We treated of 3 patients with descending necrotizing mediastinitis that is often to be fatal. There are 3 important issues regarding the treatment of this disease. A malignant tumor is generally believed to be very unlikely to metastasize to the thymus. Only three cases of thymic metastases have been reported so far in the medical literature. We report here a rare case of metastatic breast cancer to the thymus, which was detected by CT and PET scanning, and the metastasis was also confirmed by video-assisted thoracic surgery biopsy. Recognition of an unusual breast cancer metastasis, such as to the thymus, as well as the usual patterns of breast cancer metastasis will facilitate an accurate, prompt diagnosis and its appropriate treatment.

A 41-year-old man with myasthenia gravis was referred to our clinic to undergo a thymectomy by video-assisted thoracic surgery. After physical examination of the thorax and evaluation by computed tomography, a mass lesion was noticed in the neck and thymectomy was performed by partial sternotomy combined with a cervical incision. The cervical thymoma was discontinuous with the thymus. Thymoma in the neck has rarely been reported. In this case report we emphasize the possible presence of thymoma in the neck and the importance of neck evaluation in patients with myasthenia gravis.

We reported a case of a 20-year-old man with a giant leiomyoma of the esophagus resected under video-assisted thoracic surgery (VATS). The patient demonstrated an abnormal shadow on a chest x-ray and a posterior mediastinal tumor 11 cm in diameter on a computed tomogram (CT) and on magnetic resonance imaging (MRI). A leiomyoma or a neurogenic tumor of the esophagus was suspected, and VATS was performed. The resected tumor was pathologically confirmed to be a leiomyoma of the esophagus. A giant esophageal leiomyoma showing extraluminal outgrowth should be treated by VATS.

In the literature, thoracoscopic approach for pulmonary hydatid cyst is seen as small case groups, and multiple port incisions are performed in these studies. Thoracic hematogenic endometriosis is a rare pathology. A clinical course hasn't pathognomic symptoms, because of it, the diagnosis is established due to histological study. The article presented two cases of female patients, who were suffering from thoracic endometriosis. They were hospitalized to the department of thoracic surgery of Municipal multifield hospital No 2 in Saint-Petersburg. Morgagni hernia is a disease in which the abdominal contents herniate into the thoracic cavity through a congenital parasternal defect of the diaphragm resulting from an increased intra-abdominal pressure.
