Associated Injuries Are Common Among Patients With Bicycle-Related Craniofacial Fractures.

PURPOSE: Ample evidence exists on the relationship between bicycle injuries and craniofacial fractures. However, as the mechanism behind the … are relatively common in this specific patient population. Close co-operation in multidisciplinary trauma centers allowing comprehensive evaluation and treatment can be recommended for patients with bicycle-related craniofacial fracture.

Pediatric trauma telemedicine in a rural state: Lessons learned from a 1-year experience.

BACKGROUND: Previous research from our center has shown that 27% of the pediatric trauma transfers from referring facilities are potentially preventable. Our hospital is the only level 1 pediatric trauma center (PTC) in our state, and we are developing a pediatric trauma telehealth network to help keep certain injured children closer to home. We instituted a pediatric trauma telehealth program with a partnering community-based hospital in our state and aim to report our experience over the first year. METHODS: All pediatric trauma patients that presented to our partnering hospital from Januar

Classification of tibial plateau fractures using 3DCT with and without subtraction of unfractured bones.

BACKGROUND: The addition of 3D CT scans for observers evaluating tibial plateau fractures decreases agreement and diagnostic accuracy, possibly due to obstruction of the view by unfractured bones (i.e., patella, fibula, and femur) in 3D reconstructions. QUESTIONS/PURPOSES: This study compared (1) the interobserver agreement between (subgroups of) observers classifying 3DCT reconstructions of tibial plateau fractures according to Schatzker, Luo, and AO/OTA, (2) recognition of different fracture characteristics, and (3) the level of confidence on a scale from 0 to 10 with and without subtractio

[Not Available].

Penetrating neck injuries (PNIs) as a result of stabbing or deliberate self-harm are complex and potentially life-threatening. Nowadays, selective non-operative management of PNI has become common practice. Diagnostic and treatment algorithms originating from high-volume trauma centres in South-Africa and North-America are used in Dutch clinical practice. Three patients that sustained a PNI are discussed. Two patients, aged 61 and 37, only had mild signs on physical examination that justified additional diagnostic investigations. In the first patient, a penetrating oesophageal injury was found

The "found down" patient: a diagnostic dilemma.

BACKGROUND: "Found down" patients present to the emergency department (ED) after being discovered unconscious and are selected for trauma or medical evaluation based on ED triage. Occult injury is an important part of the differential diagnosis in these patients. Rational use of trauma resources and optimal care of these patients requires clear triage criteria and timely evaluation. METHODS: After an institutional review board approval was obtained, we retrospectively identified 201 "found down" patients from ED triage logs at an urban Level I trauma center between 2007 and 2011. Physician re

Leg, ankle, and foot injuries.

The emergency provider (EP) must be aware of the anatomy of the leg, ankle, and foot. The varied presentation of common injuries must be recognized as well as the unique presentations of uncommon injuries. The astute EP must rely on a focused history and a precise examination to avoid the pitfalls and missed injuries from an over-reliance on radiographic studies. In some cases, emergent orthopedic consultation is required. Potential complications associated with these injuries must be anticipated and avoided if possible.

Case report: acute management of external laryngeal trauma.

External laryngeal trauma is rare, accounting for less than 1% of all trauma cases seen at major centers. We report the case of a man who experienced multiple injuries, including an external laryngeal trauma. The primary signs and symptoms of his laryngeal trauma were hoarseness, hemoptysis, the loss of his laryngeal prominence (Adam's apple), neck tenderness, traumatic emphysema in the neck, and a small penetrating wound to the right of the laryngeal prominence. The patient underwent immediate tracheostomy and surgical exploration. On long-term followup, his voice quality and airway patency i

Initial Calcium Derangements in Major Trauma and Outcomes.

IMPORTANCE: Calcium derangements frequently occur in the setting of transfusions, but limited data suggest that derangements may occur from the trauma itself. Existing data suggest worse outcomes for trauma patients who present with calcium derangements. OBJECTIVE: To determine the incidence of calcium derangements upon emergency department (ED) arrival after major trauma and associated outcomes. DESIGN, SETTING, AND PARTICIPANTS: This prospective, multicenter cohort study was conducted from 2022 to 2024 at 3 American College of Surgeons-verified level I trauma centers. The Australian Trauma Registry (ATR): a leading clinical quality registry.

Operating since 2012 under the auspices of the Australian Trauma Quality Improvement Program (AusTQIP), the Australian Trauma Registry (ATR) has established itself as a leading clinical quality registry (CQR). Initially developed as a national program for improved safety and quality trauma care across Australian trauma centers, it has since expanded to include New Zealand, becoming one of the few bi-national trauma registries. The registry has recorded close to 100,000 episodes of care for severely injured patients since its inception, with 10.7% growth in annual inclusions. The ATR, administe…

Blunt traumatic lung injuries.

Approximately one third of patients admitted to major trauma centers in the United States sustain serious injuries to the chest. The lungs, which occupy a large portion of the chest cavity and lie in close proximity to the bony thorax, are injured in the majority of these patients directly or indirectly. A significant number of lung injuries are also associated with trauma to other critical thoracic structures. This article discusses blunt trauma injuries of the lung, which include pulmonary contusions, hematomas, lacerations, and pulmonary vascular injuries.

Arterial Bullet Embolization Secondary to Direct Cardiac Injury in Penetrating Chest Trauma: A Rare Challenging Case Report.

Severe thoracic injury secondary to penetrating trauma requires prompt resources and rapid decision-making by trauma centers and teams. Implementing trauma systems has significantly impacted medical and critical care quality and outcomes, including managing rare trauma injuries. We describe a report of a rare case of a 21-year-old man with a gunshot wound to the chest with injuries to the right pulmonary hilum requiring pneumonectomy and to the left atrium with bullet embolism to the right common iliac artery. In addition, the systematic approach where each phase of the individual's treatment …

Evidence based management of popliteal vessel injuries: A critical review, updates and controversies in the management of a difficult injury.

Popliteal artery injuries are rare even in urban Trauma Centers. The vast majority occur secondary to penetrating trauma. These injuries are uncommon; therefore, few Trauma Surgeons and Trauma Centers have developed significant experience with their management. Experiences reported in both military and civilian publications consistently report the highest complications and amputation rates of all vascular injuries due to popliteal vessel injuries. The popliteal artery is an end artery. Injuries cause significant ischemia that significantly threaten limb viability. The popliteal vein provides t…

Evaluating the drug prescribing pattern in a tertiary care trauma centre outpatient department in North India: A cross-sectional study.

PURPOSE: There is a need for rational prescribing of drugs to achieve healthcare outcomes. One of the major consequences of irrational drug use is the emergence of antibiotic resistance. Therefore, drug prescribing practices must be scrutinized at regular intervals to ensure patient safety. In this study, we aimed to evaluate the prescribing pattern of physicians using the 'WHO-recommended core prescribing indicators' in the OPD of a tertiary care trauma centre in North India. MATERIALS AND METHODS: This was a prospective, cross-sectional study conducted at all the outpatient departments of a
