seizures, but similar evidence is limited in humans. OBJECTIVE: To assess whether removal of the piriform cortex is associated with postoperative seizure freedom in patients with temporal lobe epilepsy (TLE) as a proof-of-concept for the relevance of this area in human TLE. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used voxel-based morphometry and volumetry to assess differences in structural magnetic resonance imagi…

Whole network, temporal and parietal lobe contributions to the earliest phases of language production.

We investigated whether it is possible to study the network dynamics and the anatomical regions involved in the earliest moments of picture naming by using invasive electroencephalogram (EEG) traces to predict naming errors. Four right-handed participants with focal epilepsy explored with extensive stereotactic implant montages that recorded temporal, parietal and occipital regions -in two patients of both hemispheres-named a total of 228 black and white pictures in three different sessions recorded in different days. The subjects made errors that involved anomia and semantic dysphasia, which

Distinct domains of impulsivity are impaired in juvenile myoclonic epilepsy but not in temporal lobe epilepsy.

OBJECTIVE: The Barratt Impulsiveness Scale (BIS-11) is the most widely used questionnaire to study impulsivity in persons with psychiatric disorders, but it has rarely been applied to persons with epilepsy. The present study aimed to evaluate the usefulness of BIS-11 as a tool to explore impulsivity in two distinct epilepsy syndromes. METHOD: The BIS-11 was applied to 20 patients with juvenile myoclonic epilepsy (JME) (32.5±8.95years old), 20 patients with temporal lobe epilepsy (TLE) (37.7±13.25years old), and 26 healthy controls (31.86±11.25years old). Subclinical seizures as a pitfall in 18F-FDG PET imaging of temporal lobe epilepsy.

A 61-year-old man with history of heroin abuse, hepatitis B, hepatitis C, and hypertension was evaluated for seizures. MRI findings were concerning for temporal epilepsy. A brain 18F-FDG PET study showed a hypermetabolic focus in the left temporal lobe, although the patient was asymptomatic during the scan. Later review of electroencephalography recordings revealed a subclinical seizure during imaging. A whole-body 18F-FDG PET scan performed 4 days later for cancer screening purposes, during which the electroencephalography tracings were normal, showed no abnormal metabolic activity in the bra…

Prenatal ultrasound and MRI findings of temporal and occipital lobe dysplasia in a twin with achondroplasia.

Thanatophoric dysplasia, hypochondroplasia and achondroplasia are all caused by FGFR3 (fibroblast growth factor receptor 3) mutations. Neuropathological findings of temporal lobe dysplasia are found in thanatophoric dysplasia, and temporal and occipital lobe abnormalities have been described recently in brain imaging studies of children with hypochondroplasia. We describe twins discordant for achondroplasia, in one of whom the prenatal diagnosis was based on ultrasound and fetal MRI documentation of temporal and occipital lobe abnormalities characteristic of hypochondroplasia, in addition to t…
