Paralytic dislocation of the hip in cerebral palsy may be prevented by soft tissue surgery and the results of two procedures are reported. Twenty hips were treated by adductor tenotomy alone and this was unsuccessful in every case. An alternative method was carried out in 42 hips in 25 patients. The procedure combined adductor tenotomy with release of the hip muscles including iliac crest resection, tenotomy of the rectus femoris, recession of the iliopsoas and, when necessary, either release or transposition of the knee flexors. This was much more successful in decreasing migration of the fem

Author reports on experiences with the Froimson-Oh operation for rupture of the long head of the biceps brachii. The operation is thought to be advantageous. It is stressed in connection with 2 cases that the faulty choice of the place of the keyhole and its unadequate formation may predispose to humerus fracture and therefore this stage of the operation needs special care. The patient has to be warned, beside early movement, to adequate caution.

A 62-year-old man was seen for rupture of the flexor digitorum superficialis and profundus to the index finger. X-ray films showed longstanding Kienböck's disease with a completely fragmented lunate bone with fragments lying in front of the anterior lip of the radius. Tendon transfer restored active flexion to the index finger.

This report concerns the post-operative examination of 26 patients operated for 27 lunate bone necrosis. The following treatments were given: conservative (7), alloplastic (6), partial or total wrist arthrodesis (6), interposition tendon graft (5) and denervation of the wrist (1). The longest period of observation, an average of 13 4/12 years (maximum 21 years) was with the methods which have been grouped together under the heading conservative, after A. Beck (2), Konjetzny (2), Schneider (2), and Matti (1), the shortest period with the alloplastics and interposition tendon graft. Strict crite

The biomechanics of the Murphy procedure for correction of equinus in cerebral palsy are presented. A series of 79 surgical procedures on 48 patients, with a follow-up period of from one to four years, is reviewed. Correction of equinus was succeswful in 89-9 per cent of the procedures. Using the Murphy procedure the following advantages over other methods of correction are anticipated: (1) no loss on 'push-off' in gait; (2) no loss of correction (recurrence) during longitudinal growth of the child, necessitating repeated surgery; and (3) no need for continual night bracing during the growth p

Extensor indicis proprius (EIP) opponensplasty is one of the commonly used techniques to restore thumb abduction and opposition in patients with thenar muscle dysfunction from various causes of median nerve palsy. However, its subcutaneous route around the distal ulna may not represent a straight line of pull, and part of the extensor hood sometimes has to be harvested along with the EIP tendon to gain adequate length to reach the insertion. The purpose of the study is to present the alternative method of EIP opponensplasty and report the clinical outcomes. The modified EIP opponensplasty was

BACKGROUND: Use of the flexor hallucis longus (FHL) tendon is well described for several tendon augmentation procedures. Harvesting the FHL through an open medial approach is commonly done, but is associated with anatomic risks. Recently, several authors have described a minimally invasive (MI) technique to harvest the FHL tendon utilizing a hamstring tendon stripper commonly used in ACL reconstruction. The purpose of this study was to evaluate the safety and effectiveness of harvesting the FHL tendon using this novel minimally invasive retrograde approach.
METHODS: The FHL tendon was harvest

Camitz opponensplasty using the palmaris longus has been used in patients undergoing open carpal tunnel release. It is considered to have several advantages over other opponensplasty techniques, but it provides weak flexion and pronation, which are prerequisites for opposition. To address this shortcoming, we have used a modified Camitz procedure with a pulley at the radial side of the dissected flexor retinaculum and have assessed the results in comparison with the conventional Camitz procedure. Both procedures provided significant improvements in palmar abduction and Disabilities of the Arm,

Posterolateral instability (PLI) is common with posterior cruciate ligament tears and is less common with anterior cruciate ligament tears, and isolated PLI is rare. There are varying degrees of PLI with respect to pathologic external tibial rotation, and varus laxity. Surgical treatment of PLI must address all components of the PLI (popliteus tendon, popliteofibular ligament, lateral collateral ligament, and the lateral-posterolateral capsule), and other structural injuries. Successful posterior cruciate ligament and anterior cruciate ligament surgery depends upon recognition and treatment of

Advancements in the understanding of anatomy, kinematics, and physiology should improve future treatment of anterior cruciate ligament injured knees. The ultimate goal of full restoration of an anterior cruciate ligament injured knee to preinjury status, may be possible in the distant future through genetic manipulation inducing regeneration of tissues. In the midterm future, resorbable stents with incorporated bioactive growth factors have the potential of inducing normal anterior cruciate ligament anatomy without the need for detrimental harvesting of the patient's tissues, or risk of microb

The extensor indicis proprius (EIP) is regarded as one of the most consistent muscles of the dorsal forearm, originating from the distal thi … and localized dorsal hand pain. From a surgical standpoint, this variation is directly relevant to tendon transfer procedures, reconstructive hand surgery, and decompression of the fourth dorsal compartment, where failure to recognize such anatomy may lead to inadvertent tendon injury or suboptimal outcomes. This case expands the documented morphological spectr …
